 THE FOLLOWING ITEMS ARE REQUIRED PRIOR TO ISSUANCE OF A
 DEMO PERMIT:
 1. FBC B 107.2.1, PROVIDE ARCHITECT/ENGINEER APPROVAL
 LETTER FOR FOOTINGS SUPPORTING EXISTING BUILDING. CITY
 INSPECTORS WHO HAVE VISITED THE SITE HAVE CONCERNS
 ABOUT THE STRUCTURAL INTEGRITY OF THE EXISTING
 STRUCTURE AND HOW IT MAY HAVE BEEN COMPROMISED BY
 UNPERMITTED WORK. CALCULATIONS MAY BE REQUIRED.
 2. FBC B 105.4.1.2, SEE EXPIRED PERMIT APPLICATION
 14040355. IT APPEARS THAT GAS WORK WAS DONE WITHOUT A
 PERMIT. IF WORK HAS BEEN REMOVED, A GAS PERMIT IS
 REQUIRED BY A LICENSED CONTRACTOR SO PROPER AND SAFE
 REMOVAL CAN BE CONFIRMED BY THE LICENSED CONTRACTOR AND
 INSPECTED BY THE CITY INSPECTOR. THIS CAN BE DONE AS A
 SUB PERMIT TO THIS MASTER PERMIT. THE SUB PERMIT CAN BE
 EMAILED TO [email protected]; THE ORIGINAL WILL BE
 REQUIRED AT TIME OF PICKUP. REVISE PLAN TO INDICATE
 REMOVAL. PLEASE INCLUDE A REFERENCE TO THIS PERMIT
 NUMBER IN THE EMAIL SUBJECT LINE.
 IF GAS WORK PROPOSED UNDER 14040355 WAS INSTALLED AND
 IS TO REMAIN, PROVIDE A PLAN AND GAS PERMIT
 APPLICATION.
 3. FBC B 105.4 & FBC B 105.5, PLEASE ADVISE STATUS OF
 THE GREASE TRAP INSTALLED UNDER PERMIT 15070296. IF
 THIS IS STILL INSTALLED, APPLY FOR A ?FINAL INSPECTION
 ONLY? PERMIT AT THE BUILDING DIVISION. THE DESCRIPTION
 WILL BE ?FINAL INSPECTION ONLY TO CLOSE OUT EXPIRED
 PERMIT 15070296?. A CONTRACTOR IS NOT REQUIRED IF ALL
 WORK WAS PREVIOUSLY PERFORMED BY THE LICENSED
 CONTRACTOR.
 IF THE GREASE TRAP WAS REMOVED, REVISE PLAN TO INDICATE
 SUCH.
 4. EXTERMINATION LETTER- PROVIDE A LETTER FROM A
 PROFESSIONAL EXTERMINATING COMPANY STATING THAT THE
 PROPERTY HAS BEEN INSPECTED AND OR TREATED FOR RODENTS.
 5. COMPLETE THE DEMO DEBRIS FORM AND PAY THE FEES,
 ?DEMO DEBRIS DISPOSAL FEE?

 AFTER THE FORM IS SENT TO [email protected], THE FEES
 WILL BE ADDED TO YOUR PERMIT. YOU CAN THEN PAY THE FEES
 ONLINE PRIOR TO PERMIT ISSUANCE.
 6. FBC B 105.4 & FBC B 105.5, THERE ARE TWO EXPIRED
 PERMITS FOR THIS ADDRESS; 14030860 MECHANICAL HOOD, AND
 14030861, HOOD SUPPRESSION. IF WORK HAS BEEN REMOVED,
 REVISE PLAN
 TO INCORPORATE SCOPE INTO THIS PERMIT. IF WORK HAS NOT
 BEEN REMOVED, APPLY FOR NEW PERMITS SO THE WORK CAN BE
 INSPECTED AND FINALED.
 7. FBC EB 504/505, THIS STRUCTURE IS LOCATED IN A
 SPECIAL FLOOD HAZARD AREA WITH A BFE OF 12.1, FREEBOARD
 OF 2?. IT IS NOT CLEAR IF THE IMPROVEMENTS DONE TO THIS
 STRUCTURE WILL CONSTITUTE A ?SUBSTANTIAL IMPROVEMENT?.
