 CONSERVATION. PLEASE HAVE FORM PROPERLY SIGNED BY THE
 OWNER OR AUTHORIZED AGENT TO HELP EXPEDITE THE REVIEW
 PROCESS.
 NOTE: THIS BUILDING IS CONSIDERED A "RENOVATED
 BUILDING". THEREFORE, IT IS NOT EXEMPT FROM THE ENERGY
 CODE. SEE SEC. R101.4.2 FBC-ENERGY CONSERVATION.

 THE OWNER PRIOR TO RECEIVE THE PERMIT AS REQUIRED BY
 SEC. R103.1.1.2 FBC-ENERGY CONSERVATION, 6TH EDITION
 (2017).

 FORM. THE ZIP CODE FOR THIS ADDRESS IS 33405 NOT 33409.
