 ASSEMBLY OCCUPANCY CLASSIFICATION AS DEFINED IN THE
 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN
 AREA AND HAS AN OCCUPANT CONTENT OF GREATER THAN 500
 PERSONS.
  
 I EMAILED MARIA DIAZ FROM AHS RESIDENTIAL A COPY OF THE
 THRESHOLD INSPECTOR AGREEMENT ON 2/13/18.
  
 2. SINCE ALL OF THE STRUCTURAL AND M,E,P INSPECTIONS
 WILL BE COMPLETED BY GFA FROM START TO PROJECT
 COMPLETION THIS WILL ALSO BE A PRIVATE PROVIDER PROJECT
 ONCE THE THRESHOLD INSPECTIONS ARE COMPLETED.
 CLEARLY SHOW ON THE THRESHOLD AND PRIVATE PROVIDER
 APPLICATIONS THE SCOPE OF INSPECTIONS TO BE COMPLETED
 FOR EACH PROJECT.
  
 A. WHEN A FEE OWNER OR THE FEE OWNER?S CONTRACTOR
 ELECTS TO USE A PRIVATE PROVIDER FOR INSPECTION
 SERVICES ONLY, IT IS BEST TO SUBMIT THE ?NOTICE TO
 BUILDING OFFICIAL? FORM (61G20-2.005-2002-01) AT TIME
 OF PERMIT APPLICATION. WHEN THE ?NOTICE TO BUILDING
 OFFICIAL? IS NOT SUBMITTED AT PERMIT APPLICATION, IT
 MUST BE SUBMITTED NO LESS THAN 7 BUSINESS DAYS PRIOR TO
 THE FIRST SCHEDULED INSPECTION PER 553.791 (4). THE FEE
 OWNER OR CONTRACTOR MUST SUBMIT THE ?NOTICE TO BUILDING
 OFFICIAL? TO THE BUILDING DIVISION. DEVELOPMENT
 SERVICES. THE LICENSE AND PERMIT SPECIALISTS SHALL THEN
 DIRECT THE ?NOTICE TO BUILDING OFFICIAL? TO AN
 ASSISTANT BUILDING OFFICIAL TO REVIEW THE STATUS OF THE
 FORM.
  
 B. THE ASSISTANT BUILDING OFFICIAL SHALL REVIEW THE
 APPLICANT'S SUBMITTAL FOR COMPLIANCE WITH FS 553.791;
 AND SHALL VERIFY THAT "NOTICE TO THE BUILDING OFFICIAL?
 OF USE OF PRIVATE PROVIDER (61G20-2.005-2002-01) HAS
 BEEN PROPERLY FILLED OUT, ALL QUALIFICATION STATEMENTS
 AND RESUMES ARE COMPLETE, AND PROOF OF INSURANCE AS
 OUTLINED IN 553.791 (16) HAVE BEEN SUBMITTED.
  
 3. THE CONSTRUCTION COST FOR THIS PROJECT IS OVER $5
 MILLION. PRIVATE PROVIDER SHALL HAVE A MINIMUM POLICY
 LIMIT OF $2 MILLION PER OCCURRENCE AND $4 MILLION IN
 THE AGGREGATE. FS 553.791 (16).
