 CONSTRUCTION, OR THE RENOVATION OR IMPROVEMENT OF AN
 EXISTING BUILDING, WHERE TOTAL CONSTRUCTION COSTS ARE
 EQUAL TO OR GREATER THAN $750,000.00. ALL BUILDINGS
 WITHIN NEW PLANNED DEVELOPMENTS SHALL BE ASSESSED
 CUMULATIVELY TOWARDS THIS ART IN PUBLIC PLACES
 REQUIREMENT, EVEN IF THE BUILDINGS ARE PERMITTED

 FOR THE CONSTRUCTION OF $23,760,506.00 IS LOW. THE
 CALULATED VALUE FOR THE BUILDING IS $26,297,570.00. FBC
 109.3 BUILDING PERMIT VALUATION.
 IF, IN THE OPINION OF THE BUILDING OFFICIAL, THE
 CLAIMED VALUATION OF BUILDING, ALTERATION, STRUCTURE,
 ELECTRICAL, GAS, MECHANICAL, OR PLUMBING SYSTEMS
 APPEARS TO BE UNDER ESTIMATED ON THE APPLICATION, THE
 PERMIT SHALL BE DENIED. FOR PERMITTING PURPOSES,

 LANDSCAPE, IRRIGATION, AUGER CAST PILE AND OR
 FOUNDATION OR STRUCTURE ONLY PERMIT WILL BE SEPARATE
 REVIEWS AND ASSOCIATED FEES WITH EACH PERMIT.
 PLEASE PROVIDE AN ITEMIZED LIST AND SUBTRACT THESE LINE
 ITEMS AND THEIR VALUE FROM THE OVERALL BUILDING VALUE
 STATED ON THE PERMIT APPLICATION FOR THIS PROJECT. THE
 REMAINING BALANCE WILL EFFECTIVELY BE THE VALUE OF THE

 ASSEMBLY OCCUPANCY CLASSIFICATION AS DEFINED IN THE
 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN

 A) FBC (CITY AMENDMENTS) 110.8.1 THRESHOLD INSPECTION
 PLAN. THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OF RECORD. THE STRUCTURAL
 INSPECTION PLAN MUST BE SUBMITTED TO THE ENFORCING
 AGENCY PRIOR TO THE ISSUANCE OF A BUILDING PERMIT FOR
 THE CONSTRUCTION OF A THRESHOLD BUILDING. THE PURPOSE
 OF THE STRUCTURAL INSPECTION PLAN IS TO PROVIDE
 SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO THAT
 THE BUILDING CAN BE ADEQUATELY INSPECTED FOR COMPLIANCE
 WITH THE PERMITTED DOCUMENTS.

 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE.
  
 C) THRESHOLD BLDG. REQUIRED STATEMENT: W.P.B. AMENDMENT
 110.8.4.4 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE MINIMUM
 BUILDING CODES AND THE APPLICABLE FIRE-SAFETY STANDARDS
 AS DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND CHAPTER 633 FLORIDA STATUTES.
  
 D) FOR THE SPECIAL INSPECTOR PLEASE PROVIDE A RESUME OF
 INSPECTION EXPERIENCE ON PREVIOUS PROJECT. THE RESUME
 MUST BE ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED
 HEREIN. THE BUILDING OFFICIAL OR HIS DESIGNEE WILL
 REVIEW THE RESUME. AFTER THE RESUME IS REVIEWED, AN
 INTERVIEW WILL BE SCHEDULED. UNDER NO CIRCUMSTANCES
 WILL AN INSPECTOR BE INTERVIEWED FOR WORK WITHOUT FIRST
 MEETING THE ABOVE CRITERIA.
  
 E) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR HISTORY
 FOR THIS TYPE OF CONSTRUCTION.
  
 F) NOTE: FOR DIGITAL PLANS, AFTER PERMIT ISSUANCE,
 PLEASE E-MAIL THE PERMIT LIBRARIAN OF THE PERMIT BEING
 ISSUED, THE PERMIT LIBRARIAN WILL THEN E-MAIL YOU THE
 APPROVED AND BATCH STAMPED PLANS SO YOU MAY PRINT YOUR
 PLANS, TWO SETS OF PLANS WILL NEED TO BE PRINTED IN
 COLOR, (1) FOR THE THRESHOLD INSPECTOR, THE OTHER FOR
 THE MEP INSPECTORS OTHER PLANS YOU WISH TO PRINT MAYBE
 IN BLACK & WHITE.
  
 G) 110.8.4.3 ALL SHORING AND RESHORING PROCEDURES,
 PLANS AND DETAILS SHALL BE SUBMITTED TO THE ENFORCEMENT
 AGENCY FOR RECORDKEEPING. EACH SHORING AND RESHORING
 INSTALLATION SHALL BE SUPERVISED, INSPECTED AND
 CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.
  
 4) ON PAGE 026, SHEET S000, ASCE 7-16 IS INCORRECTLY
 SPECIFIED. CHAPTER 35 OF THE 2017 FBC REFERENCES ASCE
 7-10.
  
 5) ON PAGE 005, SHEET G100, THE 2015 FBC IS SPECIFIED
 AND THIS PROJECT IS UNDER THE 2017 FBC.
  
 6) ON PAGE 005, SHEET G100, THE CODE SECTIONS ARE
 SPECIFIED AS IBC 2015 RATHER THAN FBC 2017.
  
 7) BASED ON THE SURVEY THAT WAS SUBMITTED THER ARE TWO
 BUILDINGS ON THE SAME LOT THAT ARE ADJACENT TO THE
 PROPOSED BUILDING WHICH WILL REQUIRE AN IMAGINARY LINE
 TO BE ESTABLISHED BETWEEN THE BUILDINGS TO DETERMINE
 THE FIRE SEPARATION DISTANCE. THE FIRE SEPARATION
 DISTANCE IS REQUIRED IN ORDER TO DETERMINE THE EXTERIOR
 WALL, AREA OF OPENINGS PERMITTED AND OPENING
 PROTECTIONS. THE EXISTING BUILDINGS WILL HAVE TO
 COMPLY, AS WELL AS, THE PROPOSED BUILDING. PROVIDE
 PLANS AND CALCULATIONS TO SHOW COMPLIANCE. FBC

 FOR THE PURPOSES OF DETERMINING THE REQUIRED WALL AND
 OPENING PROTECTION, PROJECTIONS AND ROOF-COVERING
 REQUIREMENTS, BUILDINGS ON THE SAME LOT SHALL BE
 ASSUMED TO HAVE AN IMAGINARY LINE BETWEEN THEM.
  
 WHERE A NEW BUILDING IS TO BE ERECTED ON THE SAME LOT

 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E 492.
  
 B) FBC-B 2014 1207.2 AIR-BORNE SOUND.
 WALLS, PARTITIONS AND FLOOR/CEILING ASSEMBLIES
 SEPARATING DWELLING UNITS FROM EACH OTHER OR FROM
 PUBLIC OR SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION
 CLASS (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED)
 FOR AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM
 E 90.
  
 C) FLAME SPREAD- FBC-B 2014 603.1. EXCEPTION 2.
 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A

 IN WIND-BORNE DEBRIS REGIONS, GLAZED OPENINGS IN
 BUILDINGS SHALL BE IMPACT RESISTANT OR PROTECTED WITH
 AN IMPACT-RESISTANT COVERING MEETING THE REQUIREMENTS
 OF ANSI/DASMA 115 (FOR GARAGE DOORS AND ROLLING DOORS)
 OR TAS 201, 202 AND 203, AAMA 506, ASTM E1996 AND ASTM
 E1886 REFERENCED HEREIN, OR AN APPROVED
 IMPACT-RESISTANT STANDARD AS FOLLOWS:
  
 1.GLAZED OPENINGS LOCATED WITHIN 30 FEET (9144 MM) OF
 GRADE SHALL MEET THE REQUIREMENTS OF THE LARGE MISSILE
 TEST OF ASTM E1996.
  
 2.GLAZED OPENINGS LOCATED MORE THAN 30 FEET (9144 MM)
 ABOVE GRADE SHALL MEET THE PROVISIONS OF THE SMALL
 MISSILE TEST OF ASTM E1996.
  
 23) 1609.1.2.1 LOUVERS.
 LOUVERS PROTECTING INTAKE AND EXHAUST VENTILATION DUCTS
 NOT ASSUMED TO BE OPEN THAT ARE LOCATED WITHIN 30 FEET
 (9144 MM) OF GRADE SHALL MEET THE REQUIREMENTS OF AMCA
 540 OR SHALL BE PROTECTED BY AN IMPACT-RESISTANT COVER
 COMPLYING WITH THE LARGE MISSILE TEST OF ASTM E1996 OR
 AN APPROVED IMPACT-RESISTANCE STANDARD. LOUVERS
 REQUIRED TO BE OPEN FOR LIFE SAFETY PURPOSES SUCH AS
 PROVIDING A BREATHABLE ATMOSPHERE SHALL MEET THE
 REQUIREMENTS OF AMCA 540.
  
 24) WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A
 RESPONSE LETTER ADDRESSING EACH ITEM ALONG WITH THE
