 WE HAVE REVIEWED YOUR ENGINEERING DEVELOPMENT
 APPLICATION AND NEED MORE INFORMATION BEFORE WE CAN
 APPROVE THE PLANS.
  
 1. PLEASE SUBMIT A COPY OF THE ENGINEERING DEVELOPMENT
 APPLICATION WHICH HAS BEEN SIGNED BY THE OWNER.
  
 2. PLEASE SUBMIT THE ENGINEERING DEVELOPMENT REVIEW
 FEE, WHICH IS CALCULATED ON PAGE 10 OF THE CERTIFIED
 COST ESTIMATE FORM. ONCE YOU SUBMIT A REVISED FORM (SEE
 COMMENTS 3 AND 4 BELOW), WE CAN EMAIL YOU AN INVOICE.
  
 3. THE SUBMITTED CERTIFIED COST ESTIMATE FORM DID NOT
 INDICATE WHICH LINE ITEMS WOULD BE COVERED UNDER
 BUILDING PERMITS. TYPICALLY, A PAVING PERMIT AND
 UNDERGROUND FIRELINE PERMIT WILL ALSO BE REQUIRED FOR
 PROJECTS OF THIS NATURE. PUTTING A "Y" IN THE GRAY
 COLUMN ON THE COST ESTIMATE FORM WILL LOWER THE
 CALCULATED ENGINEERING DEVELOPMENT REVIEW FEE.
  
 4. PLEASE SUBMIT A STORMWATER POLLUTANT PROTECTION PLAN
 AND INCLUDE THE QUANTITIES IN THE CERTIFIED COST
 ESTIMATE FORM.
  
 5. PLEASE SUBMIT ESTIMATES FOR THE PEAK POTABLE WATER
 DEMAND, SANITARY SEWER FLOW, AND SOLID WASTE COLLECTION
 YARDAGE.
  
 6. HAS COUNTY APPROVAL BEEN RECEIVED FOR THE PROPOSED
 CONSTRUCTION ENTRANCE OFF OF HAVERHILL ROAD?
  
 7. ON SHEET C103 OF THE TOWER PHASE ONE PLANS, THE 12"
 TO 15" STORM LINE EAST OF THE POOL IS NOT SHOW. PLEASE
 REVIEW SHEET P100B OF BUILDING PERMIT 14120909/
 14120908 FOR THE LOCATION AND SIZE OF THE LINE.
  
 8. PLEASE REVIEW THE AREA NEAR THE NORTHWEST CORNER OF
 THE PROPOSED BUILDING ON SHEET C107 OF THE TOWER PHASE
 ONE PLANS. THERE IS AN ADDITIONAL SANITARY LINE SHOWN
 ON THE ARCHITECTURAL PLANS FOR THE DINING ROOM
 EXPANSION NEAR THE EASTERN DOUBLE DOORS WHICH IS NOT
 SHOWN ON SHEET C107. IT IS DIFFICULT TO DIFFERENTIATE
 BETWEEN THE EXISTING, PROPOSED, AND TO BE REMOVED
 UTILITY LINES. IT APPEARS THAT THE NOTE LEADERS POINT
 TO WATER LINES INSTEAD OF FIRE LINES IN A FEW
 LOCATIONS. IT ALSO APPEARS THAT THE FDC'S ARE NOT
 CONNECTED TO FIRE (OR WATER) LINES.
  
 NOTE:
 APPLICANTS FOR DEMOLITION AND/OR CONSTRUCTION PERMIT
 SHALL SUBMIT A BOND OR DEPOSIT IN AN AMOUNT COVERING
 THE COST ASSOCIATED WITH IMPLEMENTATION OF THE SWPPP
 AND LAND STABILIZATION OF THE SITE. THE EROSION CONTROL
 BOND AMOUNT IS TYPICALLY THE TOTAL OF THE NPDES ITEMS
 LISTED ON THE CERTIFIED COST ESTIMATE FORM. THE LAND
 STABILIZATION BOND IS TYPICALLY THE COST OF SODDING OR
 SEEDING THE AREA DISTURBED DURING CONSTRUCTION. THE
 BOND(S) WILL BE RELEASED UPON APPROVED INSPECTION OF
 TIE-BEAMS OR SIMILAR VERTICAL CONSTRUCTION. (CITY CODE
 SECTION 94-450) THE BONDS WILL BE DUE AT THE
 PRE-CONSTRUCTION MEETING.
