 1) FAILED. SHEET A.1: FAIR HOUSING ACCESSIBILITY
 GUIDELINES: BECAUSE THE BUILDING WAS CONSTRUCTED IN
 ACCORDANCE WITH THE ACCESSIBILITY REQUIREMENTS OF THE
 FAIR HOUSING ACT AND THE DESIGN GUIDELINES FOR USABLE
 BATHROOMS IN DWELLING UNITS, THE ALTERATIONS THAT ARE
 PROPOSED IN THE BATHROOM SHALL ALSO BE IN COMPLIANCE
 WITH THE FAIR HOUSING ACT. THE FAIR HOUSING
 ACCESSIBILITY GUIDELINES ARE INCLUDED IN THE FLORIDA
 ACCESSIBILITY CODE- 2017 SIXTH EDITION. REFER TO
 REQUIRMENT 7 USUABLE KITCHENS AND BATHROOMS AND PAGE
 7.33 IN THE FAIR HOUSING ACT DESIGN MANUAL.
 THE ARCHITECT SHALL SPECIFY WHICH USABLE BATHROOM
 SPECIFICATION TYPE A OR B WAS CHOSEN AS THE ORIGINAL
 DESIGN. PLEASE REFER TO PAGES 7.34- 7.39 OF THE FAIR
 HOUSING DESIGN ACT MANUAL FOR EXPLANATORY INFORMATION
 ON SPECIFICATION A & B. AN ENLARGED DIMENSIONED
 BATHROOM DRAWING WITH ELEVATION DETAILS WILL BE
 REQUIRED IN ORDER TO VERIFY COMPLIANCE FOR THE NEW
 FIXTURE INSTALLATIONS- SECTION 107.2.1 WPB. (PLEASE
 PROVIDE THE SPECIFICATION TYPE A OR B ON THE PLAN.)
  
 4) FAILED. BASED ON EITHER SPECIFICATION A OR B PLEASE
 REFER TO PAGES 6.12- 6.13 AND PROVIDE AND ELEVATION
 DETAIL SHOWING THE SHOWER CONTROLS LOCATION, AND THE
 GRAB BAR AND SEAT REINFORCEMENT DETAILS. SHOW THE
 LENGTHS, WIDTHS AND HEIGHTS AND THE TYPE OF
 REINFORCEMENT MATERIAL TO BE USED. SEE SECTION 603.1
 FOR ALLOWABLE MATERIALS IN NON-COMBUSTIBLE BUILDINGS.
 (SHOWER CONTROLS LOCATION AND GRAB BAR REINFORCEMENT
 DETAILS NOT PROVIDED.)
  
 PLEASE NOTE: AS AN ALTERNATIVE TO MEETING THE
 GUIDELINES FOR THE BATH ALTERATIONS, THE OWNER AND
 DESIGNER OF RECORD FOR THE PROPOSED PROJECT MAY PROVIDE
 A SIGNED AND NOTARIZED AFFIDAVIT WHICH WOULD INDICATE
 ACKNOWLEDGEMENT THAT THE PROPOSED DESIGN DOES NOT MEET
 THE REQUIREMENTS OF THE FAIR HOUSING ACCESSIBILITY
 GUIDELINES, BUT WILL ALSO REQUIRE THE OWNER TO AGREE TO
 REVERT THE UNIT BACK TO COMPLIANCE AT TIME OF SALE IF
 REQUESTED BY THE BUYER. IF THIS OPTION IS CHOSEN,
 PLEASE SEND AN EMAIL TO [email protected] AND I WILL SEND A
 COPY OF THE FAIR HOUSING AFFIDAVIT.
  
 WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A RESPONSE
 LETTER ADDRESSING EACH ITEM ALONG WITH THE CITY
