 CORRECT A CODE VIOLATION SPECIFICALLY AND CONTINUOUSLY
 NOTED IN EACH REJECTION, INCLUDING, BUT NOT LIMITED TO,
 EGRESS, FIRE PROTECTION, STRUCTURAL STABILITY, ENERGY,
 ACCESSIBILITY, LIGHTING, VENTILATION, ELECTRICAL,
 MECHANICAL, PLUMBING, AND GAS SYSTEMS, OR OTHER
 REQUIREMENTS IDENTIFIED BY RULE OF THE FLORIDA BUILDING
 COMMISSION ADOPTED PURSUANT TO CHAPTER 120, THE LOCAL
 GOVERNMENT SHALL IMPOSE, EACH TIME AFTER THE THIRD SUCH
 REVIEW THE PLANS ARE REJECTED FOR THAT CODE VIOLATION,
 A FEE OF FOUR TIMES THE AMOUNT OF THE PROPORTION OF THE
 PERMIT FEE ATTRIBUTED TO PLANS REVIEW.
  
 THE SUBMITTED ARCHITECTURAL PLANS SHOW THE ORIGINAL
 PLAN DATE OF 4/22/21 WITH NO REVISIONS AND NO REVISIONS
 DESIGNATED WITH EITHER A CLOUD OR DELTA SIGN.
  
 1) 3RD3RD TIME TEST REPORTS SUBMITTED FOR AN 8 INCH
 CONCRETE SLAB NOT A 6 INCH SLAB.
 1ST ROUND OF COMMENTS. PLEASE NOTE THE DATA SHEET
 SUPPLIED FOR PROFLEX MSC (MEGA SOUND CONTROL) IS NOT A
 TEST REPORT. THE CITY IS IN RECEIPT OF A FLAME SPREAD
 REPORT FOR THE PROFLEX MSC BUT NOT THE IIC AND STC
 REPORTS ON A 6-INCH SLAB FOR TILE.

 NO DROPPED CEILING:
 PLEASE PROVIDE ALL THREE TEST REPORTS FOR TYPE I AND OR
 TYPE II BUILDINGS SHOWING COMPLIANCE WITH:
  
 1A) 3RD REQUEST. FBC-B 2020 1207.3 STRUCTURE-BORNE
 SOUND. FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS
 OR BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E-492.
  
 1B) 3RD REQUEST. FBC-B 2020 1207.2 AIR-BORNE SOUND.
 WALLS, PARTITIONS AND FLOOR/CEILING ASSEMBLIES
 SEPARATING DWELLING UNITS FROM EACH OTHER OR FROM
 PUBLIC OR SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION
 CLASS (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED)
 FOR AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM
 E 90.
  
 1C) COMPLIED. THIS REPORT IS FOR PROFLEX SUPER SIM 90.
 FLAMESPREAD/ SMOKE DEVELOPMENT.
  
 2) COMPLIED. 2ND REQUEST.
  
 3) 3RD REQUEST. THE PROPOSED FLOOR PLAN DOES NOT STATE
 IN THE SHOWERS WHAT TYPE OF BACKER BOARD IS TO BE
 INSTALLED. 2020 FBC-B 2509.2 BASE FOR TILE. MATERIALS
 USED AS A BASE FOR WALL TILE IN TUB AND SHOWER AREAS
 AND WALLS AND CEILING PANELS IN SHOWER AREAS SHALL BE
 OF MATERIALS LISTED IN TABLE 2509.2 AND INSTALLED IN
 ACCORDANCE WITH MANUFACTURERS RECOMMENDATIONS. WATER
 RESISTANT GYPSUM BACKING BOARD SHALL BE USED AS A BASE
 FOR TILE IN WATER CLOSET COMPARTMENT WALLS WHEN
 INSTALLED IN ACCORDANCE WITH GA-216 OR ASTM C840 AND
 THE MANUFACTURERS RECOMMENDATIONS.
  
 4) A TRANSMITTAL LETTER LISTING THE ORIGINAL REVIEW
 COMMENT NUMBER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICATION PAGE WHERE
 THE CHANGES CAN BE FOUND WILL HELP TO EXPEDITE YOUR
