 TIMES FOR FAILURE TO CORRECT A CODE VIOLATION
 SPECIFICALLY AND CONTINUOUSLY NOTED IN EACH REJECTION,
 INCLUDING, BUT NOT LIMITED TO, EGRESS, FIRE PROTECTION,
 STRUCTURAL STABILITY, ENERGY, ACCESSIBILITY, LIGHTING,
 VENTILATION, ELECTRICAL, MECHANICAL, PLUMBING, AND GAS
 SYSTEMS, OR OTHER REQUIREMENTS IDENTIFIED BY RULE OF
 THE FLORIDA BUILDING COMMISSION ADOPTED PURSUANT TO
 CHAPTER 120, THE LOCAL GOVERNMENT SHALL IMPOSE, EACH
 TIME AFTER THE THIRD SUCH REVIEW THE PLANS ARE REJECTED
 FOR THAT CODE VIOLATION, A FEE OF FOUR TIMES THE AMOUNT
 OF THE PROPORTION OF THE PERMIT FEE ATTRIBUTED TO PLANS

 PROJECT PER THE SIGNED CONTRACT SUBMITTED. A CHANGE OF
 CONTRACTOR FORM IS REQUIRED FOR ALL TRADES.
  
 2) 2ND REQUEST: PROVIDE EXISTING FLOOR PLAN AS PER CITY
 RECORDS UNDER PERMIT # 17030482 AND INSPECTORS PHOTOS
 THERE IS SIGNIFICANT CHANGE TO THE FLOOR PLAN. FBC
 107.3.5. OWNER PURCHASED HOME ON 7/16/18, YOU CONTRACT
 LIST YOUR FIRST SITE VISIT AS 10/1/18. THE FIELD
 INSPECTOR FOUND WORKERS ON-SITE WORKING. ANY AND ALL
 WORK UNPERMITTED WORK IS THE RESPONSIBILITY OF THE
 CURRENT OWNER. PROVIDE AN EXISTING FLOOR PLAN AND A
 PROPOSED FLOOR PLAN.FBC 107.3.5
  
 3) PROVIDE A COMPLETE SANITARY RISER ISOMETRIC SHOWING
 ALL NEW WORK. FBC 107.3.5
 (A) CONSTRUCTION DOCUMENTS SHALL BE DIMENSIONED AND
 DRAWN UPON SUITABLE MATERIAL. RISER ISOMETRIC IS IN
 PENCIL.PENCIL IS NOT PERMITTED. FBC 107.2.1

 PROVIDE DRAWING OF PAN SHOWING PRE-PITCH @ 2% AND SHOW

 5) 2ND REQUEST:EXPOSE ALL WORK REQUIRING INSPECTION. IT
 SHALL BE THE DUTY OF THE PERMIT APPLICANT TO CAUSE THE
 WORK TO REMAIN ACCESSIBLE AND EXPOSED FOR INSPECTION
 PURPOSES. WPB AMEND TO THE FBC 110.1
