 CONSTRUCTION ELEMENTS, COMPONENTS, SYSTEMS, AND SPACES
 SHALL COMPLY WITH THE REQUIREMENTS OF THE FLORIDA
 BUILDING CODE, BUILDING.

 SCOPE OF WORK; CONVERTING PT TREATMENT ROOM TO EOS
 IMAGING ROOM. RAISE CEILING, DEMOCASEWORK, POWER & DATA
 FOR NEW EQUIPMENT.
  
 1) THE FLOOR PLANS LS101 SHOW THE EOS ROOM AS PART OF
 THE 1-HOUR FIRE BARRIER, ACCORDING TO YOUR LIFE SAFETY
 LEGEND. PLEASE PROVIDE THE RATING FOR OPENING FIRE
 PROTECTION ASSEMBLIES, RATINGS AND MARKINGS IN
 ACCORDANCE WITH THE 2020 FBC-BUILDING TABLE 716.5.
  
 2) EOS ROOM IS A LOW DOSAGE X-RAY ROOM. THE FLOOR PLAN
 A-101 STATES THE DOOR TO REMAIN CLOSED DURING X-RAY
 USE. PLEASE REVIEW IN THE 2020 FBC-BUILDING CODE 465.6
 DOORS, INTERLOCKS AND WARNING SYSTEMS.
  
 3A) SHEET A101. THERE IS A NOTE TO SEE PHYSICIST REPORT
 FOR LEAD WALL LININGS AND LEAD LINED WINDOW & WALL SEE
 PHYSICIST REPORT. NOTE THIS REPORT WAS NEVER SUBMITTED.
  
 3B) SHEET A101. BEFORE THE PHYSICIST REPORT IS
 SUBMITTED TO THE ARCHITECT FOR REVIEW BEFORE SUBMITTING
 TO THE BUILDING DIVISION FOR REVIEW. FL 61G1-23.015 (2)
 THE ARCHITECT IS RESPONSIBLE FOR SUPERVISING AND
 REVIEWING ALL PROJECT DATA, REPORTS, SHOP DRAWINGS ETC.
  
 4) A TRANSMITTAL LETTER / NARRATIVE LISTING THE
 ORIGINAL REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF
 THE REVISION MADE, IDENTIFYING THE SHEET OR
 SPECIFICATION PAGE WHERE THE CHANGES CAN BE FOUND WILL
 HELP TO EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR
