 CHECKLIST.
 REVIEW UNDER 2004 FLORIDA RESIDENTIAL CODE WITH 2007
 REVISIONS.
  
 1- NEED RECORDED NOTICE OF COMMENCEMENT. SEC. 713.13
 FLORIDA STATUTE.
 **NOTE: THIS MAY BE RECORDED AT TIME OF PERMIT PICK UP.
  
 2- AFTER PLANS ARE REVISED, BRING THEM TO PALM BEACH
 COUNTY TO CHECK IF IMPACT FEES ARE DUE. BRING BACK
 STAMPED PLANS AND COPY OF PAID RECEIPT IF FEES ARE DUE.
 (REPEAT COMMENT). PLANS SUBMITTED NEED TO HAVE PALM
 BEACH COUNTY IMPACT FEES STAMP.
  
 3- PROVIDE 2 COPIES OF PRODUCT APPROVALS AS REQUIRED BY
 DCA RULE 9B-72 FOR:
 A) STORM SHUTTERS. (REPEAT COMMENT). NONE SUBMITTED.
 IT'S NO CLEAR IF DOORS ARE GOING TO NEED HAVE IMPACT
 GLAZING OR NOT. THERE WAS NO PRODUCT APPROVAL SUBMITTED
 FOR DOUBLE FRENCH DOORS AND PRODUCT APPROVAL SUBMITTED
 FOR SINGLE DOOR (ENTRY?) REQUIRES SHUTTERS. CLARIFY AND
 COORDINATE PRODUCT APPROVALS ACCORDINGLY.
  
 ** IF SHUTTERS ARE REQUIRED, THEN PROVIDE COMPLETE
 INSTALLATION SCHEDULE. (REPEAT COMMENT). SEE ATTACHED
 SAMPLE. AND CLEARLY MARK ON SHUTTERS DRAWINGS SPECIFIC
 MOUNTING CONDITIONS WALL SECTION (EG: WALL MOUNT, BUILD
 OUT, ETC) AND SPECIFIC ANCHOR TYPE AND SPACING TO BE
 USED TO VERIFY COMPLIANCE WITH THE PRODUCT APPROVAL AND
 CALCULATED DESIGN PRESSURES.
  
 B) ENTRY DOOR AS SHOWN ON FLOOR PLANS. (REPEAT
 COMMENT). PRODUCT APPROVAL FL5114 SUBMITTED IS FOR
 GLAZED DOOR. FLOOR PLANS SHOW SOLID DOOR.
 CLARIFY/REVISE AS REQUIRED. THIS GLAZED DOOR REQUIRES
 SHUTTERS.
 COORDINATE WITH ITEM #3A ABOVE.
  
 C) PAIR OF FRENCH DOORS AS SHOWN ON PLANS. NONE
 SUBMITTED AT THIS TIME. (REPEAT COMMENT).
  
 D) FLAT ROOF ASSEMBLY. (REPEAT COMMENT). THERE WAS NOT
 PRODUCT APPROVAL SUBMITTED. COORDINATE WITH ITEM # D(2)

 USED TO FASTEN THE FLAT ROOF BASE SHEET. (REPEAT

 *** ENGINEER'S RAS 117 CALCULATIONS SUBMITTED:
 (1) ROOFING SYSTEM: THERE IS NOT WARRIOR ROOFING
 MANUFACTURER MODIFIED ROOF SYSTEM. SEE PRODUCT APPROVAL
 AND REVISE AS REQUIRED. REFER TO PRODUCT APPROVAL
 SPECIFIC APPROVED ASSEMBLY TO BE INSTALLED AND REVISE
 AS REQUIRED.
  
 (2) PRODUCT APPROVAL: NOA #07-0111.03 WAS SUPERSEDED BY
 NOA 07-0730.03. SEE ATTACHED COPY FROM MIAMI-DADE
 WEBSITE. PROVIDE CURRENT NOA.
  
 (3) WHERE IS THE MAXIMUM DESIGN PRESSURE OF 525 LBS/FT2
 TAKEN FROM?. SEE PRODUCT APPROVAL.
  
 (4) NAIL ENHANCING DESCRIPTION IS NOT CLEAR. PROVIDE
 SKETCH SHOWING NAIL ENHANCING. COORDINATE WITH PRODUCT
 APPROVAL.
  
 (5) CLARIFY WHERE IS THE FM APPROVED, ASTM D-4601, TYPE
 112 AND UL INFORMATION DESCRIBED IN THE LAST PARAGRAPH
 TAKEN FROM? AND COORDINATE INFORMATION WITH PRODUCT
 APPROVAL SUBMITTED.
  
 E) PRECAST LINTELS AS SPECIFIED ON PLANS. (REPEAT
 COMMENT. NOT ADDRESSED).
  
 4- ENGINEER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS AS
 REQUIRED BY SEC. 106.3.3 CITY AMENDMENTS TO FBC 2004.
  
 5- BRING ANY REVISED PLANS FOR HISTORIC REVIEW APPROVAL
 STAMP PRIOR TO SUBMITTAL TO THIS OFFICE.
  
 6- SHEET 4.1:
 A) CLARIFY WHAT'S CONNECTOR #22. SHOWN ON ROOF FRAMING
 PLAN. WHAT'S LUS10 AS SHOWN ON CONNECTOR SCHEDULE?.
 (REPEAT COMMENT. NOT ADDRESSED). UNABLE TO FIND THIS
 CONNECTOR. SEE MANUFACTURER CATALOG AND REVISE AS
 REQUIRED.
  
 B) COMPONENT AND CLADDING PRESSURE ZONES (REPEAT
 COMMENT): REVISE INTERMEDIATE NAILING SPACING ON ROOF
 ZONE 3. NEED TO BE 4"O.C. ALSO, REVISE INTERMEDIATE
 NAILING SPACING FOR ROOF ZONE 1 AND 2 (6"O.C.). SEE
 SEC. R803.2.3.1 OF 2004 FLORIDA RESIDENTIAL CODE AND
 REVISE AS REQUIRED.
  
 C) NEED TO PROVIDE ROOF CROSS VENTILATION AS REQUIRED
 BY SEC. R806 OF 2004 FLORIDA RESIDENTIAL CODE. THERE
 ARE 2 VENTILATION OPENINGS SPECIFIED. THEY DON'T
 PROVIDE ROOF CROSS VENTILATION BECAUSE ROOF FRAMING
 PLANS SHOWS 2X12 SOLID ROOF RAFTERS. (REPEAT COMMENT).
 NOTE ON SHEET 4.1 REGARDING NEW BEAM OPEN TO EXISTING
 RAFTERS TO PROVIDE CROSS VENTILATION DOESN'T RESOLVE
 THE CROSS VENTILATION ISSUE. LOCATION OF VENTS ON NORTH
 AND SOUTH ELEVATIONS ONLY PROVIDE VENTILATION AT THE
 END ROOF RAFTERS. AND THERE IS NOT VENTILATION PROVIDED
 FOR INTERIOR ROOF RAFTERS. SEE SEC. R806.2 FOR
 ALTERNATIVE WAY OF COMPLYING WITH THIS SECTION AND
 REVISE AS REQUIRED. ALSO, USING ICYNENE INSULATION MAY
 BE ANOTHER ALTERNATIVE. IF USING ICYNENE, THEN PROVIDE
 MANUFACTURER SPECIFICATIONS AND COORDINATE WITH ENERGY
 CALCULATIONS.
  
 D) CLARIFY ROOF SCUPPERS AND ROOF DRAINS. (REPEAT
 COMMENT). CLARIFICATION OF SIZES WAS REQUESTED ON
 PREVIOUS CHECKLIST. SIZES WERE REVISED BUT OVERFLOW
 SCUPPER CAN'T BE 2" MAX. ABOVE ROOF DECK AND JUST ABOVE
 ROOF DRAIN IF ROOF DRAIN IS 4"X12". REVISE AS REQUIRED
 TO COMPLY WITH SEC. R903.4 OF 2004 FLORIDA RESIDENTIAL
 CODE.
  
 7- SHEET 5.1:
 A) EXTERIOR DOOR HEAD JAMB DETAIL. REVISE SIZE OF
 STRUCTURAL BUCK. 1X4 STRUCTURAL BUCK IS NOT ADEQUATE TO
 PROVIDE DOOR FRAME FULL SUPPORT AS REQUIRED BY SEC.
 R613.6 OF 2004 FLORIDA RESIDENTIAL CODE. ALSO, SEE
 PRODUCT APPROVAL FOR FRAME SIZE. (REPEAT COMMENT). SIZE
 WAS CHANGED FROM 2X4 TO 1X4. STILL THE WIDTH OF THIS
 BUCK DOESN'T PROVIDE FULL SUPPORT TO THE DOOR FRAME.
 REVISE AS REQUIRED.
  
 B) COMPONENTS AND CLADDING PRESSURES ZONES. CLARIFY WHY
 NAILING SPACING (4" AND 6") IS MORE STRICT FOR WALL
 ZONE 4 THAN WALL ZONE 5 (6" AND 12"). ON THE OTHER
 HAND, WALLS IN THIS PROJECT ARE MASONRY WALLS, NOT WOOD
 FRAMED WALLS.
  
 8- SHEET 6.1, TYPICAL WALL SECTION:
 A) CLARIFY SPECS GIVEN FOR THE FLAT ROOF ASSEMBLY. SEE
 PRODUCT APPROVAL FOR CORRECT INFORMATION. INFORMATION
 SUBMITTED IS FOR PITCHED ROOFS, NOT FOR LOW SLOPE
 ROOFS. REVISE AS REQUIRED. (REPEAT COMMENT).
 UNDERLAYMENT SPECIFICATIONS NEED TO BE COORDINATED WITH
 SELECTED APPROVED FLAT ROOF ASSEMBLY. REVISE
 SPECIFICATIONS AS REQUIRED.
  
 9- REVISE ENERGY CALCULATIONS:
 A) PROVIDE PROJECT ADDRESS AND COMPLETE ALL THE
 INFORMATION ON TOP OF FORM. (REPEAT COMMENT. NOT
 ADDRESSED).
  
 B) ARE THE ENERGY CALCULATIONS FOR THE ENTIRE HOUSE OR
 FOR AREA OF WORK ONLY. IS NEW INSULATION PROVIDED FOR
 THE ENTIRE HOUSE IN WALLS AND CEILINGS. ALSO, SEE TYPE
 OF WALLS ON EXISTING STRUCTURE. THIS INFORMATION IS NOT
 SHOWN ON PLANS. (REPEAT COMMENT. NOT ADDRESSED).
  
 C) PROVIDE COMPLETE MANUFACTURER'S SPECIFICATIONS
 SHOWING THE U-FACTOR AND SHGC FACTOR SHOWN ON ITEMS 7A
 AND 7B WHEN THE DEFAULT FACTOR IS NOT USED. (IN
 ADDITION WINDOWS AND DOORS NEED TO SHOW SUCH RATING FOR
 INSPECTION AFTER INSTALLATION). (REPEAT COMMENT. NOT
