 PACKET, THE PROPOSED FUTURE LAND USE MAP AMENDMENT
 (FLUMA) APPLICATION DOES NOT IMPACT THE PARCEL LOCATED
 AT 2100 S. DIXIE HIGHWAY, JUST SOUTH OF BARCELONA ROAD
 (PCN: 74-43-43-27-21-002-0010). ONLY TWO PARCELS (2000
 AND 2020 SOUTH DIXIE HWY) ARE BEING CONSIDERED FOR THE
 FLUMA. THE INFORMATION PROVIDED IN THE JUSTIFICATION
 LETTER NEEDS TO BE CORRECTED (INCLUDING ACREAGE) AND
 SIMPLIFIED TO ADDRESS ONLY THE PARCELS IMPACTED, SO AS
 NOT TO CAUSE ANY CONFUSION (HISTORY ON THE PROJECT CAN
 BE INCLUDED IN THE JUSTIFICATION STATEMENT; HOWEVER,
 THE PURPOSE OF THE APPLICATION NEEDS TO BE CLEAR).
  
 2.) THE DEVELOPMENT APPLICATION NEEDS TO BE REVISED TO
 REFLECT ONLY THOSE PARCELS IMPACTED BY THE FLUMA. THE
 ORIGINAL APPLICATION REFERENCES PARCELS THAT ARE NOT
 APPLICABLE TO THE FLUMA. PLEASE ADDRESS.
  
 3.) THE CALCULATIONS PROVIDED IN THE LEVEL OF SERVICE
 (LOS) ANALYSIS SUMMARY TABLE SEEMS TO BE INCORRECT.
 PLEASE REVIEW CALCULATIONS AND CORRECT. (I.E. A
 RESIDENTIAL LAND USE USES THE NUMBER OF DWELLING UNITS
 (NOT FAR) TO CALCULATE THE RESULTING IMPACTS FOR THE
 PUBLIC FACILITIES).
  
 4.) A FUTURE LAND USE MAP AMENDMENT (FLUMA) TRAFFIC
 STUDY IS REQUIRED. THE STUDY SHOULD CONTAIN A DETAILED
 ANALYSIS COMPARING THE MAXIMUM POTENTIAL IMPACTS FOR
 THE EXISTING AND PROPOSED LAND USE FOR THE PARCELS
 AFFECTED BY THE PROPOSED LAND USE CHANGE ONLY. THE
 TRAFFIC IMPACT STATEMENT (PREPARED BY SIMMONS & WHITE,
 AND SEALED BY BRYAN KELLEY, P.E. ON 6/3/16) IS OUTDATED
 AND PROVIDES AN ANALYSIS FOR A PROPOSED DEVELOPMENT
 PROJECT, WHICH INCLUDES ADDITIONAL PARCELS NOT RELATED
 TO THE FLUMA.
  
 5.) SINCE THIS IS A SINGLE FUTURE LAND USE MAP
 AMENDMENT APPLICATION, THE LOS ANALYSIS SHOULD ALSO
 INCLUDE A TABLE CONSOLIDATING THE IMPACTS FROM THE
 THREE SEPARATE LOS TABLES FOR EACH TYPE OF FACILITY.
  
 6.) THE LOS ANALYSIS SHOULD INCLUDE STATEMENTS
 SUMMARIZING AND EXPLAINING THE IMPACTS FOR EACH
 FACILITY (PROVIDING JUST THE LOS ANALYSIS SUMMARY TABLE
 IS INSUFFICIENT).
  
 7.) STATEMENTS FROM EACH SERVICE PROVIDER SHALL BE
 SUBMITTED, CONFIRMING THAT THERE IS SUFFICIENT CAPACITY
 TO ACCOMMODATE THE PROPOSED LAND USE CHANGE. THE
 STATEMENTS SHOULD INCLUDE CAPACITY DATA.
  
 8.) PLEASE NOTE THAT SOLID WASTE SERVICE IS PROVIDED BY
 THE SOLID WASTE AUTHORITY (NOT THE CITY OF WEST PALM
 BEACH), AND THAT "SCHOOL CONCURRENCY" IS NO LONGER
 RELEVANT (WORDING SHOULD BE CHANGED TO SCHOOL CAPACITY
 AVAILABILITY DETERMINATION (SCAD) IN THE LOS SUMMARY
 TABLES)
  
 9.) THE CALCULATIONS FOR THE BASE LAND USE SQUARE
 FOOTAGES SEEM TO BE INACCURATE. PLEASE EXPLAIN HOW THE
 SQUARE FOOTAGES WERE CALCULATED.
  
 10.) AN UPDATED ADDRESS LIST WAS PROVIDED WITH THE
 4/5/18 RESUBMITTAL; HOWEVER, NO ENVELOPES WERE INCLUDED
 IN THE PACKET. SINCE THERE ARE OVER 50 MAILING
 ADDRESSES LISTED, THE APPLICANT WILL BE RESPONSIBLE FOR
 MAILING ALL REQUIRED NOTICES, AND REQUIRED TO PROVIDE A
 NOTARIZED AFFIDAVIT TO STAFF FOR EACH PUBLIC HEARING AS
 THE APPLICATION PROCEEDS THROUGH THE PUBLIC HEARING
 PROCESS. INSTRUCTIONS FOR THE ENVELOPES IS PROVIDED ON
 PAGES 4 AND 5 OF THE 2018 DEVELOPMENT APPLICATION.

 WRITTEN FORMAT.
  
 * WHEN RESUBMITTING, PLEASE PROVIDE A MINIMUM OF

 NOTE THAT CHANGES ON THE RESUBMITTED PLANS MAY RESULT
 IN ADDITIONAL COMMENTS.
