 ADDRESS THE FOLLOWING COMMENTS AND RE-SUBMIT:
  
 NOTICE: ORIGINAL ROOF DESIGN UNDER MASTER PERMIT
 20111187 IS A WOOD DECK OVER MANUF. WOOD TRUSSES WITH
 R-20 ICYNENE SPRAY FOAM INSULATION.
 NOA19-0424.06 SPECIFIES POLYGLASS MODIFIED BITUMEN ROOF
 SYSTEM OVER LIGHTWEIGHT CONCRETE. IF THE ROOF DESIGN
 WILL BE CHANGED, PROVIDE A REVISION TO PLAN PAGE S-3
 UNDER MASTER PERMIT 20111187. DESIGNER SHALL
 RE-EVALUATE THE DEAD LOAD ON THE MANUF. WOOD TRUSSES.
  
 1. FL5259-R28 (UNDERLAYMENT) HAS EXPIRED.
 PROVIDE THE FL PRODUCT APPROVALS OR MIAMI-DADE NOA FOR
 THE PROPOSED ROOFING UNDERLAYMENT TO BE INSTALLED.
 FL PRODUCT APPROVALS SHALL INCLUDE THE COVER SHEETS,
 EVALUATION REPORT AND INSTALLATIONS INSTRUCTIONS.
 NOTE: NOA???S DO NOT HAVE COVER SHEETS.
 FBC 107.3.4 PROVIDE PRODUCT APPROVALS FOR THOSE
 PRODUCTS WHICH ARE REGULATED BY FAC RULE 9N-3 REVIEWED
 AND APPROVED IN WRITING BY THE DESIGNER OF RECORD.
