 1) THE SUBMITTED COMMUNICATION FACILITY PLACEMENT
 PERMIT - CITY RIGHTS-OF-WAY MUST BE APPROVED PRIOR TO
 THIS PERMIT BEING APPROVED. THE PLANS INCLUDED WITH
 THIS ELECTRICAL PERMIT MUST MATCH THE PLANS APPROVED
 WITH THE AFOREMENTIONED PERMIT. SOME OF THE COMMENTS
 BELOW ARE INCLUDED ON THE PLACEMENT PERMIT REVIEW AS
 WELL AS WITH THIS PERMIT.
  
 2) SHEET T1 LISTS THE SCOPE OF WORK INCLUDES THE
 INSTALLATION OF AN ENCLOSURE CABINET. NONE OF THE PLANS
 SHOW AN ENCLOSURE CABINET. PLEASE CLARIFY WHAT THIS
 MEANS. THE CITY DOES NOT PERMIT FREESTANDING OR POLE
 MOUNTED CABINETS.
  
 3) THE SURVEY AND THE ENGINEERING PLANS THAT ARE USED
 AS BASE PLANS TO SHOW THE AREA SURROUNDING THE POLE DO
 NOT ACCURATELY DEPICT THE EXISTING CONDITIONS.
 SPECIFICALLY, THE CURB LINE AND PLANTER LOCATIONS EAST
 OF THE POLE ARE INCORRECTLY DEPICTED. THE SURVEY AND
 PLANS NEED TO BE CORRECTED.
  
 4) ONCE THE BASE CONDITIONS ARE ACCURATELY SHOWN, THE
 PROPOSED POLE NEEDS TO BE SHOWN IN LINE WITH THE
 EXISTING PALM TREES TO CREATE A UNIFORM STREETSCAPE
 ALONG THE SOUTH SIDE OF FERN STREET. PLEASE NOTE THAT
 STEALTH POLE SHOWN ON SHEET C3, ELEVATION AND EQUIPMENT
 PLAN, HAS A 14-INCH DIAMETER. SCALING THE POLE ON THE
 SURVEY AND SITE PLAN SHOW A SEVEN (7) INCH DIAMETER
 POLE. THE SURVEY AND SITE PLAN NEED TO SHOW THE CORRECT
 POLE SIZE AND DIMENSIONS FROM THE VARIOUS FEATURES
 SHOWN.
  
 5) THE UTILITY POWER PLAN INCLUDED SHOWS THE
 UNDERGROUND POWER GOING UNDER THE 360 SOUTH ROSEMARY
 BUILDING ON PRIVATE PROPERTY WHICH IS NOT PERMITTED. AN
 ALTERNATIVE ROUTE THAT IS WITHIN THE PUBLIC
 RIGHT-OF-WAY IS REQUIRED.
  
 NOTE: PRIOR TO BEGINNING THE PLACEMENT OF THE POLE, THE
 UNDERGROUND FIBER, THE HANDHOLES, ETC. A RIGHT-OF-WAY
 USE PERMIT MUST BE APPROVED BY THE CITY'S ENGINEERING
 SERVICES DIVISION.
