 PERMIT 21020863 - FIBER 1400 NORTHPOINT CROWN CASTLE
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) SUBMIT A COMPLETED COMMUNICATIONS FACILITIES
 PLACEMENT PERMIT (CITY RIGHTS-OF-WAYS) APPLICATION.
  
 2) SURVEY - NO SURVEY WAS PROVIDED.
  
 A. THE SURVEY, PREPARED BY A FLORIDA REGISTERED
 SURVEYOR, MUST CLEARLY SHOW THE RIGHT-OF-WAY LINE, THE
 SIDEWALK AND THE UTILITY EASEMENT LISTED ON THE
 ENGINEERING PLANS.
  
 B. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY. IF THE TRENCHING IS PROPOSED WITHIN FIVE (5)
 FEET OF AN EXISTING UTILITY, YOU NEED TO PROVIDE A
 LETTER FROM THAT UTILITY THAT THE PROPOSED TRENCH DOES
 NOT INTERFERE WITH THE PROVISIONS OF THEIR SERVICES.
  
 C. THE TREES AND THEIR ROOT ZONE NEED TO BE SHOWN.
 NOTE: THE ENGINEERING PLANS AND THE PHOTOS SHOW THAT
 THE TRENCH WILL IMPACT THE ROOT ZONE. THE FIBER BORE
 NEEDS TO BE CLEARLY SHOWN AVOIDING DAMAGE TO THE TREE
 ROOT ZONES. SEE COMMENT #3 FOR SOLUTION.
  
 D. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK IS BEING INSTALLED ON PRIVATE
 PROPERTY. NOTE: THE ENGINEERING PLANS MUST BE AT A
 10FT, 20FT, OR 40FT SCALE, NOT THE 50FT SCALE THAT WAS
 PROVIDED.
  
 3) THE ENGINEERING PLAN SHOWS A 12 FOOT WIDE UTILITY
 EASEMENT ON THE 1400 NORTHPOINT PARKWAY PROPERTY
 IMMEDIATELY ADJACENT TO THE RIGHT-OF-WAY LINE (SUBJECT
 TO VERIFICATION BY COMMENT #2A. IN ORDER TO NOT DAMAGE
 THE ROOT SYSTEM OF THE TREE SOUTH OF STA 0+45 BY
 TRENCHING TO STA 0+90, YOU NEED TO TRENCH EAST FROM STA
 0+45 TO THE SIDEWALK AND THEN TRENCH SOUTH ALONG THE
 WEST SIDE OF THE SIDEWALK TO STA 0+90. THE MAJORITY OF
 THE TRENCHING WILL OCCUR WITHIN THE UTILITY EASEMENT
 AND WILL BE OF SUFFICIENT DISTANCE FROM THE EXISTING
 TREE SO AS NOT TO DAMAGE THE ROOTS. ALTERNATIVELY YOU
 MAY BORE BETWEEN THE TWO STAS, PROVIDED THAT THE BORE
 IS GREATER THAN THREE (3) FEET IN DEPTH.
  
 4) ESTIMATED SCHEDULE - NO SCHEDULE WAS PROVIDED.
 PROVIDE A NARRATIVE OF THE ESTIMATED SCHEDULE OF THE
 PROPOSED WORK.
  
 5) WHEN RESUBMITTING THE PLANS, INCLUDE THE PERMIT
 NUMBER (21020863) AND A WRITTEN RESPONSE TO THE
 COMMENTS.
  
 6) THE INSTALLATION CONTRACTOR MUST CONTACT SUNSHINE
 811 TO VERIFY THE LOCATION OF ALL UTILITIES AND VERIFY
 ALL DEPTHS PRIOR TO CONSTRUCTION. ALL CITY UTILITIES
 MUST BE LOCATED BY SOFT DIG METHODS BEFORE STARTING THE
 WORK.
  
 7) THE RIGHT-OF-WAY USE PERMIT REQUIRES YOU TO PROVIDE
 A CERTIFICATE OF LIABILITY INSURANCE, WHICH IN
 ACCORDANCE WITH SECTION 78-406 OF THE CITY CODE
 SPECIFICALLY NAMES THE CITY OF WEST PALM BEACH, ITS
 COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED.
  
 8) MARKER POLES MAY NOT BE INSTALLED WITHIN THE CITY
 RIGHT-OF-WAY OR ON PRIVATE PROPERTY. THE ONE SHOW IN
 THE PHOTO NEEDS TO BE REMOVED. IF A MARKER IS REQUIRED
 IT NEEDS TO BE IN THE HANDHOLE.
