 THE APPLICANT MUST FILE RELEVANT JUSTIFICATION,
 INCLUDING, BUT NOT LIMITED TO, AN AFFIDAVIT BY A
 LICENSED ENGINEER WITH DOCUMENTED EXPERTISE IN RADIO
 FREQUENCY PROPAGATION, DEMONSTRATING THAT DESPITE

 CONSIDERED A CONCEALED ATTACHED ANTENNA AND SUBJECT TO
 THE LEVEL II REVIEW. HOWEVER, REPLACEMENT OF THE POLE
 FOR THE ACCOMMODATION OF A WIRELESS FACILITY, INCLUDING
 THE PROPOSED ANTENNA CANISTERS, IS CONSIDERED A
 CONCEALED FREESTANDING TOWER AND SUBJECT TO THE LEVEL
 III REVIEW. THE APPLICATION AS PROPOSED IS CLASSIFIED
 AS A LEVEL III REVIEW AND ADDITIONAL SUBMITTAL ITEMS

 RESIDENTIAL PLANNED DEVELOPMENT (RPD) ? ORDINANCE NO.
 2608-93. THE SETBACKS FOR LOT 11 ARE AS FOLLOWS: FRONT
 ? 20 FEET, SIDE ? 10 FEET, REAR ? 20 FEET. MECHANICAL
 EQUIPMENT MAY ENCROACH UP TO 4 FEET INTO A REQUIRED
 SETBACK (ZLDR SEC. 94-305(4)). PLEASE INDICATE SETBACKS
 ON THE SITE PLAN TO INDICATE COMPLIANCE (ZLDR SEC.
 94-325(A)(5)).
  
 B. EASEMENT CONSENT FORM WILL BE REQUIRED FOR GROUND

 E. REVIEW RESPONSE TO STANDARDS FOR CODE COMPLIANCE.
 RESPONSES INDICATE THAT EXISTING LANDSCAPING WILL
 CONCEAL THE CABINETS; HOWEVER, THERE IS NO LANDSCAPING
 ALONG THE SOUTHERN PROPERTY LINE AND THE SITE IS
 COMPLETELY VISIBLE FROM THE STREET. PLEASE REVIEW
 RESPONSES TO STANDARDS.
