 OF THE CITY OF WEST PALM BEACH CODE OF ORDINANCES, THE
 CITY OF WEST PALM BEACH IS REQUESTING THAT THIS
 PROPOSED SMALL WIRELESS FACILITY BE MOVED TO ANOTHER
 LOCATION IN THE RIGHT-OF-WAY AND PLACED ON AN
 ALTERNATIVE UTILITY POLE OR ON A NEW UTILITY POLE TO BE
 INSTALLED WITHIN THE RIGHT-OF-WAY.
 - CLEMATIS IS AN IMPORTANT STREET AND ROW TO THE CITY.
 - CONFLICTS WITH PROPOSED CITY AND BRIGHTLINE BIKE
 SHARING FACILITIES.
 - SPECIAL EVENTS APPROVAL OF LOCATION NOT GRANTED,
 SUGGESTS ALTERNATIVE LOCATIONS ALONG OLIVE AND DIXIE.
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1. PROPOSED POLE LOCATION IS SUBJECT TO SPECIAL EVENTS
 APPROVAL. SPECIAL EVENTS SUGGESTS ALTERNATIVE LOCATIONS
 TO CLEMATIS ST. (OLIVE AND DIXIE).
  
 2. PROVIDED SURVEY IS NOT ACCURATE/COMPLETE. PLEASE
 PROVIDE A COMPLETE SURVEY SHOWING THE FULL SCOPE OF
 WORK.
  
 3. PROPOSED POLE LOCATION MAY CONFLICT WITH PENDING
 CITY AND BRIGHT LINE BIKE SHARING FACILITIES. MAY HAVE
 TO SHIFT OR SUGGEST ALTERNATIVE POLE LOCATION.
  
 4. DO YOU INTEND TO INSTALL A NEW UNDERGROUND ELECTRIC
 LINE OR CONNECTION? IF SO POTENTIAL CONFLICTS WITH CITY
 DECORATIVE STREET LIGHT CONNECTIONS, LANDSCAPING
 (STREET TREES), AND STRUCTURAL SOIL.
  
 5. PLEASE PROVIDE THE NECESSARY PUBLIC UTILITY LETTERS
 GRANTING APPROVAL OF THE BORE PITS, HANDHOLES, AND
 UNDERGROUND ELECTRICAL FOR THIS PROPOSED POLE LOCATION.
  
 6. PLEASE PROVIDE A TYPICAL CROSS-SECTION SHOWING THE
 DEPTH OF THE UNDERGROUND ELECTRICAL COMPONENTS.
  
 7. PLEASE NOTE: THE EXISTING DECORATIVE LIGHTS SHOWN ON
 THE SURVEY ARE 16.8 FEET TALL (CITY CODE REQUIREMENT
 FOR CLEMATIS STREET). THE SUPPORT POLE INCLUDED WITH
 THE APPLICATION IS 24 FEET IN HEIGHT, WHICH EXCEEDS THE
 ALLOWANCE LISTED IN SECTION 94-411(G)(1) OF THE CITY
 CODE.
  
 8. ANTENNA POLE COLOR AND HEIGHT SHALL BE ANALOGOUS TO
 THE EXISTING DECORATIVE CITY STREET LAMPS ALONG
 CLEMATIS ST.
  
 9. PROVIDE ADDITIONAL DETAILS ENSURING THAT THE POLE
 GROUNDING FOR THE PROPOSED LOCATION DOES NOT CONFLICT
 WITH PLANTING BEDS, STRUCTURAL SOIL, WATER METER, OR
 STREET LIGHTS.
  
 10. THE POLE STEALTH DESIGN MEETS CITY STANDARDS.
