 NARCISSUS POLE
  
 NO RESPONSE PROVIDED: IN ACCORDANCE WITH SECTIONS
 78-402(B) AND 78-411(B)(4) OF THE CITY OF WEST PALM
 BEACH CODE OF ORDINANCES, THE CITY OF WEST PALM BEACH
 IS REQUESTING THAT THIS PROPOSED SMALL WIRELESS
 FACILITY BE MOVED TO ANOTHER LOCATION IN THE
 RIGHT-OF-WAY AND PLACED ON AN ALTERNATIVE UTILITY POLE
 OR ON A NEW UTILITY POLE TO BE INSTALLED WITHIN THE
 RIGHT-OF-WAY.
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) UPDATED COMMENT: ENGINEERING PLANS
  
 A. THE ENGINEERING PLANS WERE SUBMITTED AT A 1/16TH = 1
 FOOT SCALE, WHICH DOES NOT COMPLY WITH THE CITY CODE
 SCALE REQUIREMENT OF 10FT, 20FT, OR 40FT SCALE. PROVIDE
 THE PLANS AT THE REQUIRED SCALE.
  
 B. THE 100 BLOCK OF CLEMATIS STREET IS CURRENTLY BEING
 RECONSTRUCTED. THE ENGINEERING PLANS THAT YOU SUBMITTED
 DO NOT SHOW THE FINAL RECONSTRUCTION CONDITIONS AND
 THUS DO NOT ACCURATELY SHOW THE PLACEMENT LOCATION FOR
 THE PROPOSED FACILITIES. PROVIDE ACCURATE PLANS BASED
 ON THE FINAL RECONSTRUCTION OF THE CLEMATIS STREET
 RIGHT-OF-WAY.
  
 2) REPEAT COMMENT: SURVEY OF UNDERGROUND FACILITIES -
 THE BASE OF THE POLE IS BEING INSTALLED UNDERGROUND SO
 THIS IS REQUIRED.
  
 A. AS PREVIOUSLY STATED, CLEMATIS STREET IS BEING
 RECONSTRUCTED, INCLUDING UNDERGROUND UTILITY WORK AS
 WELL AS THE USE OF A SUSPENDED PAVEMENT SYSTEM TO
 PERMIT AND EXPANDED TREE ROOT GROWTH. ONCE THE
 RECONSTRUCTION IS COMPLETED, PROVIDE A SURVEY SHOWING
 ALL UNDERGROUND UTILITIES AND FEATURES, SUCH AS THE
 SUSPENDED PAVEMENT SYSTEM.
  
 B. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 3) REPEAT COMMENT: SURVEY OF ABOVEGROUND FACILITIES - A
 NEW POLE IS BEING INSTALLED ABOVEGROUND SO THIS IS
 REQUIRED.
  
 A. AS PREVIOUSLY STATED, CLEMATIS STREET IS BEING
 RECONSTRUCTED. ONCE THE RECONSTRUCTION IS COMPLETED,
 PROVIDE A SURVEY SHOWING ALL ABOVEGROUND FEATURES,
 INCLUDING LANDSCAPE WITH THE PROPOSED POLE NOT
 INTERFERING WITH ABOVE GROUND FACULTIES, INCLUDING
 LANDSCAPE.
  
 B. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 C. PLEASE NOTE THAT THE OUTDATED SURVEY DOES NOT
 PROVIDE THE SIZE OF THE EXISTING UTILITY POLE BASE
 DIAMETER, WHICH WILL NEED TO BE CORRECTED ON THE
 REVISED SURVEY.
  
 D. IDENTIFY ALL ABOVEGROUND COMMUNICATION FACILITIES IN
 THE AREA TO INCLUDE AN INVENTORY OF THE REGISTRANT'S
 EXISTING FACILITIES - NO INFORMATION ON EXISTING
 FACILITIES WAS PROVIDED. SHOW THIS INFORMATION ON THE
 PLANS AND PROVIDE A NARRATIVE. IF THERE ARE NO
 FACILITIES, THIS SHALL BE NOTED ON THE SURVEY.
  
 E. PLEASE NOTE: THE EXISTING DECORATIVE LIGHT SHOWN ON
 THE SURVEY IS 18 FEET TALL; HOWEVER, THIS MAY CHANGE
 WITH THE RECONSTRUCTION OF CLEMATIS STREET. THE SUPPORT
 POLE INCLUDED WITH THE APPLICATION IS 27 FEET IN
 HEIGHT, WHICH EXCEEDS THE ALLOWANCE LISTED IN SECTION
 94-411(G)(1) OF THE CITY CODE. THE FINAL CITY LIGHT
 POLE HEIGHT SHALL BE USED FOR THE FINAL SUPPORT POLE
 HEIGHT.
  
 4) UPDATED COMMENT: CONCEALMENT METHODS DESCRIPTIONS -
 WHILE THE POLE INCLUDES THE CABLES INSIDE THE POLE AND
 A SHEATHED ANTENNA, IT SHOWS FUTURE ANTENNAS THAT ARE
 NOT CONCEALED AND A DETACHED EQUIPMENT CABINET. NEITHER
 OF THESE IS IN CONFORMANCE WITH THE CITY CONCEALMENT
 STANDARDS OUTLINED IN SECTION 78-411 OF THE CITY CODE.
 PROVIDE PLANS AND NARRATIVE ON HOW THE INSTALLATION
 COMPLIES WITH THE CITY'S CONCEALMENT REQUIREMENT.
  
 5) UPDATED COMMENT: RESTORATION ESTIMATE - YOU MUST
 PROVIDE A RESTORATION ESTIMATE FOR THE SPECIFIC
 LOCATION OF THE POLE, NOT A GENERIC ESTIMATE THAT IS
 NOT LINKED TO THE SPECIFIC LOCATION AND WORK TO BE
 DONE. THE CLEMATIS STREET RECONSTRUCTION WILL DICTATE
 WHAT RESTORATION IS REQUIRED.
  
 6) REPEAT COMMENT: RADIO FREQUENCY INTERFERENCE - NO
 INFORMATION WAS PROVIDED THAT THE PROPOSED FACILITIES
 WILL NOT CAUSE RADIO FREQUENCY INTERFERENCE WITH THE
 CITY'S PUBLIC SAFETY COMMUNICATIONS EQUIPMENT. PROVIDE
 A STATEMENT REGARDING RADIO FREQUENCY INTERFERENCE.
  
 7) UPDATED COMMENT: EVIDENCE OF INSURANCE COVERAGE
  
 A. THE CERTIFICATE OF LIABILITY INSURANCE LISTS ONLY
 THE CITY OF WEST PALM BEACH AS THE ADDITIONAL INSURED.
 IN ACCORDANCE WITH SECTION 78-406(A) OF THE CITY CODE
 OF ORDINANCES, THE CERTIFICATE OF LIABILITY INSURANCE
 NEEDS TO BE REVISED TO NAME THE CITY OF WEST PALM
 BEACH, ITS COMMISSIONERS, OFFICERS, EMPLOYEES AND
 AGENTS AS ADDITIONAL INSURED. NOTE: SINCE THERE ARE
 ADDITIONAL INSURED, THE "ADDITIONAL INSURED" COLUMN
 NEEDS TO BE CHECKED OFF.
  
 B. THE CERTIFICATE OF LIABILITY INSURANCE STATES THAT
 THE INSTALLATION IS IN ST. LUCIE COUNTY. THE WORK IS
 ACTUALLY WITHIN WEST PALM BEACH AND THE CERTIFICATE OF
 LIABILITY INSURANCE NEEDS TO BE CHANGED TO REFLECT
 THIS.
  
 C. THE CERTIFICATE OF LIABILITY INSURANCE EXPIRED ON
 JUNE 30, 2019. YOU MUST PROVIDE AN ACTIVE CERTIFICATE
 OF LIABILITY INSURANCE.
  
 D. THE PROVIDED CERTIFICATE OF LIABILITY INSURANCE IS
 FOR VERIZON WIRELESS, LLC. SINCE THE COMMUNICATIONS
 FACILITIES PLACEMENT PERMIT APPLICATION LISTS VERIZON
 WIRELESS PERSONAL COMMUNICATIONS LP AS THE REGISTRANT,
 THIS IS WHO NEEDS TO BE THE INSURED ON THE CERTIFICATE
 OF LIABILITY INSURANCE.
  
 E. ONCE THE CORRECT CERTIFICATE OF LIABILITY INSURANCE
 HAS BEEN SUBMITTED, THE CITY'S RISK MANAGEMENT
 DEPARTMENT WILL REVIEW TO ENSURE THAT IT COMPLIES WITH
 CITY POLICY.
  
 8) REPEAT COMMENT: START OF OPERATION - AN ATTESTATION
 FROM THE COMMUNICATIONS FACILITIES SERVICE PROVIDER
 THAT COMMUNICATIONS SERVICES FACILITIES WILL BE
 INSTALLED AND OPERATIONAL ON THE PROPOSED STRUCTURE NO
 LATER THAN NINE MONTHS AFTER THE DATE THE APPLICATION
 IS APPROVED IS REQUIRED. PROVIDE THE REQUIRED
 ATTESTATION.
  
 9) REPEAT COMMENT: PUBLIC UTILITY LETTERS, TELEPHONE,
 NATURAL GAS, ELECTRIC SERVICE, AND TELEVISION CABLE -
 THE PUBLIC UTILITY LETTERS WERE NOT PROVIDED. PROVIDE
 COPIES OF THE PUBLIC UTILITY LETTERS.
  
 10) REPEAT COMMENT: ELECTRONIC FORMAT OF SUBMITTAL -
 THE SUBMITTAL WAS NOT PROVIDED IN ELECTRONIC FORM. A
 COPY OF ALL OF THE SUBMITTAL MATERIAL MUST BE SUBMITTED
