 AGENCY SHALL REQUIRE A (WRITTEN/ TYPED) SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OF RECORD. THE STRUCTURAL
 INSPECTION PLAN MUST BE SUBMITTED TO THE ENFORCING

 SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO THAT
 THE BUILDING CAN BE ADEQUATELY INSPECTED FOR COMPLIANCE
 WITH THE PERMITTED DOCUMENTS.

 2. THRESHOLD BLDG., REQUIRED STATEMENT: FS553.79(7)(D),
 W.P.B. AMENDMENT 110.3.7.4.4 ALL PLANS FOR THE BUILDING
 WHICH ARE REQUIRED TO BE SIGNED AND SEALED BY THE
 ARCHITECT OR ENGINEER OF RECORD CONTAIN A STATEMENT
 THAT, TO THE BEST OF THE ARCHITECT?S OR ENGINEER?S
 KNOWLEDGE, THE PLANS AND SPECIFICATIONS COMPLY WITH THE
 APPLICABLE MINIMUM BUILDING CODES AND THE APPLICABLE
 FIRESAFETY STANDARDS AS DETERMINED BY THE LOCAL
 AUTHORITY IN ACCORDANCE WITH THIS CHAPTER AND CHAPTER
 633.
  
 2ND REVIEW, I WAS UNABLE TO LOCATE THIS STATEMENT ON
 THE ENGINEER'S PLANS.
 STATEMENT WAS ON AN A SHEET BUT NOT CLEAR WHO IS
 SIGNING THAT SHEET (SEE OTHER COMMENT REGARDING TITLE
 BLOCK), STATEMENT NOT CONSISTENT WITH FLORIDA STATUTE
 (LACKS REFERENCE TO BUILDING CODES).
  
 SEE ALSO THRESHOLD REVIEW FOR ADDITIONAL REQUIREMENTS
 (SI AGREEMENT, RESUMES, ETC.).
  
 3. SOIL BEARING VALUES. THE TEST BY UNIVERSAL
 ENGINEERING SCIENCES REFERENCES THE TEST IS PRELIMINARY
 GEOTECHNICAL ENGINEERING REPORT BANYAN CAY- HOTEL
 PLEASE NOTE THE BORING SAMPLES TAKEN CH-1 AND CH-2 ARE
 SHOWN FOR THE CLUBHOUSE, THIS SOILS REPORT HAS NO
 BORINGS SHOWN IN THE REPORT FOR THE HOTEL. NOTE FOR THE
 TENNIS CLUB CENTER UP TO 6 FEET OF MUCK WAS ENCOUNTERED
 WITHIN THE BUILDING FOOTPRINT. PLEASE COMPLY WITH: 2014
 FBC-B 1803.3 BASIS OF INVESTIGATION.
 SOIL CLASSIFICATION SHALL BE BASED ON OBSERVATION AND
 ANY NECESSARY TESTS OF THE MATERIALS DISCLOSED BY
 BORINGS, TEST PITS OR OTHER SUBSURFACE EXPLORATION MADE
 IN APPROPRIATE LOCATIONS. ADDITIONAL STUDIES SHALL BE
 MADE AS NECESSARY TO EVALUATE SLOPE STABILITY, SOIL
 STRENGTH, POSITION AND ADEQUACY OF LOAD-BEARING SOILS,
 THE EFFECT OF MOISTURE VARIATION ON SOIL-BEARING
 CAPACITY, COMPRESSIBILITY, LIQUEFACTION AND
 EXPANSIVENESS.
  
 2ND REVIEW, NOT ADDRESSED. PROVIDE A CURRENT REPORT.
 HAVE SEOR INCORPORATE A SPECIFIC REFERENCE TO THE
 UPDATED SOILS REPORT ON THE PLAN, OR PROVIDE A SHOP
 DRAWING REVIEW OF THE SOILS REPORT.

 5. 2ND REVIEW, SET BACK/ SEPARATION FROM PROPOSED
 BUILDING TO PROPERTY LINE(S) AND TO ASSUMED PROPERTY
 LINES IS REQUIRED, FBC B 705.3. DEPENDING ON THE
 LOCATION OF THE PROPERTY LINES AND/OR ASSUMED PROPERTY
 LINES, INFORMATION REGARDING AFFECTED BUILDINGS OR
 PROPOSED BUILDINGS MAY ALSO BE REQUIRED.
  
 A NOTE ON THE PLANS STATES THAT THIS BUILDING IS OVER
 30 FEET FROM OTHER BUILDINGS AND/OR LOT LINES. IT IS
 NOT CLEAR WHERE THE PROPERTY LINES ARE LOCATED.
 DISTANCE BETWEEN BUILDINGS NOT CLEAR (FOR INSTANCE -
 PROPOSED HOTEL TO BUILDING 11 ON SHEET C2, PROPOSED
 HOTEL TO PARKING GARAGE, HOTEL TO CLUBHOUSE), LOCATION
 OF ASSUMED PROPERTY LINE(S) NOT CLEAR.
  
 6. FROM PREVIOUS LIST: SHEET 019-S-4.06 THE PLANS
 INDICATE PRESSURES FOR THE ROOF ZONES 1-3 AND OVERHANGS
 ZONES 6&7. PRESSURES APPEAR TO BE LOW, UNKNOWNS, SLOPE
 AND MEAN ROOF HEIGHT. TABLE 1609.7(2) ADJUSTMENT
 FACTORS FOR HEIGHT & EXPOSURE. PROVIDE THE MEAN ROOF
 HEIGHT FOR THE BUILDING. THE SAME ISSUE FOR THE WINDOW
 AND DOOR SIZES AND ASSOCIATED PRESSURES (ZONES 4 & 5).
  
 COMMENT NOT ENTIRELY ADDRESSED. SLOPE WAS PROVIDED AS
 4:12 MANSARD. THE ROOF PLAN ON S4.06 APPEARS TO SHOW
 BOTH PITCHED ROOF AND FLAT ROOF AREAS. THE C&C WIND
 DESIGN PRESSURES ON THE TWO CHARTS ARE TOO LOW, EVEN
 FOR THE PITCHED ROOFS. ENGINEER'S INTENT NOT CLEAR; ARE
 ALL ROOFS C&C TO MEET THE SAME DESIGN PRESSURES FOR
 FLAT, MANSARD, AND PITCHED? IF THIS IS THE CASE, C&C
 ROOF PRESSURES TO BE "WORST CASE" CRITERIA (FLAT).
  
 SOME OF THE WALL C&C DESIGN PRESSURES SPECIFIED ALSO
 APPEAR TOO LOW (SEE 1ST REVIEW). EITHER CORRECT DESIGN
 PRESSURES OR INCORPORATE THE CALCULATIONS FOR THE C&C
 DPS INTO THE PLAN. IF THE PRESSURES ARE NOT INCREASED,
 SEND THE CALCULATIONS VIA EMAIL PRIOR TO RESUBMITTAL
 FOR PRELIMINARY REVIEW. IF THE PRESSURES ARE REVISED TO
 BE CONSISTENT WITH THE TABLES IN FBC CHAPTER 16 OR
 ASCE7-10, CALCULATIONS ARE NOT REQUIRED.

 REQUIREMENTS TO HAVE A TITLE BLOCK ON EACH SHEET WHICH
 INCLUDES THE PRINTED NAME AND LICENSE NUMBER, COMPANY
 LICENSE NUMBER (AS WELL AS OTHER INFORMATION).
  
 STRUCTURAL ENGINEER OF RECORD HAS A COMPLIANT TITLE
 BLOCK ON ALL S SHEETS. HOWEVER, REGARDING THE
 ARCHITECTURAL SHEETS, IT NOT CLEAR WHO IS SEALING THESE
 SHEETS. THE ARCHITECTURAL SHEETS LACK A TITLE BLOCK.
 THERE IS A BAR WITH ALL OF THE DESIGN PROFESSIONALS.
 THE ENGINEER'S NAME IS IN THE "SEAL" BLOCK. REVISE
 ARCHITECTURAL SHEETS TO INCLUDE ALL INFORMATION
 REQUIRED BY FAC61G16, FS481.
  
 11. CIVIL SHEETS USE NGVD29 VERTICAL DATUM. A0.0.02
 PROPOSES A FINISHED FLOOR OF 22'0' NAVD88. THERE IS A
 CONFLICT OF APPROXIMATELY 18"; REVISE TO BE CONSISTENT.
 NGVD29 IS RECOMMENDED AT THIS TIME.
  
 12. IF CIVIL SHEETS ARE USED TO SHOW COMPLIANCE WITH
 FLORIDA BUILDING CODE, THE ADVANCED BUILDING CODE
 COURSE IS REQUIRED FOR THE ENGINEER, FS471.0195.

 SHOW THAT JORGE REYES, PE HAS TAKEN THE ADVANCED

 13. THIS PERMIT WILL BE ISSUED WITH PROVISOS:
  
 A. COMPLIANCE WITH ART IN PUBLIC PLACES ORDINANCE
 78-123 REQUIRED: MS. SYBILLE WELTER, 561-822-1515,
 [email protected]. MS. WELTER WILL ADVISE VIA EMAIL WHEN
 THIS IS COMPLETE.
  
 B. FBC 110.3.7.4.3 SHORING AND RESHORE ENGINEERING TO
 BE SUBMITTED AND REVIEWED PRIOR TO INSTALLATION.
 FAILURE TO SUBMIT SHORING/RESHORE ENGINEERING TIMELY
 WILL RESULT IN A STOP WORK ORDER. AN ENGINEERED
 GUARDRAIL DETAIL (CONSTRUCTION IN PROGRESS) IS ALSO
 REQUIRED PRIOR TO INSTALLATION.
  
 C. FBC 105.13 PHASED PERMIT APPROVAL. AFTER SUBMITTAL
 OF THE APPROPRIATE CONSTRUCTION DOCUMENTS, THE BUILDING
 OFFICIAL IS AUTHORIZED TO ISSUE A PERMIT FOR THE
 CONSTRUCTION OF FOUNDATIONS OR ANY OTHER PART OF A
 BUILDING OR STRUCTURE BEFORE THE CONSTRUCTION DOCUMENTS
 FOR THE WHOLE BUILDING OR STRUCTURE HAVE BEEN
 SUBMITTED. THE HOLDER OF SUCH PERMIT FOR THE FOUNDATION
 OR OTHER PARTS OF A BUILDING OR STRUCTURE SHALL PROCEED
 AT THE HOLDER?S OWN RISK WITH THE BUILDING OPERATION
 AND WITHOUT ASSURANCE THAT A PERMIT FOR THE ENTIRE
 STRUCTURE WILL BE GRANTED. CORRECTIONS MAY BE REQUIRED
 TO MEET THE REQUIREMENTS OF THE TECHNICAL CODES. THIS
 PROVISION IS ONLY FOR THE FLORIDA BUILDING CODE, ALL
 OTHER AGENCY APPROVALS NECESSARY FOR CONSTRUCTION MUST
 BE SECURED PRIOR TO THIS PROVISION BEING APPLIED.
  
 D. THE LIFE SAFETY SHEETS WERE NOT THOROUGHLY REVIEWED.
 THIS WILL BE REVIEWED UNDER THE BUILDING PERMIT
 APPLICATION; FOUNDATION PERMIT WILL BE ISSUED AT
