 1) NOTICE AT RISK, THIS PERMIT IS FOR A FOUNDATION
 ONLY, PLEASE PROVIDE AN APPENDIX TO THE PERMIT
 APPLICATION, DESCRIPTION OF WORK, AS TO WHAT WORK IS TO
 BE ACCOMPLISHED UNDER THIS FOUNDATION PERMIT. THE CITY
 CONSIDERS A FOUNDATION PERMIT ANY WORK TO BE COMPLETED
 UNTIL THE BUILDING LEVEL IS EVEN WITH GRADE.
  
 2010 WEST PALM BEACH AMENDMENTS TO THE FLORIDA BUILDING
 CODE, CHAPTER 1, ADMINISTRATION, 107.3.3. THE BUILDING
 OFFICIAL IS AUTHORIZED TO ISSUE A PERMIT FOR THE
 CONSTRUCTION OF FOUNDATIONS OR ANY OTHER PART OF A
 BUILDING OR STRUCTURE BEFORE THE CONSTRUCTION DOCUMENTS
 FOR THE WHOLE BUILDING OR STRUCTURE HAVE BEEN
 SUBMITTED, PROVIDED THAT ADEQUATE INFORMATION AND
 DETAILED STATEMENTS HAVE BEEN FILED COMPLYING WITH
 PERTINENT REQUIREMENTS OF THIS CODE. THE HOLDER OF SUCH
 PERMIT FOR THE FOUNDATION OR OTHER PARTS OF A BUILDING
 OR STRUCTURE SHALL PROCEED AT THE HOLDER?S OWN RISK
 WITH THE BUILDING OPERATION AND WITHOUT ASSURANCE THAT
 A PERMIT FOR THE ENTIRE STRUCTURE WILL BE GRANTED.
  
 2) NOTICE ONLY; THE CITY OF WEST PALM BEACH ART IN
 PUBLIC PLACES ORDINANCE REQUIREMENT THAT APPLIES TO ANY
 REMODEL OR NEW CONSTRUCTION PROJECT FOR WHICH THE
 PERMIT APPLICATION WAS SUBMITTED AFTER 7/1/14, AND THE
 TOTAL PROJECT CONSTRUCTION COSTS ARE EQUAL TO OR
 GREATER THAN $500,000. PER CITY OF WEST PALM BEACH
 ORDINANCE NO. 4504-14, AND CITY OF WEST PALM BEACH CODE
 OF ORDINANCES, ARTICLE V, ART IN PUBLIC PLACES, SECTION
 78-123(A) AND SECTION 78-126(A), THE ART ASSESSMENT IS
 1% OF THE TOTAL PROJECT CONSTRUCTION COST.
  
 PER CITY OF WEST PALM BEACH CODE OF ORDINANCES, ARTICLE
 V, ART IN PUBLIC PLACES, SECTION 78-122, RENOVATION AND
 NEW CONSTRUCTION PROJECTS ARE ASSESSED CUMULATIVELY, SO
 THE ART ASSESSMENT WILL BE APPLICABLE TO ALL FUTURE
 PROJECTS.
  
 3) 4TH REQUEST. A FLOOD ZONE CERTIFICATE IS REQUIRED AS
 PART OF THE PLAN REVIEW DOCUMENTS WITH THE PROPOSED
 FINISH FLOOR HEIGHT. ON THE SUBMITTED FLOOD ZONE
 CERTIFICATE THE LINE ITEM FOR ATTACHED GARAGE WAS
 MARKED N/A. THERE IS 2 STORIES OF PARKING
  
 4) IN GOING TO THE 2010 FBC-B SECTION 1612.4 THE DESIGN
 AND CONSTRUCTION OF BUILDINGS LOCATED IN THE FLOOD
 HAZARD AREAS THE DESIGN SHALL BE IN ACCORDANCE WITH
 ASCE 24. BEFORE GOING TO THE ASCE 24 I HAD REVIEWED THE
 SUBMITTED COASTAL A ZONE REPORT AND THE WAVE HEIGHT
 ANALYSIS FOR FLOOD INSURANCE STUDIES V4.0 (WHAFIS). I
 CONCUR THAT THE PROPERTY IS NOT LOCATED IN A COASTAL A5
 ZONE BUT AN A5 FLOOD ZONE. COASTAL A ZONE.
  
 5) IN REVIEWING ASCE 24 SECTION 6.2.1 DRY FLOOD
 PROOFING LIMITATIONS STATES THAT IN NONRESIDENTIAL
 AREAS OF MIXED USE STRUCTURES SHALL NOT BE ALLOWED
 UNLESS SUCH STRUCTURES ARE LOCATED OUTSIDE OF THE HIGH
 RISK FLOOD HAZARD AREAS, COASTAL HIGH HAZARD AREAS, AND
 COASTAL A ZONES. THE WHAFIS STUDY DOES TAKE THE
 BUILDING OUT OF THE COASTAL A ZONE TO AN A5 FLOOD ZONE.
 TO CONSIDER THIS BUILDING A MIXED USE BUILDING, THERE
 ARE AREAS OF THE BUILDING THAT NEED TO BE REVIEWED TO
 DETERMINE IF THE BUILDING QUALIFIES AS A MIXED USE.
  
 5A) SHEET 080-A2-01 OF THE MASTER BUILDING PERMIT
 INDICATES AN AREA ROOM 103 RESIDENTS CLUB DINING.
 APPROXIMATELY 480 SQUARE FEET WITH AN OCCUPANT LOAD OF
 32 PEOPLE. THERE ARE 2 ISSUES TO CONSIDER, THE PLANS DO
 NOT SHOW ANY COOKING EQUIPMENT JUST A 2 COMPARTMENT
 SINK IN THE PANTRY AREA, ROOM 109.
 FOR THE USE OF THE DINING FACILITIES PLEASE PROVIDE HOW
 PAYMENT WILL BE MADE FOR THE USE OF TENANT SPACE AND
 SERVICES RENDERED. TO CONSIDER THIS SPACE AS A DELI,
 SMALL RESTAURANT (2010 FBC-B 309.1 RESTAURANTS AND
 DRINKING ESTABLISHMENTS WITH AN OCCUPANT LOAD LESS THAN
 50 PERSONS) AND NOT JUST AN AMENITY THE BUILDING
 OFFICIAL WILL NEED DOCUMENTATION TO SHOW THIS IS A
 MERCANTILE OCCUPANCY WHERE SERVES WERE RENDERED AND
 PAYMENT MADE.
  
 5B) SHEET 080-A2-01 OF THE MASTER BUILDING PERMIT
 INDICATES ROOM # 143 TO BE A MANICURE AND HAIR DRESSING
 SHOP OR A BEAUTY PARLOR 2010 FBC-B 304.1. THIS COULD BE
 CONSIDERED A BUSINESS OCCUPANCY .
 TO CONSIDER THIS SPACE AS A BEAUTY PARLOR AND NOT JUST
 AN AMENITY THE BUILDING OFFICIAL WILL NEED
 DOCUMENTATION TO SHOW HOW PAYMENT FOR SERVICES
 RENDERED.
  
 6) ASCE 24-05 6.2.1 DRY FLOOD PROOFING LIMITATIONS. DRY
 FLOOD PROOFING OF RESIDENTIAL AREAS OF MIXED USE
 STRUCTURES SHALL NOT BE PERMITTED.
 TECHNICAL BULLETIN 6-93, AND TECHNICAL BULLETIN 3-93.
  
 7) PLEASE PROVIDE COMPLIANCE WITH ASCE 24-05 TABLE 6-1
 THE MINIMUM ELEVATION OF FLOODPROOFING, RELATED TO BASE
 FLOOD ELEVATION (BFE) OR DESIGN FLOOD ELEVATION DFE).
 PLEASE ALSO INCLUDE THE STRUCTURE CATEGORY.
  
 8) PLEASE REVIEW ASCE 24-5 SECTION 6.2.1 DRY FLOOD
 PROOFING LIMITATIONS SHOW COMPLIANCE WITH SECTIONS 1 &
 2.
  
 9) SHOW COMPLIANCE ALSO WITH ASCE 24-5 SECTION 6.2.2
 DRY FLOODPROOFING REQUIREMENTS 1-3.
  
 10) SHOW HOW COMPLIANCE WITH ASCE 24-5 SECTION 6.2.3
 REQUIREMENTS 1-3.
  
 11) NFPA REGULATION 60.3(3) (II) TOGETHER WITH
 ATTENDANT UTILITY AND SANITARY FACILITIES, BE DESIGNED
 SO THAT BELOW THE BASE FLOOD LEVEL THE STRUCTURE IS
 WATERTIGHT WITH WALLS SUBSTANTIALLY IMPERMEABLE TO THE
 PASSAGE OF WATER AND WITH STRUCTURAL COMPONENTS HAVING
 THE CAPACITY OF RESISTING HYDROSTATIC AND HYDRODYNAMIC
 LOADS AND EFFECTS OF BUOYANCY.
  
 12) NFIP 60.3? (4) (I) A REGISTERED PROFESSIONAL
 ENGINEER OF ARCHITECT SHALL DEVELOP AND /OR REVIEW
 STRUCTURAL DESIGN, SPECIFICATIONS, AND PLANS FOR THE
 CONSTRUCTION AND SHALL CERTIFY THAT THE DESIGN AND
 METHODS OF CONSTRUCTION ARE IN ACCORDANCE WITH THE
 ACCEPTED STANDARDS OF PRACTICE FOR MEETING THE
 APPLICABLE PROVISIONS OF PARAGRAPHS (3)(II) OR (8)(II)
 OF THIS SECTION, AND (II) A RECORD OF SUCH CERTIFICATES
 WHICH INCLUDES THE SPECIFIC ELEVATIONS (IN RELATION TO
 MEAN SEA LEVEL) TO WHICH SUCH STRUCTURES ARE FLOOD
 PROOFED SHALL BE MAINTAINED WITH THE OFFICIAL
 DESIGNATED BY THE COMMUNITY?
  
 13) PLEASE REVIEW ALL OF THE REQUIREMENTS WITHIN THE
 FEMA TECHNICAL BULLETIN 3-93 FOR NON-RESIDENTIAL
 FLOODPROOFING-REQUIREMENTS AND CERTIFICATIONS:

 14) PROVISO: FBC-B 3306.9 ADJACENT TO EXCAVATIONS.
 EVERY EXCAVATION ON A SITE LOCATED 5 FEET (1524 MM) OR
 LESS FROM THE STREET LOT LINE SHALL BE ENCLOSED WITH A
 BARRIER NOT LESS THAN 6 FEET (1829 MM) HIGH. WHERE
 LOCATED MORE THAN 5 FEET (1524 MM) FROM THE STREET LOT
 LINE, A BARRIER SHALL BE ERECTED WHEN REQUIRED BY THE
 BUILDING OFFICIAL. BARRIERS SHALL BE OF ADEQUATE
 STRENGTH TO RESIST WIND PRESSURE AS SPECIFIED IN
 CHAPTER 16.
 15) PROVISO: PERIMETER FENCING UNDER A SEPARATE PERMIT.
 PLEASE PROVIDE COMPLIANCE WITH: FBC-B 3306.5 BARRIERS.
 BARRIERS SHALL BE A MINIMUM OF 8 FEET (2438 MM) IN
 HEIGHT AND SHALL BE PLACED ON THE SIDE OF THE WALKWAY
 NEAREST THE CONSTRUCTION. BARRIERS SHALL EXTEND THE
 ENTIRE LENGTH OF THE CONSTRUCTION SITE.
  
 16) PROVISO: 2010 FBC-B [F] 3309.1 WHERE REQUIRED. ALL
 STRUCTURES UNDER CONSTRUCTION SHALL BE PROVIDED WITH
 NOT LESS THAN ONE APPROVED PORTABLE FIRE EXTINGUISHER
 IN ACCORDANCE WITH SECTION 906 AND SIZED FOR NOT LESS
 THAN ORDINARY HAZARD AS FOLLOWS:
 1. AT EACH STAIRWAY ON ALL FLOOR LEVELS WHERE
 COMBUSTIBLE MATERIALS HAVE ACCUMULATED.
 2. IN EVERY STORAGE AND CONSTRUCTION SHED.
 3. ADDITIONAL PORTABLE FIRE EXTINGUISHERS SHALL BE
 PROVIDED WHERE SPECIAL HAZARDS EXIST, SUCH AS THE
 STORAGE AND USE OF FLAMMABLE AND COMBUSTIBLE LIQUIDS.
 9F) FBC-B 3309.2 FIRE HAZARDS.
 THE PROVISIONS OF THIS CODE AND THE FLORIDA FIRE
 PREVENTION CODE SHALL BE STRICTLY OBSERVED TO SAFEGUARD
 AGAINST ALL FIRE HAZARDS ATTENDANT UPON CONSTRUCTION
 OPERATIONS.
  
 17) PROVISO: [F] 3311.1 WHERE REQUIRED. IN BUILDINGS
 REQUIRED TO HAVE STANDPIPES BY SECTION 905.3.1, NOT
 LESS THAN ONE STANDPIPE SHALL BE PROVIDED FOR USE
 DURING CONSTRUCTION. SUCH STANDPIPES SHALL BE INSTALLED
 WHEN THE PROGRESS OF CONSTRUCTION IS NOT MORE THAN 40
 FEET (12 192 MM) IN HEIGHT ABOVE THE LOWEST LEVEL OF
 FIRE DEPARTMENT VEHICLE ACCESS. SUCH STANDPIPE SHALL BE
 PROVIDED WITH FIRE DEPARTMENT HOSE CONNECTIONS AT
 ACCESSIBLE LOCATIONS ADJACENT TO USABLE STAIRS. SUCH
 STANDPIPES SHALL BE EXTENDED AS CONSTRUCTION PROGRESSES
 TO WITHIN ONE FLOOR OF THE HIGHEST POINT OF
 CONSTRUCTION HAVING SECURED DECKING OR FLOORING.
  
 18)PROVISO: 109.3.10.1 HURRICANE PROTECTION. IT SHALL
 BE UNLAWFUL FOR ANY PERSON TO ALLOW CONSTRUCTION
 RELATED MATERIALS EQUIPMENT AND DEBRIS TO REMAIN LOOSE
 OR OTHERWISE UNSECURED AT A CONSTRUCTION SITE FROM 24
 HRS. AFTER A HURRICANE WATCH HAS BEEN ISSUED UNTIL THE
 HURRICANE WATCH OR WARNING HAS BEEN LIFTED. ALL SUCH
 CONSTRUCTION MATERIALS, EQUIPMENT AND DEBRIS SHALL BE
 EITHER REMOVED FROM THE CONSTRUCTION SITE OR SECURED IN
 SUCH A MANNER AS TO MINIMIZE THE DANGER OF SUCH
 CONSTRUCTION MATERIALS, EQUIPMENT AND DEBRIS CAUSING
 DAMAGE TO PERSONS OR PROPERTY FROM HIGH WINDS. ANY
 PERSONS WHO FAILS TO COMPLY TO COMPLY TO REMOVE OR
 SECURE THE CONSTRUCTION MATERIALS EQUIPMENT AND DEBRIS
 WITHIN 24 HRS. AFTER A HURRICANE WATCH HAS BEEN ISSUED
 SHALL BE SUBJECT TO A FINE NOT TO EXCEED $500.00. IN
 ADDITION TO THE ABOVE, A LICENSED CONTRACTOR WHO
 VIOLATES THIS SECTION SHALL BE SUBJECT TO DISCIPLINE
 PURSUANT TO SECTION 113 OF THIS CHAPTER.
  
 19) PROVISO: ALL CONSTRUCTION TRAILERS (MOBILE OR
 MODULAR BUILDINGS) THAT ARE GOING TO HAVE MEETING IN
 THEM WILL BE REQUIRED TO GET A PERMIT, 102.2( E )
 MOBILE OR MODULAR STRUCTURES USED AS TEMPORARY OFFICES,
 EXCEPT THAT THE PROVISIONS OF PART V (SECTION
 553.501-553.513, FLORIDA STATUTES) RELATING TO
 ACCESSIBILITY BY PERSONS WITH DISABILITIES, AND PERMITS
 SHALL BE REQUIRED FOR STRUCTURAL SUPPORT AND TIE DOWN,
 ELECTRICAL SUPPLY, AND ALL UTILITY CONNECTIONS TO SUCH
 MOBILE OR MODULAR STRUCTURES.
  
 20)PROVISO: DEPENDING ON THE SIZE OF THE CONSTRUCTION
 TRAILERS, CLOSENESS TO BUILDINGS AND OR OTHER
 CONSTRUCTION TRAILERS THE FIRE DEPARTMENT THROUGH THE
 FIRE PREVENTION CODE MAY REQUIRE FIRE SPRINKLERS.
  
 21)PROVISO: TO DATE THERE HAS BEEN NO PERMIT APPLIED
 FOR, FOR TEST PILING INCOMPLIANCE WITH THE 2010 FBC-B
 1810.3.3.1.2 LOAD TEST.
  
 22) 2ND REQUEST, NO RESPONSE COMMENT. THERE IS A
 QUESTION OF THE BONDING OF THE STEEL RE-ENFORCEMENT FOR
 ELECTRICAL. IS THERE ALSO GOING TO BE ANY LIGHTNING
 BONDING GOING TO BE REQUIRED? PERMIT WILL NEED TO BE
 PULLED FOR THE SCHEDULING OF THESE INSPECTIONS, NO
 DETAILS.
  
 23) IMPACT FEES DEFERRED TO BUILDING PERMIT.
