 SUBMITTED. AND, PREPARER AND OWNER/AGENT TO CERTIFY AND
 DATE BOTTOM OF FORM.

 COUNTY (2300 N. JOG RD.) TO VERIFY IF IMPACT FEES ARE
 DUE. BRING BACK COUNTY STAMPED PLANS AND COPY OF PAID
 RECEIPT IF FEES ARE DUE. REQUIRED BY THE COUNTY IMPACT
 FEE ORDINANCE OF THE UNIFIED LAND DEVELOPMENT CODE.
