 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THE FOUNDATION PERMIT IS TYPICALLY NOT ISSUED UNTIL
 THE PLANS FOR THE ENTIRE BUILDING HAS BEEN SUBMITTED
 AND REVIEWED. THERE IS INFORMATION THAT IS MISSING FROM
 THIS SUBMITTAL THAT IS REQUIRED IN ORDER TO PREFORM A
 COMPLETE REVIEW FOR A FOUNDATION PERMIT. FBC 107.2.1
 A) THE TYPE OF CONSTRUCTION AND OCCUPANCY TYPE ARE NOT
 SPECIFIED.
 B) THE LIFE SAFETY PLANS WERE NOT SUBMITTED.
 C) THE PARKING GARAGE IS NOT IDENTIFIED AS OPEN OR
 ENCLOSED. (PROVIDE CALCULATIONS IF OPEN.)
 D) PROVIDE ADDITIONAL INFORMATION FOR THE SWIMMING POOL
 THAT APPEARS TO BE ON THE ROOF.
  
 2) THIS PROJECT MEETS THE PARAMETERS TO BE CLASSIFIED
 AS A THRESHOLD BUILDING. THRESHOLD BUILDING / FL S. S.
 553.71(7) "THRESHOLD BUILDING" MEANING ANY BUILDING
 WHICH IS GREATER THAN (3) STORIES OR 50 FT. IN HEIGHT,
 OR WHICH HAS AN ASSEMBLY OCCUPANCY CLASSIFICATION AS
 DENIED IN THE FLORIDA BUILDING CODE WHICH EXCEEDS 5,000
 SQ. FT. IN AREA AND AN OCCUPANT CONTENT OF GREATER THAN
 500 PERSONS.
  
 A) FBC (CITY AMENDMENTS)110.8.1 THRESHOLD INSPECTION
 PLAN. THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OF RECORD. THE STRUCTURAL
 INSPECTION PLAN MUST BE SUBMITTED TO THE ENFORCING
 AGENCY PRIOR TO THE ISSUANCE OF A BUILDING PERMIT FOR
 THE CONSTRUCTION OF A THRESHOLD BUILDING. THE PURPOSE
 OF THE STRUCTURAL INSPECTION PLAN IS TO PROVIDE
 SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO THAT
 THE BUILDING CAN BE ADEQUATELY INSPECTED FOR COMPLIANCE
 WITH THE PERMITTED DOCUMENTS.
  
 B) THRESHOLD INSPECTOR: WPB AMENDMENT 110.8.1 W.P.B.
 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE.
  
 C) THRESHOLD BLDG. REQUIRED STATEMENT: W.P.B. AMENDMENT
 110.8.4.4 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE MINIMUM
 BUILDING CODES AND THE APPLICABLE FIRE-SAFETY STANDARDS
 AS DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND CHAPTER 633 FLORIDA STATUTES.
  
 D) FOR THE SPECIAL INSPECTOR PLEASE PROVIDE A RESUME OF
 INSPECTION EXPERIENCE ON PREVIOUS PROJECT. THE RESUME
 MUST BE ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED
 HEREIN. THE BUILDING OFFICIAL OR HIS DESIGNEE WILL
 REVIEW THE RESUME. AFTER THE RESUME IS REVIEWED, AN
 INTERVIEW WILL BE SCHEDULED. UNDER NO CIRCUMSTANCES
 WILL AN INSPECTOR BE INTERVIEWED FOR WORK WITHOUT FIRST
 MEETING THE ABOVE CRITERIA.
  
 E) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR HISTORY
 FOR THIS TYPE OF CONSTRUCTION.
  
 F) NOTE: FOR DIGITAL PLANS, AFTER PERMIT ISSUANCE,
 PLEASE E-MAIL THE PERMIT LIBRARIAN OF THE PERMIT BEING
 ISSUED, THE PERMIT LIBRARIAN WILL THEN E-MAIL YOU THE
 APPROVED AND BATCH STAMPED PLANS SO YOU MAY PRINT YOUR
 PLANS, TWO SETS OF PLANS WILL NEED TO BE PRINTED IN
 COLOR, (1) FOR THE THRESHOLD INSPECTOR, THE OTHER FOR
 THE MEP INSPECTORS OTHER PLANS YOU WISH TO PRINT MAYBE
 IN BLACK & WHITE.
  
 G) 110.8.4.3 ALL SHORING AND RESHORING PROCEDURES,
 PLANS AND DETAILS SHALL BE SUBMITTED TO THE ENFORCEMENT
 AGENCY FOR RECORDKEEPING. EACH SHORING AND RESHORING
 INSTALLATION SHALL BE SUPERVISED, INSPECTED AND
 CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.
  
 H) PLEASE CALL THE CITY OF WEST PALM BEACH'S AUTOMATED
 TELEPHONE INSPECTIONS LINE (561) 805-6700 FOR ALL
 BUILDING INSPECTIONS FOR AUDITING OF JOBSITE
 INSPECTIONS WITHOUT THIS THE PERMIT EXPIRES IN 6
 MONTHS, ABANDONMENT OF JOB, NO INSPECTION ACTIVITY.
  
 I) 110.8 THRESHOLD BUILDINGS, OPERATING GUIDELINES FOR
 THRESHOLD INSPECTORS #25. ASSURE THAT THE DESIGNER HAS
 REQUIRED ON THE PLANS OR SPECIFICATIONS ONE MATERIAL,
 APPROVED FOR FIRE STOPPING PENETRATIONS, FOR WALLS AND
 CEILINGS.

 BE HAPPY TO SEND YOU A COPY OF THE OCTOBER 2018
 GUIDELINES AND POLICIES FOR PROJECTS CONCERNING
 THRESHOLD/SPECIAL INSPECTORS AND RESIDENT INSPECTORS.
  
 3) SHOW COMPLIANCE WITH ART IN PUBLIC PLACES
 THE CITY OF WEST PALM BEACH ART IN PUBLIC PLACES
 ORDINANCE REQUIREMENT THAT APPLIES TO ANY REMODEL OR
 NEW CONSTRUCTION PROJECT FOR WHICH THE PERMIT
 APPLICATION WAS SUBMITTED AFTER 6/29/14, AND THE TOTAL
 PROJECT CONSTRUCTION COSTS ARE EQUAL TO OR GREATER THAN
 $750,000 WILL NEED TO COMPLY WITH THIS ORDINANCE.
 PER CITY OF WEST PALM BEACH ORDINANCE NO. 4504-14, AND
 CITY OF WEST PALM BEACH CODE OF ORDINANCES, ARTICLE V,
 ART IN PUBLIC PLACES, SECTION 78-123(A) AND SECTION
 78-126(A), THE ART ASSESSMENT IS 1% OF THE TOTAL
 PROJECT CONSTRUCTION COST.
 PER CITY OF WEST PALM BEACH CODE OF ORDINANCES, ARTICLE
 V, ART IN PUBLIC PLACES, SECTION 78-122, RENOVATION AND
 NEW CONSTRUCTION PROJECTS ARE ASSESSED CUMULATIVELY, SO
 THE ART ASSESSMENT WILL BE APPLICABLE TO ALL FUTURE
 PROJECTS.
  
 4) PROVIDE A COPY OF THE SIGNED AND SEALED SOILS
 REPORT. FBC 1803.1
  
 5) IT APPEARS FROM THE SITE PLAN AND THE PALM BEACH
 COUNTY PROPERTY APPRAISER WEBSITE THAT THE PROJECT
 COVERS TWO LOTS. A UNITY OF TITLE IS REQUIRED:

 6) PORTIONS OF THE BUILDING ARE LOCATED IN A SPECIAL
 FLOOD HAZARD AREA. THE ELEVATIONS FOR THE PROJECT ARE
 IN NGVD 88. AS OF OCTOBER 5TH, 2017 THE CITY OF WEST
 PALM BEACH HAS ADOPTED NEW FLOOD INSURANCE RATE MAPS
 WHICH USE NAVD 88 AS THE VERTICAL DATUM. THE CITY ALSO
 ADOPTED A 2' FREEBOARD WHICH REQUIRES THE FINISH FLOOR
 AND EQUIPMENT TO BE A MINIMUM OF TWO FEET ABOVE THE
 BFE. IN ORDER TO AVOID ANY CONFUSION PLEASE REVISE THE
 PLANS TO SHOW ONLY THE CURRENT FLOOD ELEVATION DATA
 (NAVD 88). ELEVATION CERTIFICATES WILL BE REQUIRED TO
 SHOW COMPLIANCE WITH THE CITY'S FLOOD POLICY. THE CITY
 REQUIRES THREE ELEVATION CERTIFICATES. FIRST, A
 PRECONSTRUCTION ELEV. CERT. BASED ON THE CONSTRUCTION
 DRAWING. SECOND, A CONSTRUCTION ELEV. CERT. BASED ON
 THE LOWEST FLOOR FORMS. THIRD A FINAL ELEV. CERT. BASED
 ON THE FINISHED BUILDING. THE CITY'S FLOOD ORDINANCE
 ARTICLE XVII - FLOOD PREVENTION AND CONTROL SEC. 94-546
 - PROVISIONS FOR FLOOD HAZARD REDUCTION.

 THE PROPERTY LINE AND BASED ON FBC TABLE 705.8 IS
 LIMITED TO AN ALLOWABLE AREA 45% OF THE EXTERIOR WALL
