 1. PLEASE PROVIDE THE DISTANCE FROM THE END OF THE
 FURTHERMOST STRUCTURE TO THE NAVIGABLE CHANNEL TO
 ENSURE THAT THE PROPOSED DOCK WILL NOT CONSTITUTE A
 HAZARD TO NAVIGATION.
 2. A COPY OF THE WARRANTY DEED SHALL BE PROVIDED.
 3. A MAILING LIST CONTAINING THE NAMES AND MAILING
 ADDRESSES, AND A MAP, OF ALL PROPERTY OWNERS WITHIN 400
 FEET OF THE SUBJECT PROPERTY SHALL BE PROVIDED FROM THE
 PALM BEACH COUNTY PROPERTY APPRAISER TO ENSURE THAT THE
 MAILING INFORMATION IS REFLECTIVE OF THE CURRENT TAX
 ROLES. THE TWO (2) SETS OF ENVELOPES SHOULD REFLECT
 THESE MAILING ADDRESSES. ADDITIONALLY, THE AFFIDAVIT
 FROM PAGE 20 OF THE DEVELOPMENT APPLICATION SHALL BE
 PROVIDED.
 4. WRITTEN APPROVAL FROM THE FLORIDA DEPARTMENT OF
 ENVIRONMENTAL PROTECTION (FDEP) AND/OR U.S. ARMY CORPS
 OF ENGINEERS.
 5. STUDIES SHALL BE PROVIDED INDICATING THAT NO MARINE
 GRASSES, ETHNIC COMMUNITIES, AND IMPORTANT ESTUARINE OR
 ECOLOGICAL FEATURES WILL BE AFFECTED BY THE PROPOSED
 CONSTRUCTION.
 6. THE CITY SERVICE AND PUBLIC UTILITY LETTERS SHALL BE
 PROVIDED.
 7. RESPONSES TO THE STANDARDS FOR CODE COMPLIANCE SHALL
 BE PROVIDED. PLEASE REFER TO PAGE 29 OF THE CITY OF
 WEST PALM BEACH DEVELOPMENT APPLICATION.
 8. A TITLE COMMITMENT SHALL BE PROVIDED FROM A TITLE
 COMPANY. THE TITLE COMMITMENT SHALL CONFIRM THAT THE
 PROPERTY DOES HAVE RIPARIAN RIGHTS IN THE LOCATION OF
 THE PROPOSED DOCK. DOCUMENTATION FROM AN ATTORNEY SHALL
 ALSO BE PROVIDED CONFIRMING/SUPPORTING THE TITLE
 COMMITMENT AND THE OWNERSHIP OF THE RIPARIAN RIGHTS.
 9. PAYMENT FOR ADVERTISING ($216) AND ENGINEERING
 SERVICES ($424) FEES SHALL BE PROVIDED.
 10. BASED UPON A BRIEF REVIEW OF THE PLANS, IT APPEARS
 THAT SOME OF THE PILINGS ARE PROPOSED OUTSIDE OF THE
 MIDDLE 1/3 OF THE PROPERTY. IF THAT IS THE CASE, AND A
 WAIVER IS BEING REQUIRED, THEN PAYMENT SHALL BE
 SUBMITTED FOR SUCH WAIVER ($300) AND THE JUSTIFICATION
 STATEMENT SHALL BE UPDATED TO REFLECT THIS REQUEST.
