 DENIED BY BUILDING
 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THIS PROJECT INVOLVES STRUCTURAL MODIFICATIONS TO A
 THRESHOLD BUILDING. A THRESHOLD INSPECTOR WILL BE
 REQUIRED. PLEASE ADDRESS THE FOLLOWING:
  
 THRESHOLD BUILDING / FL S. S. 553.71(7) "THRESHOLD
 BUILDING" MEANING ANY BUILDING WHICH IS GREATER THAN
 (3) STORIES OR 50 FT. IN HEIGHT, OR WHICH HAS AN
 ASSEMBLY OCCUPANCY CLASSIFICATION AS DEFINED IN THE
 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN
 AREA AND AN OCCUPANT CONTENT OF GREATER THAN 500
 PERSONS.
  
 A) FBC (CITY AMENDMENTS)110.8.1 THRESHOLD INSPECTION
 PLAN. THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OF RECORD. THE STRUCTURAL
 INSPECTION PLAN MUST BE SUBMITTED TO THE ENFORCING
 AGENCY PRIOR TO THE ISSUANCE OF A BUILDING PERMIT FOR
 THE CONSTRUCTION OF A THRESHOLD BUILDING. THE PURPOSE
 OF THE STRUCTURAL INSPECTION PLAN IS TO PROVIDE
 SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO THAT
 THE BUILDING CAN BE ADEQUATELY INSPECTED FOR COMPLIANCE
 WITH THE PERMITTED DOCUMENTS.
  
 B) THRESHOLD INSPECTOR: WPB AMENDMENT 110.8.1 W.P.B.
 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE.
  
 C) THRESHOLD BLDG. REQUIRED STATEMENT: W.P.B. AMENDMENT
 110.8.4.4 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE MINIMUM
 BUILDING CODES AND THE APPLICABLE FIRE-SAFETY STANDARDS
 AS DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND CHAPTER 633 FLORIDA STATUTES.
  
 D) FOR THE SPECIAL INSPECTOR PLEASE PROVIDE A RESUME OF
 INSPECTION EXPERIENCE ON PREVIOUS PROJECT. THE RESUME
 MUST BE ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED
 HEREIN. THE BUILDING OFFICIAL OR HIS DESIGNEE WILL
 REVIEW THE RESUME. AFTER THE RESUME IS REVIEWED, AN
 INTERVIEW WILL BE SCHEDULED. UNDER NO CIRCUMSTANCES
 WILL AN INSPECTOR BE INTERVIEWED FOR WORK WITHOUT FIRST
 MEETING THE ABOVE CRITERIA.
  
 E) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR HISTORY
 FOR THIS TYPE OF CONSTRUCTION.
  
 F) NOTE: FOR DIGITAL PLANS, AFTER PERMIT ISSUANCE,
 PLEASE E-MAIL THE PERMIT LIBRARIAN OF THE PERMIT BEING
 ISSUED, THE PERMIT LIBRARIAN WILL THEN E-MAIL YOU THE
 APPROVED AND BATCH STAMPED PLANS SO YOU MAY PRINT YOUR
 PLANS, TWO SETS OF PLANS WILL NEED TO BE PRINTED IN
 COLOR, (1) FOR THE THRESHOLD INSPECTOR, THE OTHER FOR
 THE MEP INSPECTORS OTHER PLANS YOU WISH TO PRINT MAYBE
 IN BLACK & WHITE.
  
 G) 110.8.4.3 ALL SHORING AND RESHORING PROCEDURES,
 PLANS AND DETAILS SHALL BE SUBMITTED TO THE ENFORCEMENT
 AGENCY FOR RECORDKEEPING. EACH SHORING AND RESHORING
 INSTALLATION SHALL BE SUPERVISED, INSPECTED AND
 CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.
  
 H) PLEASE CALL THE CITY OF WEST PALM BEACH'S AUTOMATED
 TELEPHONE INSPECTIONS LINE (561) 805-6700 FOR ALL
 BUILDING INSPECTIONS FOR AUDITING OF JOBSITE
 INSPECTIONS WITHOUT THIS THE PERMIT EXPIRES IN 6
 MONTHS, ABANDONMENT OF JOB, NO INSPECTION ACTIVITY.
  
 I) 110.8 THRESHOLD BUILDINGS, OPERATING GUIDELINES FOR
 THRESHOLD INSPECTORS #25. ASSURE THAT THE DESIGNER HAS
 REQUIRED ON THE PLANS OR SPECIFICATIONS ONE MATERIAL,
 APPROVED FOR FIRE STOPPING PENETRATIONS, FOR WALLS AND
 CEILINGS.
  
 J) PLEASE CONTACT ME WITH AN EMAIL ADDRESS AND I WILL
 BE HAPPY TO SEND YOU A COPY OF THE CURRENT (OCTOBER
 2018) GUIDELINES AND POLICIES FOR PROJECTS CONCERNING
 THRESHOLD/SPECIAL INSPECTORS AND RESIDENT INSPECTORS.
  
 2) PROVIDE A COMPLETE FLOOR PLAN OF THE BUILDING FOR
 EACH FLOOR OF THE PROJECT AND IDENTIFY THE WORK AREA ON
 EACH FLOOR. FBC 107.2.1
  
 3) PROVIDE A COVER PAGE WITH THE DESIGN PARAMETERS AND
 RELAVENT CODES (FBC EXISTING BUILDING AND OTHERS). THIS
 INCLUDES DESIGN CRITERIA, LEVEL OF ALTERATION, AND CODE
 ANALYSIS. FBC 107.2.1
  
 4) YOU MAY WISH TO DEFER SUBMITTING SOME CONSTRUCTION
 DOCUMENTS (SHOP DRAWINGS, PRODUCT APPROVALS, ETC.)
 UNTIL A LATER TIME PRIOR TO INSTALLATION. FBC
 AMENDMENTS 107.3.4.1 DEFERRED SUBMITTALS. FOR THE
 PURPOSES OF THIS SECTION, DEFERRED SUBMITTALS ARE
 DEFINED AS THOSE PORTIONS OF THE DESIGN THAT ARE NOT
 SUBMITTED AT THE TIME OF THE APPLICATION AND THAT ARE
 TO BE SUBMITTED TO THE BUILDING OFFICIAL WITHIN A
 SPECIFIED PERIOD. DEFERRAL OF ANY SUBMITTAL ITEMS SHALL
 HAVE THE PRIOR APPROVAL OF THE BUILDING OFFICIAL. THE
 REGISTERED DESIGN PROFESSIONAL IN RESPONSIBLE CHARGE
 SHALL LIST THE DEFERRED SUBMITTALS ON THE CONSTRUCTION
 DOCUMENTS FOR REVIEW BY THE BUILDING OFFICIAL. THE
 REGISTERED DESIGN PROFESSIONAL IN RESPONSIBLE CHARGE
 SHALL LIST THE DEFERRED SUBMITTALS ON THE CONSTRUCTION
 DOCUMENTS FOR REVIEW BY THE BUILDING OFFICIAL.
 DOCUMENTS FOR DEFERRED SUBMITTAL ITEMS SHALL BE
 SUBMITTED TO THE REGISTERED DESIGN PROFESSIONAL IN
 RESPONSIBLE CHARGE WHO SHALL REVIEW THEM AND FORWARD
 THEM TO THE BUILDING OFFICIAL WITH A NOTATION
 INDICATING THAT THE DEFERRED SUBMITTAL DOCUMENTS HAVE
 BEEN REVIEWED AND FOUND TO BE IN GENERAL CONFORMANCE TO
 THE DESIGN OF THE BUILDING. THE DEFERRED SUBMITTAL
 ITEMS SHALL NOT BE INSTALLED UNTIL THE DEFERRED
 SUBMITTAL DOCUMENTS HAVE BEEN APPROVED BY THE BUILDING
 OFFICIAL.
  
 5) PLEASE PROVIDE A SIGNED ACKNOWLEDGEMENT FROM THE
 CONTRACTOR, ON LETTERHEAD, STATING THAT THE
 INSTRUCTIONS ON THE WEBSITE OF ASBESTOS PROGRAM
 COORDINATOR, FLORIDA DEPARTMENT OF HEALTH PALM BEACH
 COUNTY WILL BE FOLLOWED AND THAT NOTIFICATION WILL BE
 GIVEN TIMELY. ADDITIONAL INFORMATION REGARDING ASBESTOS
 REQUIREMENTS CAN BE FOUND ON THEIR WEBSITE: HTTP://PALM

 6) THE SCOPE OF WORK FOR THIS PROJECT WILL REQUIRE THE
 INSTALLATION OF SOME TYPE OF NEW FLOORING IN THE UNIT.
 SPECIFY THE TYPE OF FINISH FLOORING AND THE SOUND
 INSULATION UNDERLAYMENT THAT WILL BE INSTALLED. YOU
 WILL ALSO NEED TO SUBMIT COPIES OF THE ACTUAL TEST
 REPORTS FOR THE SOUND INSULATION INCLUDING FLAME SPREAD
 AND SMOKE DEVELOPED FOR THE APPLICABLE CONSTRUCTION
 CONDITIONS. FBC 603.1 FLAME SPREAD - ASTM E 84-09
 FBC 1207.2 AIR-BORNE SOUND. ASTM E 90-04 WALLS,
 PARTITIONS AND FLOOR/CEILING ASSEMBLIES SEPARATING
 DWELLING UNITS FROM EACH OTHER OR FROM PUBLIC OR
 SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION CLASS
 (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED) FOR
 AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM E
 90.
 FBC 1207.3 STRUCTURE-BORNE SOUND. ASTM E 492-09
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E 492.
  
 7) ON SHEET S100, THE FLOOR PENETRATION REPAIR DETAIL
 SPECIFIES PLYWOOD SHEATHING AND WOOD JOISTS. THIS IS
 NOT ALLOWED IN TYPE I AND II CONSTRUCTION. 718.5
 COMBUSTIBLE MATERIALS IN CONCEALED SPACES IN TYPE I OR
 II CONSTRUCTION. COMBUSTIBLE MATERIALS SHALL NOT BE
 PERMITTED IN CONCEALED SPACES OF BUILDINGS OF TYPE I OR
 II CONSTRUCTION.
  
 8) SHOW COMPLIANCE WITH FBC 1014.1 FOR HANDRAILS AT THE
 PROPOSED STAIRS.
  
 9) SHOW COMPLIANCE WITH FBC 1015.1 FOR GUARDS AT THE
 PROPOSED STAIRS.
  
 10) SHOW THAT THE REMODELED BATHROOMS AND KITCHENS
 COMPLY WITH FAIR HOUSING.
  
 11) FBC 107.3.4 - PROVIDE PRODUCT APPROVALS FOR THOSE
 PRODUCTS WHICH ARE REGULATED BY FAC 61G20-3.001 PER
 FLORIDA STATUTE 553.842 (5) THAT HAVE BEEN REVIEWED AND
 APPROVED IN WRITING BY THE DESIGNER OF RECORD. INDICATE
 THE SPECIFIC ITEMS AND THEIR DESIGN PRESSURES ON THE
 PRODUCT APPROVAL BY CIRCLING OR HI-LIGHTING THEM. THESE
 ITEMS CAN BE DEFERRED IF REQUESTED, HOWEVER, THE
 DOCUMENTS MUST BE SUBMITTED AND APPROVED BY THE
 BUILDING DEPARTMENT BEFORE THEY CAN BE INSTALLED.
  
 12) PLEASE SUBMIT A RESPONSE LETTER INDICATING HOW EACH
