 LOCATION BOTH THE PERMIT APPLICATION AND THE PROPERTY
 APPRAISERS OFFICE LIST THE OWNERS JAMES & TRICIA WOOD
 OWNERS OF PROPERTY AT 3920 N FLAGLER DR. SUITE # 203
 WHEREAS THE PLANS INDICATE THE WORK BEING COMPLETED AT
 3920 N. FLAGLER SUITE # 301.
 THE PROPERTY APPRAISERS OFFICE INDICATES ANTHONY &
 PATICIA ORRICO OWN THIS PROPERTY. IF THE WOODS RECENTLY
 BOUGHT THIS PROPERTY PLEASE BRING IN YOUR CLOSING
 PAPERS SO THE LAND FILE CAN BE UPDATED IF NOT PLEASE
 CORRECT THE PLANS TO REFLECT THE CORRECT SUITE NUMBER.
 107.2.1.3 DISCREPANCY IN THE UNIT BEING REMODELED.

 CONSTRUCTION DOCUMENTS SHALL BE OF SUFFICIENT CLARITY
 TO INDICATE THE LOCATION, NATURE AND EXTENT OF THE WORK

 HOLD WILL BE PUT ON THE PERMIT ISSUANCE UNTIL THE
 NOTARIZED OWNERS SIGNATURE IS ON THE PERMIT
 APPLICATION. 105.3.1. ACTION ON APPLICATION.
  
 3) SHEET A.21 INDICATES NEW FLOORS IN THE MASTER
 BATHROOM TO BE TILE AND THE POWDER ROOM TO RECEIVE NEW
 WOOD FLOORS. PLANS DO NOT INDICATE IF THE POWDER ROOM
 FLOORING IS TO BE A 3/4 PLANK FLOORING OR IF A LAMINATE
 WOOD FLOORING. FOR TILE, STONE, MARBLE AND WOOD
 FLOORING ALL NEED TO HAVE HAD THE SOUND UNDERLAYMENT
 TESTED FLOOR ASSEMBLIES TO THE FOLLOWING STANDARDS AND
 MEET THE QUALIFICATIONS LISTED UNDER EACH OF THE
 STANDARDS FOR A 6 INCH CONCRETE FLOOR ASSEMBLY/ NO
 DROPPED CEILING:
 PLEASE PROVIDE TEST REPORTS SHOWING COMPLIANCE WITH:
  
 3A) FBC-B 2014 1207.3 STRUCTURE-BORNE SOUND.
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E 492.
  
 3B) FBC-B 2014 1207.2 AIR-BORNE SOUND.
 WALLS, PARTITIONS AND FLOOR/CEILING ASSEMBLIES
 SEPARATING DWELLING UNITS FROM EACH OTHER OR FROM
 PUBLIC OR SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION
 CLASS (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED)
 FOR AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM
 E 90.
  
 3C) FLAME SPREAD- FBC-B 2014 603.1. EXCEPTION 2.
 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A
 MATERIAL TESTED IN ACCORDANCE WITH ASTM E 84 OR UL 723.
  
 4) SHEET A3.1 INDICATES THROUGHOUT THE SUITE THE USE OF
 SOFFITS AND DROPPED CEILINGS. PLEASE NOTE THERE IS A
 DETAIL SHOWING THE USE OF METAL TRACK SYSTEM. THE PLANS
 DO NOT PROVIDE WHERE CEILING FRAMING IS TO BE ATTACHED
 TO THE CONCRETE DECK, THE TYPE OF ATTACHMENT, SPACING
 OF THE ATTACHMENT, AND PENETRATION INTO THE CONCRETE
 DECK. THE PLANS DO NOT HAVE ANY INFORMATION FOR THE
 DROPPED CEILING THROUGH OUT THE SUITE. TYPE OF FRAMING
 MATERIAL, SIZE AND SPACING OF THE FRAMING MATERIAL,
 SPACING FOR CEILING JOIST, IF STIF BACKS ARE REQUIRED
 FOR THE SPANS NOR THE THICKNESS OF THE FINISH CEILING
 MATERIAL AND TYPE OF FINISH CEILING MATERIAL. WILL ALSO
 NEED TO KNOW THE DEFLECTION LIMITATION OF THE FINISH
 CEILING MATERIAL. 2014 FBC-B TABLE 2506.2 STEEL STUDS,
 NON-LOAD BEARING ASTM C 645 , TABLE 2508.1 STEEL
 FRAMING FOR GYPSUM BOARD ASTM 745 & TABLE 1604.3
 DEFLECTION LIMITATIONS.
  
 5) WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION
 & REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
 EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR ANTICIPATED
