 PCP CONTRATOR SIGNED ON TO VERIFY LICENSURE BEFORE
 PERMIT ISSUANCE. FL. S. 489.105(P) ALSO SEE F. S.
 376.303. UNLESS THE PCP CONTRACTOR HOLDS A SECOND
 LICENSE FOR THE CANOPY, ANOTHER CONTRACTOR WILL BE
 REQUIRED TO SIGN ONTO THE PERMIT APPLICATION.
  
 2) NOTE 2 PERMITS WERE APPIED FOR 15080304 UNDERGOUND
 STORAGE TANKS AND 15080306 A 3,096 SQ. FT CANOPY. NOTE
 ONLY 2 SETS OF PLANS WERE SUBMITTED. PLEASE NOTE ONLY
 SHEET G8 SHOWS ELEVATION OF THE CANOPY, NO DETAILS,
 STURTURAL SHEETS NOR WIND DESIGN WAS INCLUDED. 107.3.5
 MINIMUM PLAN REVIEW CRITERIA FOR BUILDINGS. IF THE
 DETAILS FOR TTHE CANOPY ARE TO BE INCORPORATED INTO ONE
 SET OF PLANS, THEN 4 SETS OF PLANS WILL BE REQUIRED. 2

 REVIEWED BY P. B. C. DEPARTMENT OF ENVIROMENTAL
 RESOURSES MANAGEMENT FOR REVIEW AND APPROVAL, PLANS
 STAMPED BY THE AGENCY AND RETURNED FOR REVIEW BY THE
 CITY BUILDING DEPARTMENT. 107.3.5 MINIMUM PLAN REVIEW
 CRITERIA FOR BUILDINGS.12. ENVIROMENTAL REQUIREMENTS.
  
 4) THE LISTED FAMILY OF CODES LIST THE 2010 FBC AS THE
 CODE THE DOCUMENTS WERE DESIGNED TOO. PLEASE UPDATE THE
 PLANS TO THE 2014 5TH EDITION OF THE FLORIDA BUILDING
 CODE, ADOPTED STATWIDE ON JUNE 30/ 2015.
  
 5) SHEET G1 PROVIDES THE DEAD WEIGHT DESIGN FOR THE USE
 OF THE DEAD MEN FOR EACH TANK, THE CENTER DEAD MAN CAN
 NOT BE SHARED AS DEPICTED. THE DESIGN OF THE DEADMAN IS
 18 INCHES WIDE, 8.75 INCHES IN HEIGHT AND 36 FEET LONG.
 PLEASE PROVIDE IF THIS IS A PRECAST UNIT OR IS THIS
 POURED IN PLACE? IF PRE-CAST IS THERE STEEL REBAR
 ENCASED ? IF POURED IS THERE TO BE STEEL
 RE-INFORCEMENT? IS THERE ANCHOR PROTRUDING FROM THE
 DEADMEN? 107.2.1.3 ADDITIONAL INFORMATION IS REQUIRED.
  
 6) SHEET G3 NOTE # 4 INDICATES THE USE OF MOH STRAP
 ANCHOR SYSTEM TO BE SUPPLIED BY THE TANK MANUFACTURER
 AND THE DEAD MEN ARE TO BE PLACED PER THE MANUFACTURERS
 INSTALLATION INSTRUCTION, PLEASE SUBMIT BOTH THE MOH
 STRAP ANCHOR SYSTEM AS WELL AS THE DEAD MEN INSTALATION
 INSTRUCTIONS. THE MOH SYSTEM WILL NEED TO INDICATE THE
 TYPE OF STRAPS, CENTER TO CENTER PLACEMENT AND HOW THEY
 ARE ATTACHED TO THE DEADMEN. 107.3.5 MINIMUM PLAN

 7) SHEET G1 INDICATES THE TANKS TO BE 10 FT. DIAMETER
 AND APPROXIMATELY 15 FEET BELOW GRADE. TO DATE THERE
 HAS BEEN NO SOILS REPORT SUBMITTED AS TO THE WATER
 TABLE LEVEL, AND HOW OR IF DEWATERING WILL BE REQUIRED
 AT THIS SITE SINCE THERE IS A CANAL JUST WEST OF THIS
 PROPERTY.
 2014 FBC-B 1803.3 BASIS OF SOILS INVESTIGATION.
  
 BEFORE A PERMIT TO CONSTRUCT, MAY BE ISSUED, IMPACT
 FEES MUST BE PAID TO PALM BEACH COUNTY. THE ACTUAL
 PERMIT SET OF PLANS MUST BE STAMPED BY THAT OFFICE, AND
 A COPY OF THE PAID RECEIPT ATTACHED TO THE PERMIT
