 NOTE:
 PLUMBING REVIEW MAY BE APPROVED WITH PROVISO. PROVISO
 MEANS CONTRACTOR WILL SUBMIT REQUIRED INFORMATION AFTER
 THE PERMIT IS ISSUED BUT BEFORE INSPECTIONS TO AVOID
 CONFLICTS/DELAYS. REVISION FEES MAY APPLY. CONTRACTOR
 TO ADVICE IF PROVISO OPTION IS PREFERRED AT THIS TIME
 OR IF RE-SUBMITTING REQUIRED INFORMATION BEFORE PERMIT
 ISSUANCE.

 CITY AMENDMENTS ONCE THE ADDRESS IS GIVEN BY MICHAEL
 ORIO (SEE COMMENT ABOVE), PLEASE PROVIDE THE ADDRESS ON
 ALL PLANS.
  
 RESPONSE:
 PLANS SUBMITTED SHOW 519 58TH STREET. THIS IS NOT THE
 CORRECT ADDRESS. REVISE AS REQUIRED. PROVIDE CORRECT
 ADDRESS 701 58TH STREET.
  
 WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION &
 REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
 EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR ANTICIPATED
 COOPERATION.
  
 PLEASE, INSERT CORRECTED PAGES INTO THE SUBMITTAL AND
 REMOVE/VOID THE PREVIOUSLY REVIEWED SHEETS. PLEASE
