 1) THE SUBMITTED COMMUNICATION FACILITY PLACEMENT
 PERMIT - CITY RIGHTS-OF-WAY (21120228) MUST BE APPROVED
 PRIOR TO THIS PERMIT BEING APPROVED. THIS SUBMITTED
 PERMIT HAS BEEN UPDATED SINCE THE WCF ROW PERMIT. BOTH
 PLANS WILL NEED TO REFLECT THE SAME, CODE COMPLIANT
 CONDITIONS. THE COMMENTS BELOW ALSO APPLY TO THE WCF
 ROW PERMIT.
  
 2) THIS LOCATION IS WITHIN A HISTORIC DISTRICT. PLEASE
 PROVIDE A COPY OF THE STATE HISTORIC PRESERVATION
 OFFICE APPROVAL OF THIS LOCATION. THE APPROVAL NEEDS TO
 BE THE OFFICIAL STATE APPROVAL ON THEIR LETTERHEAD, NOT
 A TABLE ON A SHEET WITH VERIZON'S LOGO ON THE TOP AS
 THE DOCUMENT PROVIDED IS.
  
 3) SHEET C-1, SITE PLAN WITH AERIAL, SHOWS THE POLE
 SOUTH OF THE HOUSE ON THE LOT WEST OF THE PROPOSED POLE
 LOCATION, WHICH COMPLIES WITH CODE. HOWEVER; THE
 OVERLAY LINES DO NOT ALIGN WITH THE BASE AERIAL. STAFF
 IS CONCERNED THAT ALIGNING THE OVERLAY LINES WITH THE
 BASE AERIAL WILL PUSH THE PROPOSED ANTENNA INTO THE
 FRONT OF THE HOUSE WHICH IS NOT PERMITTED. PLEASE MAKE
 SURE THAT THE OVERLAY LINES MATCH THE BASE AERIAL AND
 THAT THE PROPOSED POLE IS CLEARLY SHOWN SOUTH OF THE
 HOUSE. STAFF HAS PROVIDED CHRISTINA WARR WITH A SNIP OF
 THIS AREA.
  
 4) SHEET P-1 SHOWS THE EXCAVATION BOX FOR THE HANDHOLE
 ADJACENT TO THE UTILITY POLE PROVIDING THE POWER
 EXTENDING PAST THE EDGE OF THE SIDEWALK, WHICH PUTS IT
 ON TO PRIVATE PROPERTY, WHICH IS NOT PERMITTED. ALL
 WORK MUST BE ONLY IN THE RIGHT-OF-WAY. *** THIS WAS NOT
 UPDATED FROM THE WCF ROW PERMIT COMMENT.
  
 5) THE POLE STYLE AND HEIGHT MEET CITY REQUIREMENTS.
 THE POLE COLOR AT THIS LOCATION SHALL BE RAL/RAYCAP
 7042 TRAFFIC GREY A.
