 1) PLEASE CORRECT THE PERMIT APPLICATION ADDRESS FIELD
 AS WELL AS ADDRESS ON THE PLANS. THE CORRECT PARCEL #
 IS 74-4343-03-26-003-0000 THE CORRESPONDING ADDRESS IS
 4300 N FLAGLER DR. THE ORIGINAL BUILDING UNDER PERMIT #
 06101058 WAS ALSO BUILT AND PERMITED UNDER 4300 N
 FLAGLER DR. NOTE THE ORIGINAL PLANS WERE NEVER
 CORRECTED FOR THE ADDRESS.
  
 2) THE NEW 2ND FLOOR AREA THAT WAS PERMITED ORIGINALLY
 AS A OPEN MEZZANINE (06101058 SHEET 9/17), IT WILL NOW
 BECOME A TRUE SECOND FLOOR. WHILE MEZZANINES ARE ALOUD
 TO HAVE OPEN STAIRWAYS, A TRUE SECOND FLOOR WITH A
 OCCUPANT LOAD OF 107 OCCUPANTS WILL REQUIRE TWO EXIT
 ENCLOSURES PER 2010 FBC-B 1022.1.
 1 HOUR FIRE RATING, EXIT ENCLOSURES SHALL LEAD DIRECTLY
 TO THE EXTERIOR OF THE BUILDING OR SHALL BE EXTENDED TO
 THE EXTERIOR OF THE BUILDING WITH AN EXIT PASSAGEWAY
 CONFORMING TO THE REQUIREMENTS OF SECTION 1023, EXCEPT
 AS PERMITTED IN SECTION 1027.1. AN EXIT ENCLOSURE SHALL
 NOT BE USED FOR ANY PURPOSE OTHER THAN MEANS OF EGRESS.
  
 3) IN THE ORIGINAL BUILDING PERMIT # 06101058 THERE IS
 TWO INTERIOR STAIRS FOR THE PORTION OF BUILDING THAT
 WAS CONSIDERED 2 STORY, WITH THE ADDITIONAL SQUARE
 FOOTAGE BEING ADDED THE DISTANCE APART BETWEEN THE TWO
 STAIRS DOES NOT MEET THE 1/3RD OF THE LENGTH OF THE
 MAXIMUM OVERALL DIAGONAL DIMENSION OF THE AREA SERVED.
 SEE 2010 FBC-B 1015.2.1 EXCEPTION # 2. A THIRD ENCLOSED
 VERTICAL EXIT ENCLOSURE WILL BE REQUIRED TO BE ABLE TO
 MEET THE 1/3RD THE DISTANCE APART ON THE DIAGONAL.

 A COPY OF THE PAID RECEIPT ATTACHED TO THE PERMIT
