 1- PROVIDE ENERGY CALCULATIONS (2 COPIES) AS REQUIRED
 BY SEC. N1100.3. COMPLETE ALL THE INFORMATION ON THE
 FORM AND OWNER/AGENT TO SIGN AND DATE BOTTOM OF FORM
 ALSO.
  
 2- PROVIDE PRODUCT APPROVALS (2 COPIES) AS REQUIRED BY
 DCA RULE 9N-3 FOR:
 A) WINDOWS TO BE INSTALLED.
 B) EXTERIOR DOORS TO BE INSTALLED.
 C) STORM SHUTTERS. IF DOORS AND WINDOWS DON'T HAVE
 IMPACT GLASS. AND PROVIDE SHUTTERS COMPLETE
 INSTALLATION SCHEDULE.
 D) PRECAST LINTELS AS SHOWN ON PLANS.
 E) SHINGLES AS SHOWN ON PLANS.
  
 3- ACCORDING TO SEC. R301.2.1.4.3 THIS BUILDING IS
 LOCATED ON EXPOSURE "C" NOT "B".
 A) REVISE THE DESIGN INFORMATION ON SHEET #1.
 B) REVISE THE COMPONENTS AND CLADDING DESIGN PRESSURES
 SHOWN ON SHEET #1.
  
 4- SPECIFY CARBON MONOXIDE ALARMS ON PLANS AS REQUIRED
 BY SEC. R313.4.
  
 5- SHEET #4: SPECIFY SIZE OF ATTIC SCREEN VENTS ON WALL
 SECTION. SEC. R806.
  
 6- DOOR BUCK DETAIL ON SHEET #4:
 A) REVISE SIZE OF BUCK. NEED TO PROVIDE DOOR FRAME FULL
 SUPPORT. SEE DOOR PRODUCT APPROVAL FOR REQUIRED SIZE.

 A) SPECIFY TYPE AND SIZE OF EXISTING BEAM.
  
 B) PROVIDE COMPLETE SPECIFICATIONS FOR COLUMNS AT BOTH
 ENDS.
  
 10- SHEET #2: THIS IS NOT A LEVEL 2 ALTERATION. THIS IS
 AN ADDITION . REVISE AS REQUIRED.
  
 11- SPECIFY ATTIC SIZE AND LOCATION FOR NEW ADDITION AS
 REQUIRED BY SEC. R807.
  
 12- VALUATION DECLARED IN THE PERMIT APPLICATION IS TOO
 LOW. SEC. 108.3 CITY AMENDMENTS. VALUATION NEED TO
 INCLUDE ALL LABOR, MATERIALS AND DESIGN FEES. VALUATION
 WAS UPDATED OWES $831.32.
