 AND COPY OF PAID RECEIPT IF FEES ARE DUE. PROVISO MEANS
 THAT OWNER MAY PROVIDE THE REQUIRED INFORMATION AFTER
 THE PERMIT IS ISSUED AND BEFORE INSPECTIONS TO AVOID
 CONFLICTS/DELAYS. REVISION FEES MAY APPLY.
 OWNER/BUILDER TO ADVICE IN WRITING IF PROVISO OPTION IS
