 1) PROVIDE A COPY OF THE PALM BEACH COUNTY RIGHT-OF-WAY
 APPROVAL FOR THE PROPOSED WIRELESS COMMUNICATION
 FACILITY IN THE RIGHT-OF-WAY.
  
 2) THE SURVEY SUBMITTED WITH THE PERMIT NEEDS TO
 PROVIDE THE HEIGHT OF THE EXISTING UTILITY POLE THAT IS
 BEING USED TO ESTABLISH THE HEIGHT OF THE PROPOSED
 WIRELESS COMMUNICATION FACILITY SO THAT STAFF CAN
 VERIFY THAT THE PROPOSED 30-FOOT TALL WIRELESS
 COMMUNICATION FACILITY HEIGHT COMPLIES WITH CITY
 REGULATIONS.
  
 3) THE EMBEDMENT DETAIL ON SHEET 07 OF 17 SHOWS A
 60-INCH DIAMETER CAISSON. THE ENTIRE CAISSON IS
 REQUIRED TO BE LOCATED WITHIN THE RIGHT-OF-WAY. THE
 SURVEY SHOWS THAT THE CENTER OF THE CAISSON IS LOCATED
 2.8 FEET FROM THE RIGHT-OF-WAY LINE, WHICH PROVIDES
 THAT THE EDGE OF CAISSON IS 3.6 INCHES FROM THE EDGE OF
 THE RIGHT-OF-WAY. HOWEVER, THE ENGINEERING PLANS ALL
 SHOW THE CENTERLINE OF THE CAISSON AT TWO FEET FROM THE
 RIGHT-OF-WAY LINE, WHICH RESULTS IN ENCROACHMENT OF 9.6
 INCHES ONTO THE PRIVATE PROPERTY. ALL PLANS MUST
 CLEARLY DEMONSTRATE THAT THE ENTIRE FACILITY IS LOCATED
 WITHIN THE RIGHT-OF-WAY.
  
 4) PROVIDE A PLAN SHOWING THE UNDERGROUND POWER ROUTE
 FROM THE SOURCE TO THE POLE. THE UNDERGROUND POWER MUST
 CLEARLY BE SHOWN TO BE EITHER WITHIN A RIGHT-OF-WAY OR
 WITHIN A GENERAL UTILITY EASEMENT IF ON PRIVATE
 PROPERTY. IF ANY PORTION OF THE UNDERGROUND POWER IS
 WITHIN A CITY RIGHT-OF-WAY, THEN COMMUNICATIONS
 FACILITY PLACEMENT PERMIT IN CITY RIGHTS-OF-WAY
 APPROVAL IS REQUIRED PRIOR TO THE APPROVAL OF THIS
 PERMIT.
  
 5) THE INFORMATION PROVIDED TO ADDRESS THE COMMENTS
 ABOVE, MAY RESULT IN ADDITIONAL COMMENTS.
  
 6) THE POLE STYLE MEETS CITY REQUIREMENTS. THE POLE
 COLOR AT THIS LOCATION SHALL BE EQUIVALENT TO
 RAL/RAYCAP 7042 TRAFFIC GREY A.
