 FOLLOWING:
  
 1.) PROVIDE A DESCRIPTION (IN NARRATIVE FORMAT) FOR THE
 AVAILABILITY OF AND THE DEMAND ON THE FOLLOWING
 MANDATORY PUBLIC FACILITIES: POTABLE WATER, SANITARY
 SEWER, SOLID WASTE, DRAINAGE, TRAFFIC, SCHOOLS AND
 PARKS/RECREATION. THE ANALYSIS MUST INCLUDE DATA
 ENSURING THAT THE ADOPTED LEVEL OF SERVICE STANDARDS
 ARE ACHIEVED AND WILL BE MAINTAINED. MAXIMUM DENSITIES
 AND INTENSITIES FOR THE EXISTING AND PROPOSED FUTURE
 LAND USE DESIGNATIONS SHALL BE USED IN THE ANALYSIS FOR
 EACH PUBLIC FACILITY.
  
 2.) PROVIDE WRITTEN STATEMENTS FROM ALL SERVICE
 PROVIDERS VERIFYING THAT THERE IS SUFFICIENT CAPACITY
 TO MEET THE DEMAND OF THE PROPOSED LAND USE CHANGE.
  
 3.) THE APPLICATION INCLUDES A LOS ANALYSIS TABLE.
 PLEASE NOTE THAT THIS TABLE IS SUPPOSED TO BE A SUMMARY
 TABLE OF THE INFRASTRUCTURE LOS STATEMENTS THAT NEED TO
 BE PROVIDED IN THE APPLICATION.
  
 4.) THE APPLICANT'S COMPREHENSIVE PLAN AMENDMENT
 TRAFFIC ANALYSIS PREPARED BY PINDER TROUTMAN
 CONSULTING, INDICATES THAT THE PROPOSED LAND USE
 DESIGNATION FOR THIS AMENDMENT IS MULTIFAMILY MEDIUM
 DENSITY (MF20) AND THAT THE MAXIMUM DEVELOPMENT
 POTENTIAL FOR THIS DESIGNATION IS 20.26 DWELLING UNITS
 PER ACRE FOR A TOTAL OF 249 DWELLING UNITS IN THE 12.29
 ACRE SITE. THE MULTIFAMILY MEDIUM DENSITY DESIGNATION
 SHOULD BE INDICATED AS MFMD AND THE MAXIMUM DEVELOPMENT
 POTENTIAL FOR THIS DESIGNATION IS 20 UNITS PER ACRE
 (NOT 20.26 UNITS PER ACRE) RESULTING IN A TOTAL OF 245
 UNITS BEING ALLOWED FOR THIS SITE. THE TRAFFIC ANALYSIS
 SHOULD BE REVISED TO REFLECT THE MAXIMUM DEVELOPMENT
 POTENTIAL OF 245 UNITS, INSTEAD OF THE 249 UNITS
 REFLECTED IN THE CURRENT ANALYSIS.
  
 5.) THERE APPEARS TO BE A DISCREPANCY BETWEEN THE
 TRAFFIC IMPACTS REFLECTED IN THE LOS ANALYSIS TABLE AND
 THE IMPACTS REFLECTED IN THE COMPREHENSIVE PLAN
 AMENDMENT TRAFFIC ANALYSIS. THIS DISCREPANCY NEEDS TO
 BE RECONCILED.
  
 6.) CONFIRM WHICH MUNICIPALITY WILL BE THE PROVIDER OF
 WATER AND SEWER FOR THIS SITE. THE ANALYSIS FOR WATER
 AND SEWER SHOULD REFLECT THE LOS STANDARDS FOR THAT
 PROVIDER.
  
 7.) PROVIDE A LETTER FROM THE SCHOOL DISTRICT OF PALM
 BEACH COUNTY ESTABLISHING SCHOOL CONCURRENCY FOR THE
 PROPOSED CHANGE IN LAND USE TO RESIDENTIAL.
  
 8.) PROVIDE WRITTEN CONFIRMATION THAT THE PUBLIC
 UTILITIES DEPARTMENT HAS REVIEWED THE PROPOSAL FOR
 WATER, SEWAGE TREATMENT, STORM DRAINAGE AND GARBAGE
 COLLECTION SERVICES.
  
 9.) PROVIDE LETTERS FROM THE FOLLOWING PUBLIC UTILITIES
 CONFIRMING THAT THE PROPOSED PROJECT DOES NOT ENCROACH
 INTO THEIR EASEMENTS OR OTHERWISE INTERFERE WITH THE
 PROVISIONS OF THEIR SERVICES: AT&T, FPU.

 PLANNING DIVISION MAY TAKE NO FURTHER ACTION ON THE
