 ACTION: DENIED
  
 1. REVISE PLAN TO INCLUDE ALL INFORMATION REQUIRED TO
 BE ON THE TITLE BLOCK FOR THE ARCHITECT, FLORIDA
 STATUTE 481, FLORIDA ADMINISTRATIVE CODE 61G1-16.004.
  
 2. COMMENT FROM FIRST REVIEW, NOT ADDRESSED:
  
 PLEASE IDENTIFY THE SCOPE OF WORK. THE PERMIT
 APPLICATION INDICATES REPAIRS, THIS APPEARS TO BE A
 LEVEL II ALTERATION UNDER THE WORK AREA METHOD OF THE
 2014 EXISTING BUILDING CODE. PLEASE DECLARE LEVEL OF
 ALTERATION:
  
 REPAIRS. REPAIRS, AS DEFINED IN CHAPTER 2, INCLUDE THE
 PATCHING OR RESTORATION OR REPLACEMENT OF DAMAGED
 MATERIALS, ELEMENTS, EQUIPMENT OR FIXTURES FOR THE
 PURPOSE OF MAINTAINING SUCH COMPONENTS IN GOOD OR SOUND
 CONDITION WITH RESPECT TO EXISTING LOADS OR PERFORMANCE
 REQUIREMENTS.
  
 LEVEL 1 ALTERATIONS INCLUDE THE REMOVAL AND REPLACEMENT
 OR THE COVERING OF EXISTING MATERIALS, ELEMENTS,
 EQUIPMENT, OR FIXTURES USING NEW MATERIALS, ELEMENTS,
 EQUIPMENT, OR FIXTURES THAT SERVE THE SAME PURPOSE.
  
 LEVEL 2 ALTERATIONS INCLUDE THE RECONFIGURATION OF
 SPACE, THE ADDITION OR ELIMINATION OF ANY DOOR OR
 WINDOW, THE RECONFIGURATION OR EXTENSION OF ANY SYSTEM,
 OR THE INSTALLATION OF ANY ADDITIONAL EQUIPMENT.
  
 2ND REVIEW, PROJECT DESCRIPTION IS "REPAIRS." IT IS NOT
 CLEAR WHAT IS BEING REPAIRED.
 SOME WORK WAS ALREADY DONE WITHOUT PERMIT. THE FIRST
 PLAN SUBMITTED SHOWS DEMO OF THE ACOUSTIC CEILING; NEW
 PLAN DOES NOT SHOW THIS WORK.
  
 CONFIRM THAT ALL WORK PROPOSED, INCLUDING WORK DONE
 WITHOUT PERMITS, IS INCLUDED ON THE PLAN.
  
 REVISE PLAN TO SHOW NATURE OF "REPAIRS" OR DECLARE
 LEVEL OF ALTERATION. BASED ON THE PLAN SUBMITTED, THIS
 APPEARS TO BE A LEVEL 2 ALTERATION; REVISE PLAN
 ACCORDINGLY.
  
 3. COMMENT FROM FIRST REVIEW, NOT ADDRESSED:
  
 PLEASE INSPECT BEFORE REMOVAL OF WALL, NOTE ON THE
 PLANS WHERE THE DOOR, SIDELIGHTS, DOOR FRAME, AND WALL
 SECTION ARE BEING REMOVED THIS IS A NON-BEARING WALL.
 107.2.1.3 ADDITIONAL INFORMATION IS REQUIRED.
  
 4. PLAN REFERENCES EXISTING MARBLE, SHOWS REMOVAL OF
 CARPET IN SOME AREAS AS WELL AS INSTALLATION OF TILE.
  
 SEE FBC A 404.3; REVISE PLAN TO SHOW PROPOSED ELEVATION
 CHANGES BETWEEN THE NEW TILE AREAS AND EXISTING AREAS
 WHICH WILL NOT BE ALTERED.
  
 5. FBC 109, PROVIDE A COPY OF THE CONTRACT FOR THIS
 JOB. A COST BREAKDOWN (LABOR, MATERIALS, ETC.) MAY ALSO
 BE REQUIRED. VALUE ON APPLICATION IS UNREASONABLY LOW.
  
 6. WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION
 & REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY, COLLATE AND STAPLE INTO SETS OF PLANS. A
 TRANSMITTAL LETTER LISTING THE ORIGINAL REVIEW COMMENT
 NUMBER, WITH A DESCRIPTION OF THE REVISION MADE,
 IDENTIFYING THE SHEET OR SPECIFICATION PAGE WHERE THE
 CHANGES CAN BE FOUND WILL HELP TO EXPEDITE YOUR PERMIT.
