 PLEASE PROVIDE WRITTEN RESPONSES TO COMMENTS.
  
 1) THE PLANS SHOW THAT THE POLE ON WHICH THE ANTENNAS
 ARE PROPOSED TO BE MOUNTED STRADDLES THE
 RIGHT-OF-WAY/PROPERTY LINE. PLEASE PROVIDE VERIFICATION
 IN THE FORM OF A SURVEY TO VERIFY THE EXACT LOCATION OF
 THE PROPERTY LINE IN RELATIONSHIP TO THE POLE. STAFF IS
 BASING THE COMMENTS ON THE POLE PARTIALLY ON PRIVATE
 PROPERTY.
  
 2) PROVIDE CONSENT FROM FPL, THE OWNER OF THE UTILITY
 POLE, THAT PERMITS THE PLACEMENT OF THE ANTENNAS AND
 RELATED EQUIPMENT ON IT.
  
 3) THE ANTENNAS ARE LOCATED ON THE PORTION OF THE POLE
 THAT IS ON PRIVATE PROPERTY; THEREFORE, THIS IS A
 WIRELESS COMMUNICATION FACILITY THAT IS NOT LOCATED
 WITHIN THE RIGHT-OF-WAY AND REQUIRES THE APPROVAL OF A
 CITY PRIVATE PROPERTY WIRELESS COMMUNICATION PERMIT IN
 ACCORDANCE WITH ARTICLE X OF THE CITY'S ZONING AND LAND
 DEVELOPMENT REGULATIONS. PLEASE CONTACT ME FOR A COPY
 OF THE WIRELESS COMMUNICATION PERMIT APPLICATION.
  
 4) THE CITY REQUIRES THAT THE EQUIPMENT CABINETS
 (UTILITY METER, DISCONNECT AND NEMA BOX) BE SCREENED,
 IN THIS CASE WITH A DECORATIVE WRAP. PLEASE SHOW HOW
 THIS WILL OCCUR. NOTE: WHILE THE PLANS CALL THE
 EQUIPMENT CABINETS' HEIGHT AT A MAXIMUM OF SIX (6)
 FEET, PLEASE CONSIDER HAVING THEM MOUNTED ON A SHORTER
 POLE TO MINIMIZE THE VISUAL IMPACT.
