 SLADE IS A MULTI-STORY CONDOMINIUM BUILDING THAT WAS
 DESIGNED AND CONSTRUCTED IN ACCORDANCE WITH THE FAIR
 HOUSING DESIGN ACT GUIDELINES. CONCERNING THE KITCHEN
 AND BATH ALTERATIONS, THE DESIGN PROFESSIONAL OF RECORD
 SHALL INDICATE ON THE PLANS WHICH OF THE TWO BATHROOM
 TYPES IS THE BASIS OF DESIGN- SPECIFICATION TYPE A OR B
 ON THE PLANS. PLEASE REFER TO PAGES 7.34- 7.39 OF THE
 FHDA DESIGN MANUAL FOR EXPLANATORY INFORMATION ON
 SPECIFICATION A & B AND SHOW COMPLIANCE.
  
 3) REFER TO CHAPTER 7 (REQUIREMENT 7 USABLE KITCHENS &
 USABLE BATHROOMS) AND PROVIDE ENLARGED, DIMENSIONED
 KITCHEN AND BATHROOM DRAWINGS AND DETAILS. PROVIDE THE
 FOLLOWING LIST OF DETAILS ON THE PLANS:
 PAGES 7.1- 7.6 SHOW THE CLEAR FLOOR SPACE DIAGRAMS AND
 DIMENSIONS FOR THE KITCHEN SINK AND APPLIANCES.
 PAGES 7.7- 7.11 SHOW THE CLEARANCES BETWEEN KITCHEN
 COUNTERS AND ALL OPPOSING ELEMENTS.
 PAGES 7.12- 7.16 SHOW THE REMOVABLE SINK AND APPLIANCE
 CABINET DOORS INCLUDING KNEE SPACES (IF USING FORWARD
 APPROACHES)
 PAGES 7.40-7.46 SHOW THE WATER CLOSET CLEAR FLOOR
 SPACES.
 PAGES 7.47- 7.52: SHOW THE LAVATORY CLEAR FLOOR SPACE
 DIAGRAM AND DIMENSION, SINK HEIGHT, AND KNEE AND TOE
 CLEARANCES (IF USING A FORWARD APPROACH).

 PROVIDE DRAWINGS AND DETAILS SHOWING GRAB BAR
 REINFORCEMENT LOCATIONS AND INSTALLATION DETAILS.

 PLEASE NOTE: AS AN ALTERNATIVE TO MEETING THE
 GUIDELINES FOR THE BATH ALTERATIONS, THE OWNER AND
 DESIGNER OF RECORD FOR THE PROPOSED PROJECT MAY PROVIDE
 A SIGNED AND NOTARIZED AFFIDAVIT WHICH WOULD INDICATE
 ACKNOWLEDGEMENT THAT THE PROPOSED DESIGN DOES NOT MEET
 THE REQUIREMENTS OF THE FAIR HOUSING ACCESSIBILITY
 GUIDELINES, BUT WILL ALSO REQUIRE THE OWNER TO AGREE TO
 REVERT THE UNIT BACK TO COMPLIANCE AT TIME OF SALE IF
 REQUESTED BY THE BUYER. IF THIS OPTION IS CHOSEN,
 PLEASE SEND AN EMAIL TO [email protected] AND I WILL SEND A
 COPY OF THE FAIR HOUSING AFFIDAVIT.
  
 WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A RESPONSE
 LETTER ADDRESSING EACH ITEM ALONG WITH THE CITY
