 REVIEW CHECKLIST.
 CODE: FBC 6TH EDITION (2017) AND CITY AMENDMENTS.
  
 NOTE:
 BUILDING REVIEW MAY BE APPROVED WITH PROVISO. PROVISO
 MEANS CONTRACTOR WILL SUBMIT REQUIRED INFORMATION AFTER
 THE PERMIT IS ISSUED BUT BEFORE INSPECTIONS TO AVOID
 CONFLICTS/DELAYS. REVISION FEES MAY APPLY. CONTRACTOR
 TO ADVICE IF PROVISO OPTION IS PREFERRED AT THIS TIME
 OR IF RE-SUBMITTING REQUIRED INFORMATION BEFORE PERMIT
 ISSUANCE.
  
  
 NOTE: SEC. 801.3 FBC-EXISTING BUILDING. ALL NEW
 CONSTRUCTION ELEMENTS, COMPONENTS, SYSTEMS, AND SPACES
 SHALL COMPLY WITH THE REQUIREMENTS OF THE FLORIDA
 BUILDING CODE, BUILDING.
  
  
 ORIGINAL COMMENT:
 1- ACCORDING TO SEC. R301.2.1.4 FBC-RESIDENTIAL THIS
 BUILDING IS LOCATED IN EXPOSURE "D" NOT "C". REVISE THE
 DESIGN CRITERIA SHOWN ON SHEET A-3 AT THE BOTTOM LEFT
 OF THE PAGE. REVISE THE BUILDING EXPOSURE AND ALL THE
 DESIGN WIND PRESSURES ACCORDINGLY.
  
 RESPONSE:
 THE BUILDING EXPOSURE WAS REVISED ON SHEET A-3 BUT THE
 DESIGN PRESSURES WERE NOT REVISED. THE SAME DESIGN
 PRESSURES FROM PREVIOUS REVIEW WERE SPECIFIED.
 ARCHITECT STATES THAT DOORS & WINDOWS DESIGN PRESSURES
 ARE FOR WORST CASE. USING TABLE R301.2(2)
 FBC-RESIDENTIAL AS A REFERENCE, THE DESIGN PRESSURES
 FOR WALL OPENINGS ARE LOW. WORST CASE WOULD BE THE
 MINIMUM EFFECTIVE AREA OF 10PSF, AT THE MEAN ROOF
 HEIGHT OF 25 FEET, AT THE END ZONE (#5) AND BUILDING
 EXPOSURE "D". SEE TABLE R301.2(3) FBC-RESIDENTIAL FOR
 PROPER ADJUSTMENTS.
 REVISE DOORS AND WINDOWS DESIGN PRESSURES OR PROVIDE
 CALCULATIONS FOR EACH OPENING. SEC. 107.2.1.2 CITY
 AMENDMENTS.
  
 ORIGINAL COMMENT:
 2- SPECIFY CARBON MONOXIDE ALARMS BY THE BEDROOMS AS
 REQUIRED BY SEC. R315.1 FBC-RESIDENTIAL.
  
 RESPONSE:
 PARTIALLY COMPLIED. SMOKE ALARM WITH CARBON MONOXIDE
 COMBO ALARM WAS SPECIFIED FOR THE BEDROOM IN THE SECOND
 FLOOR ADDITION. NEED TO SPECIFY THE CARBON MONOXIDE
 ALARM FOR ALL EXISTING BEDROOMS ALSO.

 5- ADDITION IS DESIGNED WITH UNVENTED ATTIC. ARCHITECT
 IS SPECIFYING R-30 ICYNENE SPRAY INSULATION. PROVIDE
 ICC EVALUATION REPORT (2 COPIES) FOR THE ICYNENE R-30
 INSULATION TO VERIFY COMPLIANCE WITH SEC. R316
 FBC-RESIDENTIAL.
 NOTE: ARCHITECT TO REVIEW AND APPROVE THE REPORT IN
 WRITING PRIOR TO SUBMITTAL TO THIS OFFICE
  
 RESPONSE:
 A) ARCHITECT'S RESPONSE LETTER STATES THAT THE
 INSULATION WILL BE R-20 BUT SPECIFICATION ON SHEET A-4
 WERE REVISED TO R-13. CLARIFY WHAT IS THE CORRECT R
 VALUE. SEC. 107.2.1 CITY AMENDMENTS.
  
 B) ICC EVALUATION REPORT SUBMITTED IS OLD. ESR-3199
 REPORT SUBMITTED WAS SUPPOSED TO BE RENEWED ON 04/2017.
 SEE UPPER RIGHT CORNER OF REPORT SUBMITTED. PROVIDE
 COPIES OF CURRENT REPORT.
 NOTE: ARCHITECT, PLEASE STAMP THE FRONT PAGE OF THE
 REPORT NOT THE BACK OF THE PAGE. THIS WILL FACILITATE
 PROPER SCANNING FOR RECORDS RETENTION. THANKS IN

 ADDITION. (B) REVISE CEILING R-VALUE TO MATCH PLANS.
 PROVIDE THE NAME OF THE BUILDER. SEC. R101.5.1.1.2
 FBC-ENERGY CONSERVATION.

 REPEAT COMMENT. NOT ADDRESSED. ENERGY CALCULATIONS WERE
 NOT REVISED.
  
 ORIGINAL COMMENT:
 9- DESIGNER OF RECORD TO SPECIFY ROOF TILE FASTENING AS
 REQUIRED BY SEC. 107.2.1.1 CITY AMENDMENTS. IF
 SPECIFYING SCREWS, THEN SPECIFY SIZE AND NUMBER OF
 SCREWS. IF SPECIFYING ROOF TILE ADHESIVE, THEN SPECIFY
 PADDY SIZE AND PRODUCT APPROVAL IS REQUIRED.
  
 RESPONSE:
 REPEAT COMMENT. NOT ADDRESSED. ARCHITECT'S RESPONSE
 LETTER STATES "SEE REVISED DRAWING S-2". THERE IS NOT
 INFORMATION ON SHEET S-2 REGARDING THE FASTENING OF THE
 ROOF TILE. PRODUCT APPROVALS WERE NOT SUBMITTED YET.
 THEY WILL BE SUBMITTED LATER AS A DEFERRED SUBMITTAL.
 THERE IS NO INDICATION OF HOW THE ROOF TILES ARE GOING
 TO BE FASTENED. IN ORDER TO GO FORWARD WITH THIS ITEM,
 THIS WILL BE TAKEN CARE OF WHEN PRODUCT APPROVALS ARE
 SUBMITTED.
 NOTE: PRODUCT APPROVALS WILL NEED TO BE MARKED WITH THE
 SPECIFIC FASTENING METHOD AND REVIEWED/APPROVED BY THE
 DESIGNER OF RECORD PRIOR TO SUBMITTAL TO THIS OFFICE.

 C) WINDOW MULLIONS AS SHOWN ON PLANS. AND REQUIRED:
 CLEARLY SPECIFY MULLION SIZE TO BE USED TO VERIFY
 COMPLIANCE WITH THE CALCULATED DESIGN PRESSURES.
 D)ROOF TILE AND CLEARLY MARK ROOF TILE FASTENING METHOD
 AS REQUIRED BY CITY POLICY FOR ALL ROOFING PERMITS.
 E)ROOF TILE ADHESIVE IF SPECIFIED BY THE DESIGNER OF
 RECORD.
  
 11- DESIGNER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS AS
 REQUIRED BY SEC. 107.3.4.1 CITY AMENDMENTS.
  
  
 WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION &
 REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
