 CHECKLIST.
 CODE: FBC 6TH EDITION (2017) AND CITY AMENDMENTS.
  
 NOTE:
 BUILDING REVIEW MAY BE APPROVED WITH PROVISO. PROVISO
 MEANS CONTRACTOR (OWNER/BUILDER)WILL SUBMIT REQUIRED
 INFORMATION AFTER THE PERMIT IS ISSUED BUT BEFORE
 INSPECTIONS TO AVOID CONFLICTS/DELAYS. REVISION FEES
 APPLY. CONTRACTOR TO ADVICE IF PROVISO OPTION IS
 PREFERRED AT THIS TIME OR IF RE-SUBMITTING REQUIRED
 INFORMATION BEFORE PERMIT ISSUANCE.
  
 ****NEED OWNER'S SIGNATURE ON THE ENERGY CALCULATIONS
 PRIOR TO ISSUANCE OF PERMIT.
  
  
 ORIGINAL COMMENT:
 1- PROVIDE ENERGY CALCULATIONS (2 COPIES) AS REQUIRED
 BY SEC. 1106 FBC-EXISTING BUILDING AND SEC. R101.5
 FBC-ENERGY CONSERVATION.
 NOTE: PLEASE MAKE SURE FORMS ARE PROPERLY SIGNED BY ALL
 REQUIRED PARTIES AND IS COORDINATED WITH PLANS TO AVOID
 CONFLICTS/DELAYS.
  
 RESPONSE:
 A) PROVIDE 2 COPIES OF ENERGY CALCULATIONS. ONLY ONE
 WAS PROVIDED. NEED TWO AS REQUIRED BY SEC. 107.1 CITY
 AMENDMENTS.
  
 B) ENERGY CALCULATIONS FORM IS REQUIRED TO BE CERTIFIED
 AT THE BOTTOM OF THE FORM BY THE OWNER, THE OWNER'S
 ARCHITECT OR OTHER AUTHORIZED AGENT LEGALLY DESIGNED BY
 THE OWNER PRIOR TO RECEIVE THE PERMIT AS REQUIRED BY
 SEC. R103.1.1.2 FBC-ENERGY CONSERVATION, 6TH EDITION
 (2017).
  
 C) PROVIDE COMPLETE AND CORRECT ON TOP OF FORM. NEED
 CORRECT OWNER'S NAME AND CORRECT JURISDICTION NUMBER .
 CITY 'S JURISDICTION NUMBER IS 604700 NOT 602700. ALSO,
 PROVIDE THE NAME OF THE BUILDER FOR THIS ADDITION. SEC.
 R101.5.1.1.2 FBC-ENERGY CONSERVATION.
  
 D) REVISE DOOR INFORMATION PAGE 3 OF 4. DOOR NOA
 17-1212.10 SUBMITTED IS FOR GLAZED DOOR. THEREFORE,
 THIS DOOR NEEDS TO BE INCLUDED IN THE WINDOW GLAZING
 AREA AS REQUIRED BY SEC. R405.5.3.3 FBC-ENERGY
 CONSERVATION. AND DOOR IS NOT LOCATED ON THE NORTH
 SIDE. THE DOOR IS LOCATED ON THE WEST SIDE. SEE PLANS
 AND REVISE AS REQUIRED.
  
 E) REVISE WINDOWS ORIENTATION AND OVERHANG DEPTH AND
 SEPARATION TO MATCH PLANS. SEC. R101.5.1.1.2 FBC-ENERGY
 CONSERVATION.
 NOTE: THERE IS ONLY ONE WINDOWS ON THE NORTH SIDE. AND,
 THERE ARE TWO WINDOWS ON THE WEST SIDE. SEE PLANS AND
 REVISE AS REQUIRED.

 7- PROVIDE PRODUCT APPROVAL (2 COPIES) AS REQUIRED BY
 FAC 61G20-3.001 FOR THE HURRICANE SHUTTERS SINCE WINDOW
 NOA 17-1212.19 AND DOOR NOA 17-1212.10 DON'T HAVE
 IMPACT GLAZING.
 PROTECTION OF OPENINGS IS REQUIRED BY SEC. R301.2.1.2
 FBC-RESIDENTIAL AND SEC. 1101.1 FBC-EXISTING BUILDING
 NOTE: NEED TO PROVIDE ALSO A SHUTTERS COMPLETE
 INSTALLATION SCHEDULE AS REQUIRED BY CITY
 IMPLEMENTATION STANDARD FOR WIND-BORNE DEBRIS IMPACT
 PROTECTION FOR THE FLORIDA BUILDING CODE. SEE CITY

 RESPONSE:
 WINDOW NOA WAS REPLACED WITH NOA 17-1212.21. THESE ARE
 IMPACT WINDOWS AND DON'T REQUIRE SHUTTERS. ON THE OTHER
 HAND, DOOR DOES REQUIRE SHUTTERS BECAUSE IT IS NOT AN
 IMPACT DOOR.
 NOTE: THIS MAY BE A PROVISO AND CONTRACTOR MAY SUBMIT
 AFTER THE PERMIT IS APPROVED BUT BEFORE INSPECTIONS.
  
 ADDITIONAL COMMENT:
 CLARIFY WHY SUBMITTING NOA 18-0529.03 FOR ALUMINUM
 MULLIONS. THERE ARE NO MULLIONS SHOWN ON PLANS FOR THE
 ADDITION.
  
 ORIGINAL COMMENT:
 8- ENGINEER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS (DOORS,
 WINDOWS, SHINGLES, SHUTTERS) AS REQUIRED BY SEC.
 107.3.4.1 CITY AMENDMENTS.

 PLANS REQUIRE REVIEW BY THE PBC IMPACT FEE OFFICE. CALL
 (561) 233-5025. UPON APPROVAL OF PLANS, YOU MAY CHECK
 OUT ONE PLAN SET AND TAKE TO THAT OFFICE FOR REVIEW.
 PLEASE RETURNED STAMPED AND WITH A COPY OF THE RECEIPT
 AS APPLICABLE. PLEASE PROVIDE THE MUNICIPAL IMPACT FEE
 QUESTIONNAIRE FORM TO THE PBC IMPACT FEE OFFICE. THE
