 FLOOR & PENTHOUSE 2 (PH-2) ON THE 15TH FLOOR. IN
 COMMUNITYPLUS UNDER PCN# 74434317050030060, I CREATED
 SEQUENTIAL UNIT RECORDS 0028 - 1601 FORUM PL # PH-1 &
 0029 - 1601 FORUM PL # PH-2. PLEASE MOVE & ATTACH THIS
 PERMIT TO EITHER ONE OF THOSE SEQUENTIAL UNIT RECORDS
 0028 OR 0029, AS I DO NOT HAVE AUTHORIZATION TO
 IMPLEMENT THIS PROCEDURE IN COMMUNITYPLUS.
