 1) PROVIDE A RECENT SURVEY, LESS THAN ONE YEAR OLD,
 WHICH INCLUDES THE LOCATIONS OF THE EXISTING TREES
 ALONG LAKE AVENUE AND THE EXISTING LIGHT POLES. THE
 SURVEY SHALL BE TO SCALE WITH ALL DIMENSION LINES
 SHOWN.
  
 2) THE SITE PLAN (OR MARKED UP SURVEY) SHALL SHOW THE
 PROPOSED NEW AND REPLACEMENT LIGHT POLES WITH THEIR
 DIMENSIONS FROM THE PROPERTY LINES. PLEASE NOTE THAT
 NEW AND REPLACEMENT POLES SHALL BE LOCATED A MINIMUM OF
 5 FEET FROM THE PROPERTY LINE.
  
 3) ONE OF THE NEW POLES ALONG LAKE AVENUE APPEARS THAT
 IT WOULD CONFLICT WITH AN EXISTING TREE. THE POLES
 SHALL BE PLACED TO AVOID TREES AND THEIR CANOPY GROWTH.
  
 4) THE SOUTHERN A2 POST APPEARS THAT IT IS NOT ON
 PARCEL 74434404000020020, BUT RATHER PARCEL
 74434404000020010. THIS WILL BE VERIFIED WITH THE
 SURVEY. IF THE POST IS ON THE NEIGHBORING PARCEL, THEN
 A SEPARATE PERMIT APPLICATION FOR THE WORK ON THE 2ND
 PARCEL IS REQUIRED. THE SAME PLANS MAY BE USED FOR
 BOTH.
  
 5) THE PHOTOMETRIC PLAN SHALL CLEARLY SHOW THE
 CENTERLINE OF LAKE AVENUE. HALF OF LAKE AVENUE ABUTTING
 THE SOUTH PORTION OF THE SITE IS RESIDENTIALLY ZONED.
 THE PHOTOMETRIC CALCULATIONS SHALL BE PROVIDED TO THE
 MIDPOINT OF LAKE AVENUE AND SHALL NOT EXCEED 0.5
 FOOTCANDLES. THE PROVIDED PHOTOMETRIC PLAN SEEMS TO
 INDICATE HIGHER LIGHT LEVELS.
  
 6) THE LIGHT FIXTURES SHALL BE DESIGNED SO THAT GLARE
 DOES NOT SPILL INTO THE RIGHT-OF-WAY (DRIVERS VISION).
 PLEASE PROVIDE INFORMATION HOW GLARE WILL BE
 CONTROLLED.
