 3. THE ADDRESS ON FILE FOR THIS PROPERTY IS 6650 45TH
 STREET, NOT 6246 AS SHOWN ON THE APPLICATION.
  
 4. PROVIDE TWO (2) COPIES OF THE SURVEY REFERENCED IN
 THE DESCRIPTION OF THE LEASE AREA (ADVANCED LAND
 SURVEYING AND MAPPING DATED 10-18-06). IF A MORE RECENT
