 THE CONTRACTOR, BUILDING OFFICIAL, AND ASSISTANT
 BUILDING OFFICIAL.
  
 THIS ADDRESS HAS A NUMBER OF EXPIRED PERMITS INCLUDING
 THE FOLLOWING ISSUES:
  
 PERMITS 14060942 AND 14050331 DID NOT OBTAIN ALL OF THE
 REQUIRED INSPECTIONS. THE WORK DEVIATED FROM THE PERMIT
 APPROVED PLANS AND WORK WAS COMPLETED WITHOUT REVISED
 DRAWINGS BEING SUBMITTED TO OUR OFFICE, STAMPED AND
 USED FOR INSPECTIONS BY TEW & TAYLOR, THE PRIVATE
 PROVIDER. ON 4/28/15 AN INSPECTION AUDIT WAS DONE AND
 THE CITY INSPECTOR NOTED THAT THE 2ND FLOOR O THE
 GARAGE BUILDING, WHICH WAS SUPPOSED TO BE STORAGE
 SPACE, HAD BEEN CONVERTED INTO HABITALE SPACE WITHOUT A
 PERMIT.
  
 THE ELECTRICAL CONTRACTOR SUBMITTED A LETTER INFORMING
 THAT OTHERS WERE WROKING ON THE PROPERTY AND THEY
 WANTED THEIR ELECTRICAL PERMITS 14090361 AND 14090363
 CANCELLED.
  
 ON 6/18/15 BARRY RATNER SUBMITTED AN INSPECTION ONLY
 PERMIT TO "REVIEW STATUS OF INSPECTIONS/PERMITS, INCL
 101 CHURCHILL RD". KEN CONRAD, CHIEF BUILDING
 INSPECTOR, ENTERED THE FOLLOWING INSPECTION NOTES
 OUTLINING NUMEROUS AREAS OF WORK OUTSIDE THE SCOPE OF
 PERMITS 14060942 AND 14050331:
  
 MET WITH MR. RATNER, A CONSULTANT HIRED BY THE NEW
 OWNER, TO DETERMINE THE STATUS OF THE WORK COMPLETED
 PER THE APPROVED PLANS AND THE INSPECTIONS PERFORMED
 AND COMPLETED BY THE PRIVATE PROVIDER, TEW AND TAYLOR.
 THE PRIVATE PROVIDER NOTICE TO BUILDING OFFICIAL WAS
 PROVIDED ON 09/09/14. THE FOLLOWING WAS OBSERVED:
  
 1) IT APPEARS THAT THE NEW ENGINEERED ROOF TRUSSES FOR
 THE FLORIDA ROOM HAVE EXCEEDED THE ALLOWABLE NET
 DEFLECTION AS ORIGINALLY DESIGNED AND ARE SAGGING AT
 MID- SPAN. TEMPORARY SHORING HAS BEEN INSTALLED TO
 PREVENT FURTHER DEFLECTION. ADVISED THE CONSULTANT THAT
 THE EOR SHALL EVALUATE THE FORMER CONDITION IMMEDIATELY
 AND REQUESTED THEY SUBMIT HIS/HER FINDINGS TO OUR
 DEPARTMENT FOR REVIEW.
  
 2) NO PRODUCT APPROVAL LABELS ON SGD /FRAMES.
  
 3) NEW GAS RANGE INSTALLED: REQUIRES A GAS PERMIT.
  
 4) NEW DECKING, RAILING, AND SPIRAL STAIRS, INSTALLED
 ON EXTERIOR 2ND FLOOR REQUIRES PERMITS AS PER THE FBC,
 SECTION 105.1.
  
 5) NEW ELECTRICAL WORK INSTALLED AT REMODELED
 BATHROOMS. REQUIRE AN ELECTRICAL PERMIT PER THE FBC,
 SECTION 105.1.
  
 6) IT APPEARS THAT THE EXISTING POOL CABANA WAS
 REMODELED AND A NEW ROOF WAS INSTALLED. AN UTILITY ROOM
 HOUSING A NEW WATER HEATER WAS ADDED AT THE WEST
 ELEVATION. REQUIRE A BUILDING, PLUMBING AND ELECTRICAL
 PERMIT.
  
 7) A 75-80 GALLON WATER HEATER WAS INSTALLED WITHIN THE
 ATTIC AT THE ATTIC ACCESS OPENING. REQUIRE A PLUMBING
 PERMIT FOR THE SAME. ENGINEERING WILL BE REQUIRED FOR
 THE ADDITIONAL LOAD ON THE EXISTING ROOF FRAMING BOTTOM
 CHORDS.
  
 8) NEW EXTERIOR DOORS INSTALLED AT THE MAIN HOUSE. NOT
 REFLECTED ON THE APPROVED PLAN.
  
 9) IT APPEARS THAT SPRAY FOAM INSULATION WAS APPLIED IN
 THE ATTIC OF THE MAIN HOUSE. THIS WAS NOT REFLECTED ON
 THE APPROVED PLAN. THE PLAN SHOULD BE REVISED TO
 SPECIFY TYPE OF PRODUCT APPLIED, IGNITION BARRIER, AND
 R-VALUE TO BE OBTAINED.
  
 10) A NEW WATER HEATER IS INSTALLED UNDER THE STAIRS AT
 THE NORTH ELEVATION AND APPEARS TO BE LOWER THAN THE
 BASE FLOOD ELEVATION.
  
 11) THE EXISTING POOL WAS REMODELED WITH NO PERMITS.
  
 12) OBSERVED THAT THERE WERE NO SMOKE ALARMS INSTALLED
 WITHIN THE EXISTING RESIDENCE AS REQUIRED BY THE
 FLORIDA BUILDING CODE (EXISTING), SECTION 1004.1.
  
 13) REQUIRE THE COMPLETION AND APPROVAL OF ALL FINAL
 INSPECTIONS AS PER THE FBC, SECTION 110.1. ALL FINAL
 DOCUMENTATION SHALL BE SUBMITTED: ELEVATION
 CERTIFICATE/INSULATION CERTIFICATE/FINAL TERMITE
 TREATMENT CERTIFICATE AND TRUSS ENGINEERING.
  
 ADVISED MR. RATNER THAT ALL AFOREMENTIONED ITEMS SHALL
 BE COMPLETED PRIOR TO THE OCCUPANCY OF THE RESIDENCE.
