 1- PROVIDE PRODUCT APPROVAL (2 COPIES) AS REQUIRED BY
 DCA RULE 9N-3 FOR:
 A) GARAGE DOOR THAT MEETS THE DESIGN PRESSURES
 CALCULATED BY THE ARCHITECT ON SHEET 3 OF 6. MIAMI-DADE
 NOA 12-0228.04 SUBMITTED DOESN'T MEET THE CALCULATED
 DESIGN PRESSURES. AND. IT WAS SUPERSEDED BY MIAMI-DADE.
 VISIT THE MIAMI-DADE WEBSITE AND DOWNLOAD CURRENT NOA
 THAT MEETS THE CALCULATED DESIGN PRESSURES.
  
 B) ALL SIMPSON STRUCTURAL CONNECTORS SPECIFIED ON
 PLANS, DETAILS, SECTIONS AND SPECIFICATIONS. ***NOTE:
 PROVIDE ONLY THE PRODUCT APPROVAL COVER PAGE AND PAGE
 SHOWING SPECIFIED CONNECTOR. (NO DONE).
  
 C) PRECAST LINTELS AS SPECIFIED ON PLANS. (NO DONE).
  
 2- ARCHITECT TO REVIEW AND APPROVE IN WRITING (NOT
 SIGNING AND SEALING) ALL THE PRODUCT APPROVALS. SEC.
 107.3.4.1 CITY AMENDMENTS TO FBC. (NO DONE).
  
 3- SPECIFY ON PLANS CARBON MONOXIDE ALARMS AS REQUIRED
 BY SEC. R315 OF 2010 FBC-RESIDENTIAL. (NO DONE).
  
 4- SOIL TEST REPORT SUBMITTED STATES "ALLOWABLE BEARING
 PRESSURE OF 1,500 PSF". WHILE, ARCHITECT SPECIFIES ON
 THE FOUNDATION PLAN "SOIL BEARING CAPACITY MIN. 2500
 PSF". EXISTING SOIL WILL NOT SUPPORT PROPOSED
 STRUCTURE. THIS NEEDS TO BE ADDRESSED BY THE ARCHITECT
 OF RECORD OR BY THE GEOTECHNICAL ENGINEER. SEC.
 107.2.1.3 OF CITY AMENDMENTS TO FBC.
  
 5- ARCHITECT TO SPECIFY ON PLANS ALL DEAD AND LIVE
 LOADS INCLUDING THE ATTIC LIVE LIVE LOADS AS REQUIRED
 BY SEC. R301.1 AND TABLE R301.5 OF 2010
 FBC-RESIDENTIAL. (NO DONE).
  
 6- PROVIDE MANUFACTURER SPECS FOR THE ICYNENE
 INSULATION SPECIFIED ON PLANS AND ENERGY CALCULATIONS.

 7- SHEET 5 OF 6: A) IDENTIFY ROOF SHEATHING ZONES AS
 REQUIRED BY FIGURE R803.2.3.1 OF 2010 FBC-RESIDENTIAL.
 NOTE THAT GABLE END REQUIRES 5'-0". (NO DONE).
  
 B) SPECIFY TRUSS CONNECTORS FOR TRUSSES THAT ARE GOING
 TO BE SUPPORTED ON EXISTING MASONRY WALL BY THE NEW

 8- WALL SECTION ON SHEET 6 OF 6; A) CLARIFY IF ATTIC IS
 DESIGNED TO BE VENTED OR UNVENTED. SEC. R806 OF 2010
 FBC-RESIDENTIAL. THERE IS ONE NOTE SAYING "MATCH
 EXISTING SOFFIT & VENTS", BUT THERE ARE SPECIFICATIONS
 FOR ICYNENE INSULATION IN THE ATTIC. REVISE AS REQUIRED
 TO MATCH. COORDINATE WITH ENERGY CALCULATIONS
 SUBMITTED. (NO DONE).
  
 B) IF DESIGNING VENTED ATTIC, THEN SPECIFY VENT SIZE
 AND SPACING. SEC. R806 OF 2010 FBC-RESIDENTIAL AND SEC.
 103 OF 2010 FBC-ENERGY CONSERVATION. (NO DONE).
  
 C) CLARIFY IF USING THE RIDGE VENT OR NOT. THERE WAS A
 PRODUCT APPROVAL SUBMITTED FOR RIDGE VENT. ARCHITECT TO
 SPECIFY ON PLANS THE RIDGE VENT IF USING IT. SEC.
 107.2.1 CITY AMENDMENTS. (NO DONE).
  
 D) IF DESIGNING UNVENTED ATTIC, THEN, SPECIFY R-VALUE
 FOR THE ICYNENE INSULATION. SEC. 103 OF 2010 FBC-ENERGY
 CONSERVATION. COORDINATE WITH ENERGY CALCULATIONS. (NO
 DONE).
  
 F) SPECIFY R-VALUE IN WALL INSULATION. COORDINATE WITH
