Project Description PBC - CHILLER BUILDING TREE ALTERATION PERMIT PERMIT NO.: TP 10-1165 LICENSEE: Palm Beach County, Audrey Wolf, Director, FD&O 2633 Vista Parkway West Palm Beach, Florida 33411 AGENT: Melanie Borkowski, Facilities Compliance, FD&O 2633 Vista Parkway West Palm Beach, Florida 33411 Description of Work: Relocation of thirteen (13) trees and removal o f fourteen (14) trees Location of Work: 301 North Olive Avenue & 215 North Olive Avenu e, West Palm Beach Landscape Plan: Sheets LP-1 - LP-3 with the last revision date of 3/13/10, were prepared by Gentile, Holloway, O???Mahoney & Associates , Inc. located at 1907 Commerce Lane, Suite 101, Jupiter, Florida 3345 8. They were signed and sealed by Ben Dolan, a registered landscape a rchitect. Conditions: 1. This tree alteration permit shall only be applicable for the reloca tion of thirteen (13) trees and the removal of fourteen (14) trees. 2. The relocation/replacement of trees shall be completed prior to the issuance of the Certificate of Occupation or Completion. A landscape inspection shall be requested from the Planning and Zoning Department subsequent to planting the trees. 3. The Licensee shall monitor the condition of the relocated tree for a period of one (1) year after installation. Should the tree fail to become established, it shall be replaced according to the tree replace ment requirements listed in Article XIV, Section 94-448(e)(2), of the Zoning and Land Development Regulations. 4. The Planning and Zoning Department may amend or extend the time fra mes contained in this Tree Alteration Permit, if the licensee has show n good faith in completing the relocation of the trees and has request ed the amendment or extension prior to the expiration of the license. Expired permits are not renewable. 5. All existing trees, plants, ground cover and sod surrounding this a rea of modification shall be in good condition at the time of final la ndscape inspection and shall meet all minimum requirements pursuant to Article XIV of the City of West Palm Beach Zoning and Land Developmen t Regulations. The irrigation system shall be completely functional a nd shall provide 100% coverage to all landscaped areas, including the right-of-way. 6. All existing trees to remain or to be relocated shall be corrective ly pruned by a Certified Arborist. 7. All future tree pruning done on site shall be performed in accordan ce with the American National Standard (ANSI) A300-2001. Palms will b e pruned leaving fronds from the 9:00 to 3:00 o???clock positions. 8. All trees and other plant material installed, replaced or relocated pursuant to Section 94-448(e) (2) of the City of West Palm Beach Code of Ordinances must be in a healthy, living condition one year from th e date of planting. 9. No grade changes are to be made beneath the drip line of preserved trees. 10. Trees shall be planted slightly above grade in holes three (3) tim es the diameter of the root ball at the surface and two (2) times the diameter of the root ball at the bottom. Trees shall be surrounded by a three (3) inch deep bed of mulch starting six (6) inches away from the trunk and extending out to a two (2) foot radius from the trunk. The trunk flare or top-most root originating from the trunk shall be v isible at the surface. 11. Temporary supports shall be removed after trees have become establ ished. 12. The licensee, by acceptance of this license, specifically agrees t o allow access at reasonable times by the Planning and Zoning Departme nt personnel for the purpose of inspection to determine compliance wit h this license. Expired licenses are not renewable. 13. The licensee shall only remove those trees so specified in the Lic ense. Any removal or damage to any other tree on the site shall const itute a violation. 14. A copy of this license and all related plans shall be kept on site during tree removal activities. 15. The applicant shall contact the Landscape Planner at: (561) 822-14 62, to schedule a final tree alteration inspection upon completion of work.
