 PLAN REVIEW RESULTS: DENIED.
  
 1) PLUMBING PLANS SHALL BE DRAWN BY THE PLUMBING
 CONTRACTOR OF RECORD OR A FL. LICENSED ENGINEER-
 FLORIDA STATUTES 471.003(2) (H) & 471.015. CONTRACTOR
 PLANS SHALL BEAR THE PRINTED NAME, LICENSE NUMBER, AND
 SIGNATURE OF THE PLUMBING CONTRACTOR OF RECORD- SEC.
 107.2.1 WPB.
  
 2) THE PLUMBING PLANS SHALL INCLUDE A SCOPE OF WORK
 OUTLINE ITEMIZING ALL REPLACED FIXTURES AND LOCATIONS
 OF NEW, REPLACED, RELOCATED PLUMBING PIPING. PLUMBING
 RISER DRAWINGS ARE REQUIRED FOR ALL NEW WATER SUPPLY
 AND SANITARY DRAINAGE AND VENT PIPING INSTALLATIONS-
 SEC. 107.2.1 AND 107.3.5.3.13 WPB.
  
 3) FAIR HOUSING COMPLIANCE:
 WPB BUILDING DEPT. RECORDS INDICATE THE RESIDENTIAL
 UNITS AT TWO CITY PLAZA- 701 S OLIVE WERE DESIGNED AND
 CONSTRUCTED IN ACCORDANCE WITH THE FAIR HOUSING DESIGN
 ACT. THE PROPOSED KITCHEN AND BATH ALTERATIONS SHALL BE
 DESIGNED AND CONSTRUCTED IN ACCORDANCE WITH THE FHDA
 MANUAL OR THE FAIR HOUSING DESIGN GUIDELINES CONTAINED
 IN THE FBC ACC. THE DESIGNER SHALL INDICATE COMPLIANCE
 WITH SPECIFICATION TYPE A OR B AND THE DRAWINGS SHALL
 REFLECT THE SPECIFICATION. PLEASE REFER TO PAGES 7.34-
 7.39 OF THE FHDA MANUAL FOR EXPLANATORY INFORMATION.
 KITCHEN- REFER TO PAGE 7.5 OF THE FAIR HOUSING ACT
 DESIGN MANUAL AND REQUIREMENT 7 (1)(A) OF THE
 GUIDELINES AND SHOW THE REQUIRED 30X48 INCH CLEAR FLOOR
 SPACE DIAGRAM AND DIMENSIONS FOR A PARALLEL WHEELCHAIR
 APPROACH AT THE SINK, AND 30X48 INCH CLEAR FLOOR SPACE
 DIAGRAM AND DIMENSION FOR THE PARALLEL OR FORWARD
 APPROACH TO THE DISHWASHER. PLEASE NOTE THAT THE
 REQUIRED CLEAR FLOOR SPACES FOR PARALLEL APPROACHES
 SHALL BE CENTERED ON THE SINK AND DISHWASHER.

 APPLIANCES

 REINFORCEMENT DETAILS- PAGES- 6.12-6.13. SHOW THE
 LENGTHS, WIDTHS AND HEIGHTS AND THE TYPE OF
 REINFORCEMENT MATERIAL TO BE USED. SEE SEC 603.1 FBC B
 FOR ALLOWABLE MATERIALS IN NON-COMBUSTIBLE BUILDINGS.
  
 PLEASE NOTE: AS AN ALTERNATIVE TO MEETING THE
 GUIDELINES FOR THE BATH ALTERATIONS, THE OWNER AND
 DESIGNER OF RECORD FOR THE PROPOSED PROJECT MAY PROVIDE
 A SIGNED AND NOTARIZED AFFIDAVIT WHICH WOULD INDICATE
 ACKNOWLEDGEMENT THAT THE PROPOSED DESIGN DOES NOT MEET
 THE REQUIREMENTS OF THE FAIR HOUSING ACCESSIBILITY
 GUIDELINES, BUT WILL ALSO REQUIRE THE OWNER TO AGREE TO
 REVERT THE UNIT BACK TO COMPLIANCE AT TIME OF SALE IF
 REQUESTED BY THE BUYER. IF THIS OPTION IS CHOSEN,
 PLEASE SEND AN EMAIL TO [email protected] AND I WILL SEND A
 COPY OF THE FAIR HOUSING AFFIDAVIT.
  
 4) A-2: PROVIDE A SHOWER PAN DETAIL- SEC. 417.5.1-
 417.5.2.6 FBC P.
  
 A) REFER TO SEC. 1002.1: THE HORIZONTAL DISTANCE FROM
 THE CENTERLINE OF THE NEW BATHTUB DRAIN OUTLET TO THE
 CENTERLINE OF THE INLET OF THE EXISTING TRAPS SHALL NOT
 EXCEED 30". PLEASE PLACE A NOTE ON THE PLAN INDICATING
 COMPLIANCE.
  
 B) RELOCATING THE SHOWER DRAIN OUTLETS WILL REQUIRE THE
 FLOOR TO BE X-RAYED TO IN ORDER TO LOCATE REINFORCING
 STEEL OR POST STRESSED FLOOR TENDONS. PLEASE PLACE A
 NOTE ON THE PLANS TO VERIFY COMPLIANCE WILL BE MET-
 SEC. 107.2.1 WPB.
  
 C) NEW DRAIN PIPE PENETRATIONS OF THE EXISTING SLAB
 WILL REQUIRE FIRESTOPPING TO SEAL THE ANNULAR SPACE
 AROUND THE PIPING. PROVIDE A UL TESTED AND APPROVED
 FIRESTOPPING ASSEMBLY DETAIL ON THE PLAN- SEC. 315.1
 FBC P AND SEC. 714.4.1.2 FBC B.
  
 4) DEFERRED SUBMITTALS- SECTION 107.3.4.1 WPB.
 MANUFACTURER?S SPECIFICATIONS ARE REQUIRED FOR THE
 SHOWER VALVE- SEC. 412.3 & 412.5 FBC P.
  
 WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A RESPONSE
 LETTER ADDRESSING EACH ITEM ALONG WITH THE CITY
 RE-SUBMITTAL FORM.
