 THE SUBMITTED COMMUNICATION FACILITY PLACEMENT PERMIT -
 CITY RIGHTS-OF-WAY (21120293) MUST BE APPROVED PRIOR TO
 THIS PERMIT BEING APPROVED. THE PLANS INCLUDED WITH
 THIS ELECTRICAL PERMIT MUST MATCH THE PLANS APPROVED
 WITH THE AFOREMENTIONED PERMIT.
  
 1. PROVIDED SURVEY IS NOT COMPLETE. PLEASE PROVIDE OR
 HIGHLIGHT THE RIGHT-OF-WAY LINES ON SURVEY (ROW LINES
 STOP AT THE FLAGLER PLAZA OR TRUMP PLAZA PROPERTY) TO
 CONFIRM THAT POWER IS BEING PULLED FROM WITHIN ROW, OR
 PLEASE SHOW A UTILITY/ACCESS EASEMENT TO PULL POWER
 FROM THE TRANSFORMER PAD WITHINTRINITY PARK.
  
 3. PLEASE PROVIDE THE NECESSARY UTILITY PROVIDER
 LETTERS GRANTING APPROVAL OF THE BORE PITS, HANDHOLES,
 AND UNDERGROUND ELECTRICAL FOR THIS PROPOSED POLE
 LOCATION. IF NO COMMUNICATION IS RECEIVED, PLEASE
 PROVIDE A LETTER OF NO RESPONSE.
  
 4. PLEASE PROVIDE AT LEAST 8 FT. CLEARANCE FROM TRUNK
 OF TREES/PALMS FROM ANY BORE PIT, HANDHOLE, OR
 EXCAVATION WORK, IN ORDER TO SAFELY BUFFER THE ROOT
 SYSTEM.
  
 5. THE POLE STEALTH DESIGN, HEIGHT AND COLOR MEETS CITY

 PROVIDE A COMPLETE SURVEY SHOWING THE FULL SCOPE OF

 2. UNDERGROUND ELECTRIC SHOWN ON DRAWINGS TO CONFLICT
 WITH EXISTING SANITATION/SEWER LINE, STORM WATER,
 ELECTRIC, COMMUNICATIONS, AND WATER, PLEASE PROVIDE THE
 NECESSARY PUBLIC UTILITY LETTERS GRANTING APPROVAL OF
 THE PROPOSED BORE PITS, UNDERGROUND ELECTRIC, AND POLE
 LOCATION.
  
 3. PROPOSED BORE PITS AND HANDHOLE ARE TOO CLOSE TO
 EXISTING PALM TREES, PLEASE ADJUST TO PROVIDE THE
 NECESSARY SPACING TO PROTECT ROOT/TREE VIABILITY.
 RECOMMENDED AT LEAST 8 FT. CLEARANCE FROM PALMS TO
 SAFELY PROVIDE A BUFFER TO THE ROOT SYSTEM.
  
 4. ANTENNA POLE COLOR SHALL BE ANALOGOUS TO THE
 EXISTING DECORATIVE CITY STREET LAMPS.
