 1) SUBMIT TWO COPIES OF A SURVEY AND A SITE PLAN
 SHOWING THE LOCATION OF THE PROPOSED ADDITION. (WAS NOT
 ADDRESSED.)
  
 2) ADDITIONAL INFORMATION IS REQUIRED TO SUPPORT THE
 ENGINEER'S LETTER AND DRAWING OF THE EXISTING
 FOUNDATION, STEM WALL AND SLAB THAT WAS INSTALLED
 WITHOUT A PERMIT OR INSPECTIONS. PLEASE SPECIFY THE
 METHODS THAT WERE USED TO DETERMINE THE FOOTER
 THICKNESS AND WIDTH, THE REINFORCING STEEL IN THE STEM
 WALL AND THE 6 X 6 W.W.F. IN THE 4" SLAB. HOW WAS THE
 COMPACTION OF THE SOIL UNDER THE SLAB VERIFIED? WERE
 ANY PHOTOGRAPHS TAKEN DURING THE CONSTRUCTION? IS THIS
 DESIGN BASED ON A PREVIOUS SET OF DRAWINGS FOR THIS
 PROJECT? WAS AN EXCAVATION ADJACENT TO THE FOOTER USED?
 IS THERE ANY RECORD OF A SOIL COMPACTION TEST FOR THE
 FOOTINGS OR SLAB? CITY AMENDMENTS 107.2.1.3
  
 3) FBC R314.3.1 REQUIRES SMOKE ALARMS TO BE INSTALLED
 AS REQUIRED FOR NEW DWELLINGS WHEN ADDITIONS OR
 ALTERATIONS REQUIRING PERMITS OCCUR. SHOW COMPLIANCE.
 THE REVISED PLANS SHOW A SMOKE ALARM IN THE FAMILY
 ROOM, WHICH IS NOT REQUIRED. BECAUSE THE FLOOR PLAN
 DOES NOT INCLUDE THE ENTIRE DWELLING, A NOTE ON THE
 PLANS STATING THAT SMOKE ALARMS WILL BE INSTALLED IN
 EACH SLEEPING ROOM AND OUTSIDE EACH SEPARATE SLEEPING
 AREA IN THE IMMEDIATE VICINITY OF THE BEDROOMS AS
 REQUIRED BY FBC R314 WILL BE SUFFICIENT TO ADDRESS THIS
 REQUIREMENT.
  
 4) COMMENT HAS BEEN ADDRESSED.
  
 5) FORM 402-2010 SPECIFIES CERTAIN PERFORMANCE CRITERIA
 THAT MUST BE MET. THE EXTERIOR WALL INSULATION FOR
 FRAME IS REQUIRED TO BE R-13 AND R-11 IS SPECIFIED ON
 THE FORM. R-11 IS INCORRECTLY SPECIFIED ON THE FORM FOR
 CONCRETE OR MASONRY " MASS" WALLS, THIS ADDITION DOES
 NOT HAVE EXTERIOR CONCRETE OR MASONRY WALLS. THERE ARE
 NO WALLS THAT ARE "ADJACENT" TO UNCONDITIONED SPACE.
 SEE FBC ENERGY CONSERVATION 402.1. PLEASE SUBMIT
 CORRECTED ENERGY FORMS.

 8) THE PRODUCT APPROVAL FOR THE TAMKO MODIFIED ROOFING

 12-1003.05. PLEASE SUBMIT THE CURRENT PRODUCT APPROVAL
 THAT HAS BEEN REVIEWED AND APPROVED IN WRITING BY THE
 DESIGNER OF RECORD. DCA RULE 9N-3 AND CITY AMENDMENTS

 9) SPECIFY THE APPROVED ASSEMBLY, THE FASTENING OPTION
 AND THE MINIMUM DESIGN PRESSURES THAT WILL BE USED TO
 INSTALL THE TAMKO ROOFING SYSTEM. PROVIDE TWO SETS OF
 SIGNED AND SEALED RAS 117 CALCULATIONS FOR ENHANCED
 FASTENING FOR THE PERIMETER AND CORNER ROOF AREAS, AS
 REQUIRED BY LIMITATION #7 ON THE LAST PAGE OF THE NOA.

 10) THE VALUE FOR THE WORK SPECIFIED ON THE APPLICATION
 OF $1500.00 IS TOO LOW. FOR PERMITTING FEE PURPOSES THE
 VALUE IS TO BE THE TOTAL REPLACEMENT VALUE INCLUDING
 STRUCTURAL, ELECTRICAL, DESIGN FEES, OVERHEAD AND
 PROFIT AS WOULD BE CHARGED BY A CONTRACTOR. PLEASE
 PROVIDE AN ACCURATE CONSTRUCTION COST VALUE. CITY
 AMENDMENTS 109.3 (WAS NOT ADDRESSED.)
  
 11) IN ORDER TO SPEED PLAN REVIEW TIME, PLEASE SUBMIT A
 RESPONSE LETTER INDICATING HOW EACH OF THE REVIEW
