 1. THIS PARCEL IS LOCATED IN A SPECIAL HAZARD FLOOD
 ZONE. DUE TO THE LARGE NUMBER OF STRUCTURES ON THIS
 SITE AND PAST PERMITS, IT IS DIFFICULT TO DETERMINE
 WHETHER THIS PROJECT CONSTITUTES A "SUBSTANTIAL
 IMPROVEMENT" OR NOT. ALL CUMULATIVE LIFETIME
 IMPROVEMENTS ARE INCLUDED IN CALCULATING WHETHER OR NOT
 A CURRENT PROJECT IS CONSIDERED A "SUBSTANTIAL
 IMPROVEMENT". IF CUMULATIVE LIFETIME IMPROVEMENTS TO A
 STRUCTURE EXCEED 50%, THE STRUCTURE IS TO BE BROUGHT
 INTO COMPLIANCE.
  
 IT IS NOT ENTIRELY CLEAR WHICH STRUCTURE IS BEING
 RENOVATED. THE LOCATION PLAN SHOWS A BUILDING IN AN
 EAST/WEST ORIENTATION AT THE CORNER OF A LOT LOCATED ON
 THE CORNER OF 45TH AND DIXIE. APPLICATION IS FOR 5200
 EAST AVENUE, WHICH IS LOCATED ON EAST AVENUE NORTH OF
 45TH STREET (NOT LOCATED ON 45TH STREET). THE PARCEL IS
 NOT RECTANGULAR IN SHAPE AS DEPICTED ON THE LOCATION
 PLAN, AND THE BUILDINGS IN THE SW CORNER ARE ORIENTED
 NORTH/SOUTH. ON THE COUNTY GIS, DIXIE HIGHWAY DOES NOT
 CROSS 45TH STREET.
  
 PLEASE PROVIDE A SITE PLAN OF THE FACILITY. THIS MAY BE
 DONE USING GIS/AERIAL PHOTOGRAPHY (CAD DRAWINGS NOT
 REQUIRED UNLESS ALREADY READILY AVAILABLE). INDICATE
 WHICH STRUCTURE IS TO BE COVERED UNDER THIS PERMIT. IF
 THE FACILITY HAS A SITE PLAN WITH BUILDING
 LABELS/NUMBERS, THIS WOULD BE HELPFUL.
  
 IN ORDER TO RESOLVE THE FLOOD ISSUE, PLEASE CONTACT THE
 MANAGEMENT TO SEE IF THEY HAVE ELEVATION CERTIFICATES
 AVAILABLE. ALTERNATIVELY, YOU MAY PROVIDE ME WITH THEIR
 INFORMATION AND I'LL CONTACT THEM DIRECTLY.

 EXPLANATION/CLARIFICATION. ELEVATION CERTIFICATES MAY
 BE SENT VIA EMAIL FOR PRELIMINARY REVIEW.
  
 2. ARCHITECT'S PRINTED LICENSE NUMBER IS TO BE INCLUDED
 AS A PART OF THE TITLE BLOCK, FAC61G1-16.004. THE
 ARCHITECT HAS BEEN NOTIFIED VIA EMAIL THAT THIS HAS
 BEEN WRITTEN ON THE PLAN FOR HER WITH HER APPROVAL.
 PLANS HAVE BEEN STAMPED (APPROVED) PENDING RECEIPT OF
 THE OTHER INFORMATION REQUESTED. IF THE PLAN IS ALTERED
 AND RESUBMITTED, PLEASE HAVE THE ARCHITECT INCLUDE THE
