 1- APPLICATION WAS SUBMITTED AS AN ELECTRICAL PERMIT
 ONLY FOR THE INSTALLATION OF 100 AMP ELECTRICAL SERVICE
 TO SMALL CELL POLE. BASED ON THE INFORMATION SUBMITTED,
 IT LOOKS LIKE A COMMUNICATIONS PERMIT IS REQUIRED. CITY
 ORDINANCE NO. 4774-18.
 NOTE: PLEASE SEE ZONING AND ENGINEERING COMMENTS.

 NOT ADDRESSED. PLEASE SEE ZONING AND ENGINEERING
 COMMENTS.
  
 ORIGINAL COMMENT:
 2- TITLE SHEET T1 SPECIFIES "INSTALLATION OF PROPOSED
 POWER FOR SMALL CELL EQUIPMENT ON PROPOSED METAL POLE".
 SCOPE OF WORK IS NOT CLEAR. IS THERE A NEW METAL POLE
 OR IS IT EXISTING INSTALLED BY FPL.?. PLANS ALSO SHOW
 VERIZON WIRELESS ENCLOSURE ATTACHED TO THE POLE. THERE
 ARE NO SPECIFICATIONS FOR THE ATTACHMENT OF THIS
 ENCLOSURE TO THE POLE. IS THIS DONE UNDER ANOTHER
 PERMIT. ? SEC. 107.2.1 CITY AMENDMENTS.
 PROVIDE PROVIDE COMPLETE DETAILS AND SPECIFICATIONS FOR
 THE PROPOSED METAL POLE AND VERIZON WIRELESS ENCLOSURE
 ATTACHMENT IF THEY ARE PART OF THIS PERMIT. SEC. 1609.1
 FBC-BUILDING.
  
 RESPONSE:
 A) IT IS NOT CLEAR IF A NEW POLE WILL BE INSTALLED OR
 NOT. IF NEW POLE IS INSTALLED, THEN PROVIDE COMPLETE
 ENGINEERING DETAILS AND SPECIFICATIONS, INCLUDING THE
 EFFECTIVE PROJECTIVE AREA OF THE POLE AND PROPOSED
 EQUIPMENT TO BE INSTALLED IN IT.
 PLANS ALSO SHOW VERIZON WIRELESS ENCLOSURES ATTACHED TO
 THE POLE. THERE ARE NO SPECIFICATIONS FOR THE
 ATTACHMENT OF THIS ENCLOSURES TO THE POLE. SEC. 1609.1
 FBC-BUILDING. ALSO, PROVIDE ALL THE WIND DESIGN DATA AS
 REQUIRED BY SEC. 1603.4 FBC-BUILDING.
  
 B)PROVIDE POLE MANUFACTURER'S SPECS MEETING THE
 ENGINEER'S DESIGN CRITERIA. ENGINEER OF RECORD TO
 REVIEW/APPROVED POLE SPECS PRIOR TO SUBMITTAL. SEC.
 107.3.4.1 CITY AMENDMENTS.
  
 ORIGINAL COMMENT:
 3- VALUATION DECLARED IN THE PERMIT APPLICATION IS LOW.
 VALUATION TO INCLUDE ALL LABOR, MATERIALS, CONTRACTOR'S
 OVERHEAD AND PROFIT AND DESIGN PROFESSIONAL FEES AS
 REQUIRED BY SEC. 109.3 CITY AMENDMENTS. UPDATE
 VALUATION AS REQUIRED, INCLUDING DESIGN PROFESSIONAL
 FEES.

 DETERMINE VALUE.
