 I. THE SPECIAL INSPECTOR AGREEMENT WAS SUBMITTED,
 HOWEVER IS OUT OF DATE. I SENT TO [email protected]

 II. PROVIDE THRESHOLD INSPECTION PLAN. 110.8.1
  
 III. THE STATED VALUE APPEARS LOW FOR THE PROPOSED
 SCOPE OF WORK. THE PERMIT VALUATION AND PERMIT FEE MUST
 BE COMMENSURATE WITH THE VALUATION AND FEE THAT A
 LICENSED CONTRACTOR WOULD PROVIDE. YOU MAY PROVIDE A
 COPY OF THE CONTRACT SIGNED BY THE OWNER OR A LIST OF
 LABOR, MATERIALS AND EQUIPMENT RENTAL TO VERIFY VALUE.

 110.8.1 THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OR ARCHITECT OF RECORD.
 THE STRUCTURAL INSPECTION PLAN MUST BE SUBMITTED TO THE
 ENFORCING AGENCY PRIOR TO THE ISSUANCE OF A BUILDING
 PERMIT FOR THE CONSTRUCTION OF A THRESHOLD BUILDING.
 THE PURPOSE OF THE STRUCTURAL INSPECTION PLANS IS TO
 PROVIDE SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO
 THAT THE BUILDING CAN BE ADEQUATELY INSPECTED FOR
 COMPLIANCE WITH THE PERMITTED DOCUMENTS. THE SPECIAL
 INSPECTOR MAY NOT SERVE AS A SURROGATE IN CARRYING OUT
 THE RESPONSIBILITIES OF THE BUILDING OFFICIAL, THE
 ARCHITECT OR THE ENGINEER OF RECORD. THE CONTRACTOR'S
 CONTRACTUAL OR STATUTORY OBLIGATIONS ARE NOT RELIEVED
 BY ANY ACTION OF THE SPECIAL INSPECTOR.
  
  
 CHAPTER 1, SECTION 110.8 OF THE CITY OF WEST PALM
 BEACH, CHAPTER 1 AMENDMENTS TO THE FLORIDA BUILDING
 CODE, CHAPTER 553.79 OF FLORIDA STATUTES; AND CHAPTER
 468, PART XII
  
 1. THRESHOLD INSPECTOR AGREEMENT SHALL BE COMPLETED AND
 SUBMITTED IN TRIPLICATE. A COPY WAS SENT TO THE
 CONTRACTOR EMAIL.
  
 2. PROVIDE RESUME OF JOB INSPECTOR (P.E., ARCH. OR 468
 INSPECTOR).
  
 3. PROVIDE LIST OF SIMILAR PROJECTS COMPLETED BY
 CONTRACTOR.
  
 4. PROVIDE STRUCTURAL INSPECTION PLAN.
  
 5. THIS BUILDING MEETS THE DEFINITION OF: THRESHOLD
 BUILDING F.S. 553.71 (12)
 ?THRESHOLD BUILDING? ANY BUILDING WHICH IS GREATER THAN
 (3) STORIES OR 50 FT. IN HEIGHT OR WHICH HAS AN
 ASSEMBLY OCCUPANCY CLASSIFICATION AS DEFINED IN THE
 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN
 AREA AND HAS AN OCCUPANT CONTENT OF GREATER THAN 500
 PERSONS.
  
 5A) THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS PURSUANT TO
 A STRUCTURAL INSPECTION PLAN PREPARED BY THE ENGINEER
 OF RECORD. THE STRUCTURAL INSPECTION PLAN MUST BE
 SUBMITTED TO THE ENFORCING AGENCY PRIOR TO THE ISSUANCE
 OF A BUILDING PERMIT.
  
 5B) THRESHOLD INSPECTOR: WPB AMENDMENT 109.3.6.2 W.P.B.
 CONSTRUCTION SERVICES DEPARTMENT REQUESTS FOR THRESHOLD
 BUILDINGS, A SPECIAL INSPECTOR IS REQUIRED BY F.S.
 553.79 (5) TO THE MINIMUM INSPECTIONS REQUIRED BY THIS
 CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED TO BE
 FILLED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED AND
 RETURNED TO THIS OFFICE AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE. CONTACT KEN

 5C) THRESHOLD BUILDING, REQUIRED. W.P.B. AMENDMENT
 110.8.1 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, ?TO THE BEST OF THE
 ARCHITECT?S OR ENGINEER?S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE FIRE-SAFETY
 STANDARDS AS DETERMINED BY THE LOCAL AUTHORITY IN
 ACCORDANCE WITH THIS SECTION AND 633 F.S.?
  
 5D) FOR THE SPECIAL INSPECTOR & DESIGNATED JOB
 INSPECTORS ? PLEASE PROVIDE A RESUME? OF INSPECTION
 EXPERIENCE ON PREVIOUS PROJECTS. THE RESUME? MUST BE
 ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED HEREIN.
 THE BUILDING OFFICIAL OR HIS DESIGNEE WILL REVIEW THE
 RESUME?. AFTER THE RESUME? IS REVIEWED, AN INTERVIEW
 MAY BE SCHEDULED.
  
 5E) JOB INSPECTOR ? FBC 110.8.6 THE BUILDING DEPARTMENT
 MAY ALLOW A SPECIAL INSPECTOR TO CONDUCT THE MINIMUM
 STRUCTURAL INSPECTION OF THRESHOLD BUILDINGS REQUIRED
 BY THIS CODE, SECTION 553.73,FLORIDA STATUTES, WITHOUT
 DUPLICATIVE INSPECTION BY THE BUILDING DEPARTMENT. THE
 BUILDING OFFICIAL IS RESPONSIBLE FOR ENSURING THAT ANY
 PERSON CONDUCTING INSPECTIONS IS QUALIFIED AS A
 BUILDING INSPECTOR UNDER PART XII OF CHAPTER 468,
 FLORIDA STATUTES, OR CERTIFIED AS A SPECIAL INSPECTOR
 UNDER CHAPTER 471 OR 481, FLORIDA STATUTES. INSPECTIONS
 OF THRESHOLD BUILDINGS REQUIRED BY SECTION 553.79(5),
 FLORIDA STATUTES, ARE IN ADDITION TO THE MINIMUM
 INSPECTIONS REQUIRED BY THIS CODE.
  
 5F) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR
 HISTORY FOR THIS TYPE OF CONSTRUCTION.
  
 5G) NOTE FOR PAPER PLANS, 3 SETS OF DOCUMENTS ARE
 REQUIRED.
  
 5H) 110.8.4.1 THE SPECIAL INSPECTOR, UPON COMPLETION OF
 THE BUILDING AND PRIOR TO THE ISSUANCE OF A CERTIFICATE
 OF OCCUPANCY OR COMPLETION, FILE A SIGNED AND SEALED
 STATEMENT WITH THE ENFORCEMENT AGENCY IN SUBSTANTIALLY
 THE FOLLOWING FORM: ?TO THE BEST OF MY KNOWLEDGE AND
 BELIEF, THE CONSTRUCTION OF ALL STRUCTURAL LOAD-BEARING
 COMPONENTS DESCRIBED IN THE THRESHOLD INSPECTION PLAN
 COMPLIES WITH THE PERMITTED DOCUMENTS AND THE SPECIALTY
 SHORING DESIGN PROFESSIONAL ENGINEER HAS ASCERTAINED
 THAT THE SHORING AND RESHORING CONFORMS WITH THE
 SHORING AND RESHORING PLANS SUBMITTED TO THE
 ENFORCEMENT AGENCY?.
  
 5I) 110.8.4.2 ANY PROPOSAL TO INSTALL AN ALTERNATE
 STRUCTURAL PRODUCT OR SYSTEM TO WHICH BUILDING CODES
 APPLY SHALL BE SUBMITTED TO THE ENFORCEMENT AGENCY FOR
 REVIEW FOR COMPLIANCE WITH THE CODES AND MADE PART OF
 THE ENFORCEMENT AGENCY?S RECORDED SET OF PERMIT
 DOCUMENTS. 110.8.4.3 ALL SHORING AND RESHORING
 PROCEDURES, PLANS AND DETAILS SHALL BE SUBMITTED TO THE
 ENFORCEMENT AGENCY FOR RECORDKEEPING. EACH SHORING AND
 RESHORING INSTALLATION SHALL BE SUPERVISED, INSPECTED
 AND CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.
  
 5J) NOTICE ONLY. PLEASE CALL THE CITY OF WEST PALM
 BEACH?S AUTOMATED TELEPHONE INSPECTION LINE @ (561)
 805-6700 FOR ALL BUILDING INSPECTIONS FOR AUDITING OF
 JOBSITE INSPECTIONS. WITHOUT THIS, THE PERMIT EXPIRES
 IN 6 MONTHS WHICH WILL RESULT IN ?ABANDONMENT OF JOB,
 NO INSPECTION ACTIVITY?. THE PERMIT WILL BECOME
 EXPIRED.
  
 6) PLEASE PROVIDE A SITE PLAN SHOWING THE PROPERTY
 LINES, CONSTRUCTION AREAS, STAGING AREAS, HOW THE
 OCCUPANTS OF THE BUILDING WILL BE SAFE GUARDED DURING
 THEIR ENTRY AND EXIT OF THE BUILDING. SHOW COMPLIANCE
 WITH 2017 FBC 3306.7 ? COVERED WALKWAYS, INCLUDING
 LIGHTING FOR THE EXIT DISCHARGE. 2017 FBC 3310.2 ?
 MAINTENANCE OF MEANS OF EGRESS. REQUIRED MEANS OF
 EGRESS SHALL BE MAINTAINED AT ALL TIMES DURING
 CONSTRUCTION, DEMOLITION, REMODELING OR ALTERATIONS AND
 ADDITIONS TO ANY BUILDING.
  
 7) IF SCAFFOLDING EXTENDS BEYOND THE PRIVATE PROPERTY
 AND ONTO CITY RIGHT OF WAY, ENGINEERING WILL REQUIRE
 RIGHT OF WAY PERMITS. PLEASE CONTACT MANNY GONZALEZ AT
 THE ENGINEERING DEPARTMENT ? (561) 494-1085.
  
 7A) IF SCAFFOLDING IS TO BE USED FOR THE HEIGHT OF THE
 BUILDING, PLEASE PROVIDE HOW THE SCAFFOLDING WILL BE
 BUILT AS WELL AS HOW THE SCAFFOLDING WILL BE ANCHORED
 TO RESIST LATERAL WIND LOADING. THERE APPEARS SAND
 BLASTING WILL ALSO BE AN ELEMENT OF THE OPERATION, HOW
 WILL THE SANDBLASTING BE CONTAINED, WILL THERE BE WIND
 SCREENS, IF SO PLEASE PROVIDE WHAT ELEMENTS WILL BE
 INTRODUCED TO KEEP THE SCAFFOLDING FROM MOVING WITH THE
 LATERAL EFFECT OF WIND ON THE WIND SCREENS. AT WHAT
 SAFETY FACTOR ARE THESE ELEMENTS FACTORED? 107.2.1.3
