 PLAN REVIEW RESULTS: DENIED.
  
 1) BASED ON THE SUBMITTED PHOTO OF THE SHOWER AND THE
 BATHROOM, THE LAYOUT PLAN NEEDS TO BE MODIFIED TO SHOW
 THE FULL SCOPE OF WORK WHICH INCLUDES DEMOLITION AND
 REMODELING TO ENLARGE THE SHOWER IN THE BATHROOM.
 PLEASE PROVIDE A DIMENSIONED PLAN THAT SHOWS THE
 EXISTING BATH AND THE REMODELED BATH- SEC. 107.2.1 WPB.
  
 2) FAIR HOUSING COMPLIANCE: SHOW THE REQUIRED
 30X48-INCH CLEAR FLOOR SPACE DIAGRAM POSITIONED FOR A
 PARALLEL APPROACH TO THE SHOWER- PAGES 7.56- 7.59 FHDA.
 PLEASE NOTE THAT NO FIXTURES ARE ALLOWED TO OVERLAP THE
 CLEAR FLOOR SPACES REQUIRED FOR THE SHOWER.
  
 3) FAIR HOUSING COMPLIANCE: REFER TO PAGES 6.12- 6.13
 OF THE FHDA MANUAL AND PROVIDE AN ELEVATION DETAIL
 SHOWING THE SHOWER CONTROLS LOCATION, AND THE GRAB BAR
 REINFORCEMENT DETAILS. SHOW THE LENGTHS, WIDTHS AND
 HEIGHTS AND THE TYPE OF REINFORCEMENT MATERIAL TO BE
 USED. PLEASE SEE SECTION 603.1 FOR ALLOWABLE MATERIALS
 IN NON-COMBUSTIBLE BUILDINGS.
  
 PLEASE NOTE: AS AN ALTERNATIVE TO MEETING THE
 GUIDELINES FOR THE BATH ALTERATIONS, THE OWNER AND
 DESIGNER OF RECORD FOR THE PROPOSED PROJECT MAY PROVIDE
 A SIGNED AND NOTARIZED AFFIDAVIT WHICH WOULD INDICATE
 ACKNOWLEDGEMENT THAT THE PROPOSED DESIGN DOES NOT MEET
 THE REQUIREMENTS OF THE FAIR HOUSING ACCESSIBILITY
 GUIDELINES, BUT WILL ALSO REQUIRE THE OWNER TO AGREE TO
 REVERT THE UNIT BACK TO COMPLIANCE AT TIME OF SALE IF
 REQUESTED BY THE BUYER. IF THIS OPTION IS CHOSEN,
 PLEASE SEND AN EMAIL TO [email protected] AND I WILL SEND A
 COPY OF THE FAIR HOUSING AFFIDAVIT.
  
 4) IT APPEARS THE SHOWER DRAIN MAY HAVE TO BE
 RELOCATRED. RELOCATING THE SHOWER DRAIN OUTLET MAY
 REQUIRE THE FLOOR TO BE X-RAYED TO IN ORDER TO LOCATE
 REINFORCING STEEL OR POST STRESSED FLOOR TENDONS.
 PLEASE PLACE A NOTE ON THE PLANS TO VERIFY COMPLIANCE
 WILL BE MET- SEC. 107.2.1 WPB.
  
 5) NEW DRAIN PIPE PENETRATIONS OF THE EXISTING SLAB
 WILL REQUIRE FIRESTOPPING TO SEAL THE ANNULAR SPACE
 AROUND THE PIPING. PROVIDE A UL TESTED AND APPROVED
 FIRESTOPPING ASSEMBLY DETAIL ON THE PLAN- SEC. 315.1
 FBC P AND SEC. 714.4.1.2 FBC B.
  
 6) THE SIGNATURE OF THE CONTRACTOR OR QUALIFIER SHALL
 BE ON THE DRAWINGS- SEC.107.2.1 WPB, AND FL. STATUTES
 471.003(2)(H) & 471.025
  
 WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A RESPONSE
 LETTER ADDRESSING EACH ITEM ALONG WITH THE CITY
 RE-SUBMITTAL FORM.
